[Study on occurrence of anterior ankle pain after distal tibial extra-articular fractures treated with retrograde tibial intramedullary nail].

OBJECTIVE: To investigate whether retrograde tibial intramedullary nail (RTN) fixation can cause postoperative anterior ankle pain in the treatment of distal tibial extra-articular fracture. METHODS: A retrospective analysis was performed for 29 patients with distal tibial extra-articular fractures caused by trauma, who received RTN treatment between January 2021 and December 2023. There were 19 males and 10 females, with an average age of 47.3 years. There were 18 cases of closed fractures and 11 cases of open fractures. The time from injury to hospital admission was 1-5 days, with an averag

Updated German National S1 Guideline on Pediatric Lower Leg Shaft Fractures.

Fractures of the diaphysis of the lower leg account for approximately 6% of all fractures in children and adolescents. A distinction must be made between isolated tibial shaft fractures, which are primarily managed conservatively, and combined lower leg shaft fractures (involving both the tibia and fibula). The latter are significantly less stable and therefore more frequently treated with osteosynthesis. If the fracture is non-displaced or tolerably displaced, immobilization in a long leg cast for about 4 weeks is sufficient. This requires that no secondary displacement occurs within the cast

Staged Emergency Caesarean Section and Cementless Total Hip Arthroplasty Within a Single Anaesthetic Episode for Pipkin Type III Femoral Head Fracture-Dislocation in a Gravid Patient: A Case Report.

Pipkin type III femoral head fracture-dislocation in a gravid patient is an exceptionally rare clinical entity. We report the case of a woman in her early 40s who presented at 36 weeks of gestation following a high-energy motor vehicle collision. Clinical examination and imaging confirmed a left posterior hip dislocation; computed tomography revealed a severely comminuted Pipkin type III fracture that precluded open reduction and internal fixation. A multidisciplinary team formulated a staged single-anaesthetic operative strategy; emergency caesarean section was performed immediately before ce

Closed reduction and percutaneous screw fixation with early rehabilitation versus cast immobilization in minimally displaced sanders type II-III calcaneal fractures: a two-center retrospective comparison of treatment protocols.

BACKGROUND: Minimally displaced intra-articular calcaneal fractures are commonly managed with cast immobilization, but prolonged immobilization may delay recovery and contribute to hindfoot stiffness. This study compared mid-term clinical and radiographic outcomes of two institutional protocols for minimally displaced Sanders type II-III calcaneal fractures: closed reduction and percutaneous screw fixation with earlier range-of-motion exercises versus cast immobilization. METHODS: This two-center retrospective cohort study included 87 patients treated between January 2018 and February 2024 wi

A case of central migration of a prosthetic femoral head following bipolar hemiarthroplasty treated with conversion to total hip arthroplasty using an acetabular reinforcement ring.

INTRODUCTION: Central migration of the prosthetic femoral head occurs in approximately 7% of cases following bipolar hemiarthroplasty (BHA) for femoral neck fractures. If treated promptly, it can be managed by cup placement; however, if left unaddressed, a large acetabular bone defect can develop. Herein, we report a case of central migration with a severe bone defect after BHA, successfully treated with conversion to total hip arthroplasty (THA) using a Burch-Schneider cage. CASE REPORT: An 86-year-old man underwent BHA at a local hospital for a right femoral neck fracture in Year X-8. He ex

Suture anchor fixation as a viable alternative to tension band wiring for inferior pole patellar fractures: a retrospective comparative study.

BACKGROUND: Inferior pole patellar fractures are difficult to treat because fixation must restore extensor continuity while minimizing hardware-related complications. This study compared the clinical outcomes of tension band wiring (TBW) and suture anchor fixation (SAF). METHODS: We retrospectively reviewed 45 adult patients with inferior pole patellar fractures treated between September 2020 and September 2023. Twenty-nine patients underwent TBW and 16 underwent SAF. The primary outcomes were fracture union time, postoperative complications, maximum knee flexion at 12 months, and Bostman pat

Treatment outcomes and contributing factors in pediatric radial neck fracture.

BACKGROUND: Few studies have investigated the contributing factors for pediatric radial neck fractures. This study aimed to analyze the radiographic and clinical factors that influence the treatment outcomes in pediatric radial neck fracture. METHODS: This retrospective study included the 112 pediatric patients with radial neck fracture. On elbow radiographs, angulation, displacement, and carrying angle were measured, and fractures were classified using the modified Judet classification. Clinical outcomes were assessed using the Flynn criteria based on the elbow ROM and carrying angle. Univar

Capitellum Fractures in the Pediatric Population: A Case Report.

