Mixed adenoneuroendocrine carcinoma (MANEC) of the ampulla of Vater with sarcomatosis: a case report.

INTRODUCTION: Biliary mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) are rare, accounting for only 4-24% of all intestinal MiNENs. Diagnosing them preoperatively remains challenging due to the limitations of biopsy and imaging. As a result, the optimal treatment strategy, particularly for postoperative management, is still poorly defined. CASE PRESENTATION: A 61-year-old woman presented to the Surgical Department of Sfax General Hospital on 23 December 2021, with a 1-month history of jaundice, yellow discoloration of the eyes and urine, and clay-colored stools. Evaluation of Extrahepatic Bile Duct Resection as a Parenchyma-Preserving Alternative in Bismuth I-II Perihilar Cholangiocarcinoma: A Comparative Single-Center Analysis.

INTRODUCTION: Perihilar cholangiocarcinoma (pCCA) is an aggressive biliary malignancy usually requiring major hepatectomy, which carries substantial morbidity and mortality. In selected patients with Bismuth-Corlette Type I-II tumors, isolated extrahepatic bile duct resection (EH) may represent a parenchyma-preserving alternative. However, comparative outcome data remain limited. This study evaluated perioperative and long-term outcomes following EH and major hepatectomy (MH). METHODS: Of 305 consecutive patients undergoing surgery for perihilar cholangiocarcinoma between 2010 and 2024, 49 wi

Duodenal atresia associated with apple-peel configuration of distal small bowel in a late preterm neonate: a rare case report and literature review.

INTRODUCTION: The association of duodenal atresia with apple-peel (Christmas tree) configuration of the distal small bowel is exceptionally rare, with only a few cases reported in the literature. This unusual combination is of particular interest because it may provide evidence supporting a vascular etiology for selected cases of duodenal atresia. We report a neonate with type III duodenal atresia associated with apple-peel bowel and absent proximal superior mesenteric artery anatomy. CASE PRESENTATION: A late-preterm (35 + 5 weeks) female neonate with antenatal suspicion of jejunal atresia p

A novel variant of pancreaticobiliary maljunction: Complete opening of the common bile duct into the duct of Santorini in a patient with pancreas divisum.

Pancreaticobiliary maljunction (PBM) is a rare congenital anomaly in which the pancreatic and bile ducts unite outside the duodenal wall, forming an abnormally long common channel that permits reciprocal reflux of pancreatic juice and bile. This leads to a high risk of recurrent pancreatitis, cholangitis, biliary stone formation, and biliary tract malignancy. Type D PBM, particularly when associated with complete pancreas divisum and aberrant bile duct drainage, represents an exceptionally rare and complex variant. We report the case of a 20-year-old female with a long-standing history of recu

Serial carbohydrate antigen 19-9 monitoring reflects occult biliary abnormalities after surgery for choledochal cyst.

BACKGROUND: Carbohydrate antigen 19-9 is widely used as a pancreatobiliary biomarker; however, its value after choledochal cyst surgery remains unclear. Because long-term benign biliary complications predominantly occur after surgery for choledochal cyst requiring extrahepatic bile duct excision, we investigated the longitudinal behavior of carbohydrate antigen 19-9 in this patient population. We evaluated whether carbohydrate antigen 19-9 levels predict postoperative biliary complications or reflect concurrent biliary abnormalities during follow-up. METHODS: We retrospectively analyzed 233 p

Jejunal Loop-Preserving and Mesentery-Sparing Reconstruction after Pancreaticoduodenectomy in a Patient with Prior Total Gastrectomy.

INTRODUCTION: Pancreaticoduodenectomy (PD) after a total gastrectomy is technically challenging because the proximal jejunum has already been utilized for an esophagojejunostomy. Consequently, reconstruction after PD requires the use of a more distal jejunal segment, and the optimal reconstruction strategy remains controversial. We report a case of a jejunal loop-preserving and mesentery-sparing reconstruction after PD in a patient with a prior total gastrectomy. CASE PRESENTATION: A 70-year-old man with a history of Roux-en-Y total gastrectomy for gastric cancer presented with obstructive ja

Isolated extreme gamma-glutamyl transferase elevation as an early indicator of ampullary adenocarcinoma in a young adult: a case report with molecular insights.

BACKGROUND: Ampullary adenocarcinoma is a rare gastrointestinal malignancy that typically occurs in older populations and characteristically presents with obstructive jaundice. Atypical biochemical presentations in young adults without hyperbilirubinemia are extremely rare and can lead to significant diagnostic delays. CASE PRESENTATION: A 37-year-old asymptomatic male presented with a rapid, isolated, and extreme elevation of serum gamma-glutamyl transferase (GGT), surging from 78 U/L to 1793 U/L over 6 months, and an elevated CA19-9 level. Total bilirubin, transaminases, and baseline liver

Necrotizing Pancreatitis Associated With an Ingested Screw Adjacent to the Ampulla of Vater.

A 78-year-old man presented with severe epigastric pain and was diagnosed with acute pancreatitis that progressed to necrotizing pancreatitis despite supportive management. No common etiologies were identified. During endoscopic nasojejunal feeding tube placement, a metallic screw was incidentally discovered in the second portion of the duodenum adjacent to the major papilla and successfully removed. The foreign body had not been detected on previous computed tomography, ultrasound, or abdominal radiography. The patient recovered completely without recurrence. This case highlights a rare peria

Serological Screening of Coxiella burnetii Phase II IgG Antibodies for the First Trimester of Ante-natal Women Using ELISA and Immunofluorescent Assay (IFA).

