Ampullary Neuroendocrine Carcinoma Presenting with Clinically Confirmed Endogenous Hyperinsulinemic Hypoglycemia.

A 65-year-old man without diabetes presented with fasting hypoglycemia. During a fasting test, plasma glucose decreased to 46 mg/dL with inappropriately elevated insulin (11.1 μU/mL) and C-peptide (8.7 ng/mL) levels, confirming endogenous hyperinsulinemic hypoglycemia. Imaging revealed a tumor of the ampulla of Vater with multiple liver metastases but no pancreatic lesions. An endoscopic biopsy revealed poorly differentiated small cell-type NEC with a Ki-67 index >90% and complete loss of RB1 expression. Despite negative insulin immunostaining, dynamic endocrine testing confirmed endogenous hy

Survival impact of involvement of lymph node stations 16b1, 9, and 8a during pancreatoduodenectomy for pancreatic and periampullary cancers (PANODE): prospective multicentre study.

BACKGROUND: Current guidelines do not recommend routine sampling of lymph node stations (LNS) 16b1, 9, and 8a during pancreatoduodenectomy for pancreatic ductal adenocarcinoma (PDAC) and other periampullary cancers because prospective multicentre studies are lacking. This study evaluated the association between the involvement of these LNS and overall survival (OS). METHODS: This prospective multicentre study included patients undergoing pancreatoduodenectomy for PDAC, distal cholangiocarcinoma, ampullary cancer, or duodenal cancer between October 2015 and April 2020. Associations between inv

Tumor-related local complications as a major contributor to mortality after definitive radiotherapy for unresectable extrahepatic bile duct cancer.

PURPOSE: This study investigated oncologic outcomes and causes of death after radiotherapy in patients with unresectable extrahepatic bile duct cancer. METHODS: We retrospectively analyzed 74 patients with localized unresectable or inoperable extrahepatic bile duct cancer treated with conventionally fractionated radiotherapy between September 2005 and February 2019. Fifty-five patients (74.3%) underwent three-dimensional conformal radiotherapy, and 19 patients (25.7%) received intensity-modulated radiotherapy. Induction chemotherapy was administered in 18 patients and 57 patients received con

Sporadic ampullary adenoma mimicking adenocarcinoma in the absence of genetic predisposition: a case report.

Ampullary tumors are uncommon and are frequently associated with hereditary cancer syndromes. Sporadic ampullary adenomas are infrequently reported and may mimic adenocarcinoma clinically and radiographically. We present a 71-year-old male with obstructive jaundice and imaging concerning for malignancy who was found to have a high-grade ampullary adenoma following attempted pancreaticoduodenectomy. Operative management required modification of the standard approach due to intraoperative anatomical distortions. This case highlights the diagnostic challenges in differentiating benign from malign

Q fever with biliary dilatation following contact with muntjac deer: a case report.

Q fever is a zoonosis caused by Coxiella burnetii. Classic presentations include atypical pneumonia and hepatitis, but bile duct dilation on imaging is rarely described. We report a case of acute Q fever in an urban resident of Chengdu, Sichuan Province, Southwestern China, who presented with cholestatic liver injury and intra- and extrahepatic bile duct dilation, highlighting an unusual manifestation that mimicked biliary tract disease. A 53-year-old man was admitted with persistent high fever (up to 41.8 °C), headache, chills, and right upper quadrant pain. Laboratory tests showed cholestati

Mixed adenoneuroendocrine carcinoma (MANEC) of the ampulla of Vater with sarcomatosis: a case report.

INTRODUCTION: Biliary mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) are rare, accounting for only 4-24% of all intestinal MiNENs. Diagnosing them preoperatively remains challenging due to the limitations of biopsy and imaging. As a result, the optimal treatment strategy, particularly for postoperative management, is still poorly defined. CASE PRESENTATION: A 61-year-old woman presented to the Surgical Department of Sfax General Hospital on 23 December 2021, with a 1-month history of jaundice, yellow discoloration of the eyes and urine, and clay-colored stools. Contrast-enhanced C

Evaluation of Extrahepatic Bile Duct Resection as a Parenchyma-Preserving Alternative in Bismuth I-II Perihilar Cholangiocarcinoma: A Comparative Single-Center Analysis.