Capitellum fractures are rare injuries in the pediatric population. Their diagnosis is particularly challenging due to the predominantly cartilaginous nature of the pediatric elbow, which often limits the sensitivity of conventional radiographs. Consequently, these fractures are frequently overlooked at initial presentation. We present the case of a nine-year-old boy who sustained indirect trauma to the left elbow after a fall from standing height. On clinical examination, the patient complained of pain, mainly in the lateral aspect of the elbow, with refusal to actively mobilize the joint and

Impact of follow-up radiographs on post-operative management after uncomplicated radius and ulna fracture fixation in small dogs.

AIMS: To evaluate how frequently radiographs, taken during the follow-up period (4-8 weeks) after uncomplicated surgery for the internal fixation of simple transverse or short oblique radius and ulna fractures in small dogs (< 15 kg), resulted in a change to the post-operative management plan, and to evaluate the long-term owner-reported outcomes. METHODS: Medical records of dogs (n = 121) undergoing internal fixation of radius and ulna fractures in a private veterinary referral hospital in Quebec (Canada) were reviewed. Cases with no intra-operative complications, an unremarkable post-operat

Pseudoaneurysm and arteriovenous fistula of the brachial artery presenting 25 years after elbow fracture: a case report.

INTRODUCTION: Post-traumatic pseudoaneurysms (PSAs) are uncommon vascular complications resulting from partial disruption of the arterial wall, leading to a contained hematoma that maintains communication with the arterial lumen. Although most PSAs develop shortly after trauma or vascular intervention, markedly delayed presentations are exceedingly rare and may pose significant diagnostic and therapeutic challenges. When associated with arteriovenous fistula (AVF) formation, these lesions carry an increased risk of progressive enlargement and sudden hemorrhage. CASE PRESENTATION: We report a

Delayed refixation of a lesser tuberosity avulsion fracture in a young male with open growth plate: successful repair four years after missed injury.

BACKGROUND: Isolated avulsion fractures of the humeral lesser tuberosity in adolescents with open growth plates are rare and frequently overlooked due to subtle clinical and radiographic findings. Delayed diagnosis may result in persistent internal rotation weakness and functional impairment. This report aims to highlight the diagnostic challenges and clinical course of a markedly delayed presentation of this uncommon injury. CASE PRESENTATION: A 19-year-old male presented with persistent weakness in shoulder internal rotation. He reported a traumatic shoulder injury at the age of 15 that had

Muscle-sparing dorsal approach for Bennett fracture osteosynthesis.

INTRODUCTION: Fractures of the first metacarpal base with partial involvement of the joint surface, known as Bennett fractures, are unstable due to static and dynamic forces. If closed reduction remains unsuccessful, an open procedure is indicated. The fracture is usually exposed via a radiopalmar approach according to Wagner to the trapeziometacarpal (TMC) joint with detachment of the thenar muscles. An alternative,muscle-sparing dorsal approach to the TMC joint and its long-term results in the treatment of Bennett fractures are described. PATIENTS AND METHODS: Over a period of 15 years, a t

Long-term functional and radiographic outcomes after removal of radial head arthroplasty.

PURPOSE: If reconstruction of the radial head using osteosynthesis is not possible, radial head arthroplasty (RHA) is performed. Despite the increasing use of RHA, little reliable data is available on the functional and clinical outcomes following removal of RHA in a mid- to long-term follow-up. METHODS: This study included 27 patients who underwent removal of RHA between 2010 and 2021. The mean follow-up period was 8.8 years (2.0 to 14.2 years). The time to removal, as well as functional elbow scores (Mayo Elbow Performance Score (MEPS), Oxford Elbow Score (OES) and Disabilities of arm, shou

Effect of prior rotator cuff repair technique and timing on clinical outcomes of reverse shoulder arthroplasty.

AIMS: Reverse shoulder arthroplasty (RSA) after prior rotator cuff repair (RCR) is commonly performed for recurrence of pain and arthritic progression; however, the influence of prior RCR timing and technique on outcomes of RSA has not been adequately explored. Our primary aim was to evaluate the relationship between the postoperative American Shoulder and Elbow Surgeons (ASES) score, whether prior RCR was performed open or arthroscopic, and the time interval between RSA and prior RCR. Secondary outcomes were forward elevation (FE) and external rotation (ER). METHODS: A prospectively collecte
