Port health control.

Almost 950,000 of the 21 million passengers passing through London (Heathrow) Airport in 1973 were seen by the health control unit, which is run by the London Borough of Hillingdon. The unit provides 24-hour medical cover and its responsibilities include x-ray examination for tuberculosis and screening passengers from smallpox-infected areas. It is suggested that, in view of changing epidemiological patterns throughout the world, there is a need to modify existing procedures rather than to abandon them. The development of a follow-up system for tracing passengers at risk and improvements in pr

A socio-epidemiological study of out-patients attending a city tuberculosis clinic in India to judge the place of specialized centres in a tuberculosis control programme.

In India, specialized tuberculosis clinics exist mostly in cities. These clinics treat mainly persons with an awareness of symptoms who present themselves of their own accord. The few persons without symptoms are those who have been advised to have an examination. The urge to attend a specialized centre, presumably motivated by suffering or discomfort from symptoms and by awareness that specialized services exist, does not appear to be strong enough to overcome all the "obstacles" that lie between the patient and the tuberculosis clinic. A distance of 4 miles (6.4 km) or more is a major obstac

A controlled trial of anti-tuberculosis chemotherapy in the early complicated pneumoconiosis of coalworkers.

A controlled trial in coalminers under the age of 50 in South Wales with sputum-negative, early complicated pneumoconiosis was used to compare three regimes: (1) chemotherapy (rest in hospital and 1 g. streptomycin, 200 mg. INH, 10 g. PAS, for 3 months; then 200 mg. INH, 10 g. PAS for 9 months); (2) rest (in hospital for 3 months); and (3) a working group. The initial and three-year radiographs were assessed by two methods in 173 (96%) of the 180 men entering the trial. No beneficial effect of chemotherapy was demonstrated and this factor was not related to the completeness of the chemotherapy

Mobile chest radiography service for general practitioners.

In an eight-year period a mobile general practitioner chest x-ray referral service examined 100,000 patients referred by their family doctors. Of the patients examined, 8% were found to have previously undiagnosed significant thoracic disease. The incidence of pulmonary tuberculosis in need of treatment was 2.7 per thousand examinations (55% of them infectious). A total of 590 cases of primary cancer of lung were found. The incidence of this disease in men aged 45 and over was 19.4 per thousand examinations. A survey of general practitioners in the area concerned showed that they welcomed the

Some aspects of laboratory investigations in a mass campaign against tuberculosis.

In a study of some of the difficulties attending the bacteriological examination of sputa in mass campaigns against tuberculosis, the authors show that culture of specimens always yields significantly more positive results than routine microscopy alone and that sputum culture is more efficient than laryngeal-swab culture. The effect and occurrence of contamination in cultures were investigated in a special trial, and it was found that even minimal contamination adversely affects mycobacterial growth and is sufficient grounds for discarding contaminated cultures. Because of the anticipated detr