INTRODUCTION: Perihilar cholangiocarcinoma (pCCA) is an aggressive biliary malignancy usually requiring major hepatectomy, which carries substantial morbidity and mortality. In selected patients with Bismuth-Corlette Type I-II tumors, isolated extrahepatic bile duct resection (EH) may represent a parenchyma-preserving alternative. However, comparative outcome data remain limited. This study evaluated perioperative and long-term outcomes following EH and major hepatectomy (MH). METHODS: Of 305 consecutive patients undergoing surgery for perihilar cholangiocarcinoma between 2010 and 2024, 49 wi

Duodenal atresia associated with apple-peel configuration of distal small bowel in a late preterm neonate: a rare case report and literature review.

INTRODUCTION: The association of duodenal atresia with apple-peel (Christmas tree) configuration of the distal small bowel is exceptionally rare, with only a few cases reported in the literature. This unusual combination is of particular interest because it may provide evidence supporting a vascular etiology for selected cases of duodenal atresia. We report a neonate with type III duodenal atresia associated with apple-peel bowel and absent proximal superior mesenteric artery anatomy. CASE PRESENTATION: A late-preterm (35 + 5 weeks) female neonate with antenatal suspicion of jejunal atresia p

A novel variant of pancreaticobiliary maljunction: Complete opening of the common bile duct into the duct of Santorini in a patient with pancreas divisum.

Pancreaticobiliary maljunction (PBM) is a rare congenital anomaly in which the pancreatic and bile ducts unite outside the duodenal wall, forming an abnormally long common channel that permits reciprocal reflux of pancreatic juice and bile. This leads to a high risk of recurrent pancreatitis, cholangitis, biliary stone formation, and biliary tract malignancy. Type D PBM, particularly when associated with complete pancreas divisum and aberrant bile duct drainage, represents an exceptionally rare and complex variant. We report the case of a 20-year-old female with a long-standing history of recu

Serial carbohydrate antigen 19-9 monitoring reflects occult biliary abnormalities after surgery for choledochal cyst.

BACKGROUND: Carbohydrate antigen 19-9 is widely used as a pancreatobiliary biomarker; however, its value after choledochal cyst surgery remains unclear. Because long-term benign biliary complications predominantly occur after surgery for choledochal cyst requiring extrahepatic bile duct excision, we investigated the longitudinal behavior of carbohydrate antigen 19-9 in this patient population. We evaluated whether carbohydrate antigen 19-9 levels predict postoperative biliary complications or reflect concurrent biliary abnormalities during follow-up. METHODS: We retrospectively analyzed 233 p

Jejunal Loop-Preserving and Mesentery-Sparing Reconstruction after Pancreaticoduodenectomy in a Patient with Prior Total Gastrectomy.

INTRODUCTION: Pancreaticoduodenectomy (PD) after a total gastrectomy is technically challenging because the proximal jejunum has already been utilized for an esophagojejunostomy. Consequently, reconstruction after PD requires the use of a more distal jejunal segment, and the optimal reconstruction strategy remains controversial. We report a case of a jejunal loop-preserving and mesentery-sparing reconstruction after PD in a patient with a prior total gastrectomy. CASE PRESENTATION: A 70-year-old man with a history of Roux-en-Y total gastrectomy for gastric cancer presented with obstructive ja

Endoscopic submucosal dissection versus endoscopic papillectomy for ampullary tumors with a lesion base ≥20 mm: a single-center retrospective comparative study (with videos).

BACKGROUND AND AIMS: Optimal endoscopic management of ampullary tumors with a large base (≥20 mm) remains unclear. We compared the outcomes of endoscopic submucosal dissection (ESD) versus endoscopic papillectomy (EP). METHODS: This retrospective study included consecutive patients undergoing ESD or EP for ≥20-mm base ampullary tumors (March 2004-April 2026). Inclusion required adenoma or mucosal carcinoma and intraductal extension ≤10 mm. After December 2020, ESD was prioritized for lesions mechanically unsuitable for en bloc snaring. Primary outcomes were en bloc and complete (margin-negati

Isolated extreme gamma-glutamyl transferase elevation as an early indicator of ampullary adenocarcinoma in a young adult: a case report with molecular insights.