OBJECTIVES: Both liver fibrosis and osteoporosis share inflammatory pathways, with liver fibrosis potentially contributing to decreased bone mineral density (BMD). The rising prevalence of non-alcoholic fatty liver disease (NAFLD) and associated liver fibrosis, especially in older populations, may increase the risk of osteoporosis, but evidence remains inconclusive. This study aims to investigate the relationship between liver fibrosis and osteoporosis in individuals over 50 years old. METHODS: This cross-sectional study used data from the Bushehr Elderly Health Program (BEHP), a cohort of 2,

Correlation Between Different Shapes of Major Papilla With Outcomes of Endoscopic Retrograde Cholangiopancreatography in a Prospective Study.

INTRODUCTION: Several factors affect the difficulty, clinical success, and side effects related to endoscopic retrograde cholangiopancreatography (ERCP), of which the endoscopic appearance of the major papilla is one of the most important. This study aimed to investigate the outcomes related to ERCP in relation to major papilla morphology. METHODS: This study was performed prospectively on 581 patients undergoing ERCP. During ERCP, the endoscopist determined the morphology of the major papilla before the cannulation. Primary outcomes included overall operative success rate and post-ERCP pancr

Endoscopic retrograde cholangiopancreatography resection of a neuroendocrine tumor in the papilla of Vater.

Neuroendocrine tumors (NETs) of the Papilla of vater are rare. We present the case of a 48-year-old male who was referred after a papillary mass was detected in an upper GI endoscopy. MRCP showed bile duct dilation and a 9 mm nodule in the papilla of vater. Endoscopic ultrasonography identified a hypoechoic lesion. Snare papillary resection of the enlarged papilla was performed. The snare tip was positioned at the tumor apex, gradually expanded to encircle the base, and tightened to ensure complete tumor inclusion. A papillectomy was performed, followed by pancreatic duct stenting after guidew

'Posterior (SMA-first) Approach' Versus 'Standard Approach' in Terms of Resection Margin and Long-Term Survival After Pancreaticoduodenectomy for Periampullary and Pancreatic Carcinoma: A Randomized Trial.

INTRODUCTION: SMA-first-approach in pancreaticooduodenectomy (PD) was found to be associated with better oncological outcomes as compared to the standard approach both in our pilot study and a few retrospective studies. Hence, we performed a randomized controlled trial to compare these two approaches in terms of resection margins, lymph node-retrieval, and long-term survival. METHODS: Between January 2014 to December 2018, all the consecutive patients undergoing PD for periampullary cancer at the Department of Gastrointestinal Surgery, AIIMS, New Delhi were assessed for randomization. Difficulty in cannulation and risk of complications after endoscopic retrograde cholangiopancreatography according to the type of papilla of Vater in a tertiary center.

INTRODUCTION: cannulation of the papilla of Vater is a critical step in endoscopic retrograde cholangiopancreatography (ERCP), with direct implications for procedural success and the risk of adverse events. Although endoscopist experience and anatomical variations are known contributors, the role of papillary morphology remains insufficiently explored in Latin America. This study aimed to assess the association between papilla type and cannulation difficulty or complications. MATERIALS AND METHODS: adults with a native major papilla undergoing ERCP at a tertiary center in Bogotá were prospect

Data augmentation method for computer-aided diagnosis using specular reflection.

UNLABELLED: Drug-resistant Plasmodium falciparum has been on the rise, mainly due to point mutations in the pfDHFR or dihydrofolate reductase gene. This has heightened urgency to discover and develop new antimalarial drugs. This research utilized Pharmacophore Modeling, Virtual Screening, Molecular Docking and Molecular Dynamics Simulation techniques to identify potential pfDHFR inhibitors from virtual databases. Ligand-based pharmacophore hypothesis (H1) (r = 0.949, r2 = 0.883) was formulated using a training set of 28 compounds that demonstrated pfDHFR inhibitory activity across four orders

Advanced ampullary cancer: post-hoc analysis of the ABC-01, ABC-02, and ABC-03 clinical trials.

Ampullary carcinoma (AC) is a rare malignancy. It is often classified within biliary tract cancers (BTC) but lacks dedicated treatment guidelines. This post-hoc analysis evaluated outcomes of patients with advanced AC enrolled in the ABC-01, ABC-02, and ABC-03 clinical trials to provide reference data for future studies. All patients with advanced AC formed the "Descriptive cohort," while those AC treated with cisplatin-gemcitabine (CisGem) comprised the "CisGem-treated cohort." Among 534 trial participants, 28 (5.24%) had AC, and 17 received CisGem. The median age was 63.93 years, and 75.00%

The Risk of Developing Periampullary Tumors After Cholecystectomy: A Systematic Review and Meta-analysis.

BACKGROUND: The link between cholecystectomy and the risk of periampullary tumors (PTs) is uncertain. The purpose of this systematic review and meta-analysis was to examine the available evidence on this association. METHODS: A systematic literature search was conducted in PubMed, Embase, and Web of Science for relevant studies published between 1996 and 2024. We considered studies that reported relative risks (RRs) for PTs after cholecystectomy. The Newcastle-Ottawa Scale was used to assess the risk of bias. RESULTS: The analysis includes 5 studies (n=107,476). A forest plot of individual s

AI-powered hierarchical classification of ampullary neoplasms: a deep learning approach using white-light and narrow-band imaging.

BACKGROUND: Endoscopic diagnosis of Ampulla of Vater (AoV) lesions remains challenging owing to complex morphology and limited representative images, particularly for high-risk dysplastic lesions. This study aimed to develop a hierarchical deep learning framework for the stepwise classification of ampullary lesions using white-light (WL) and narrow-band endoscopic images (NBI). METHODS: The framework employs three sequential binary classifications: (1) normal vs. abnormal, (2) adenoma vs. cancer, and (3) high-grade dysplasia (HGD) vs. low-grade dysplasia (LGD) within adenomas. Each stage uses