BACKGROUND: Ampullary adenocarcinoma is a rare gastrointestinal malignancy that typically occurs in older populations and characteristically presents with obstructive jaundice. Atypical biochemical presentations in young adults without hyperbilirubinemia are extremely rare and can lead to significant diagnostic delays. CASE PRESENTATION: A 37-year-old asymptomatic male presented with a rapid, isolated, and extreme elevation of serum gamma-glutamyl transferase (GGT), surging from 78 U/L to 1793 U/L over 6 months, and an elevated CA19-9 level. Necrotizing Pancreatitis Associated With an Ingested Screw Adjacent to the Ampulla of Vater.

A 78-year-old man presented with severe epigastric pain and was diagnosed with acute pancreatitis that progressed to necrotizing pancreatitis despite supportive management. No common etiologies were identified. During endoscopic nasojejunal feeding tube placement, a metallic screw was incidentally discovered in the second portion of the duodenum adjacent to the major papilla and successfully removed. The foreign body had not been detected on previous computed tomography, ultrasound, or abdominal radiography. The patient recovered completely without recurrence. This case highlights a rare peria

Efficient purification and biochemical characterization of a thermostable and organic solvent-stable α-amylase from Bacillus licheniformis SO7.

UNLABELLED: Upper extremity deep vein thrombosis (UEDVT) is relatively rare, and much less as an initial presentation of systemic lupus erythematosus (SLE). Primary UEDVT should be considered in individuals with unilateral arm swelling where the brachial, axillary, and subclavian veins are frequently involved. SLE is a chronic autoimmune disease that predominantly affects women of childbearing age and of African descent. Patients present with clinical features ranging from arthritis and arthralgias (over 90% of patients with SLE) to life-threatening hematologic, or central nervous system invol

Serological Screening of Coxiella burnetii Phase II IgG Antibodies for the First Trimester of Ante-natal Women Using ELISA and Immunofluorescent Assay (IFA).

OBJECTIVES: Both liver fibrosis and osteoporosis share inflammatory pathways, with liver fibrosis potentially contributing to decreased bone mineral density (BMD). The rising prevalence of non-alcoholic fatty liver disease (NAFLD) and associated liver fibrosis, especially in older populations, may increase the risk of osteoporosis, but evidence remains inconclusive. This study aims to investigate the relationship between liver fibrosis and osteoporosis in individuals over 50 years old. METHODS: This cross-sectional study used data from the Bushehr Elderly Health Program (BEHP), a cohort of 2,

Exploring the Role of MET Gene as a Potential Biomarker and Therapeutic Target in Ampullary Cancer.

BACKGROUND: The incidence of Ampullary carcinoma (AC) has dramatically increased. Nearly 50% of patients develop recurrence indicating need for additional prognostic markers and treatment options. Emerging evidence suggests that Mesenchymal Epithelial Transition (MET) genes play a role in tumorigenesis and can be a useful therapeutic target, however, its role in AC remains unexplored. METHODS: This study investigated the methylation status using methylation specific PCR, mRNA expression using quantitative PCR and protein expression using Immunohistochemistry of the MET proto-oncogene in 61 su

Acute pancreatitis associated with periampullary hamartomatous polyp in Peutz-Jeghers syndrome.

We report the case of an 11-year-old girl with Peutz-Jeghers syndrome (PJS) and a history of bilateral oophorectomy for a sex cord-stromal tumor, who presented with acute abdominal pain, nausea, and vomiting. Laboratory findings indicated acute pancreatitis, and imaging revealed pancreatic enlargement with a large lobulated polypoid lesion (6.2 × 3.8 cm) in the periampullary duodenum, causing biliary and pancreatic ductal dilatation without gallstones. Upper endoscopy confirmed the lesion, and surgical duodenotomy with polypectomy and cholecystectomy was performed. Histopathological analysis i

Correlation Between Different Shapes of Major Papilla With Outcomes of Endoscopic Retrograde Cholangiopancreatography in a Prospective Study.

INTRODUCTION: Several factors affect the difficulty, clinical success, and side effects related to endoscopic retrograde cholangiopancreatography (ERCP), of which the endoscopic appearance of the major papilla is one of the most important. This study aimed to investigate the outcomes related to ERCP in relation to major papilla morphology. METHODS: This study was performed prospectively on 581 patients undergoing ERCP. During ERCP, the endoscopist determined the morphology of the major papilla before the cannulation. Primary outcomes included overall operative success rate and post-ERCP pancr
