Endoscopic retrograde cholangiopancreatography resection of a neuroendocrine tumor in the papilla of Vater.

Neuroendocrine tumors (NETs) of the Papilla of vater are rare. We present the case of a 48-year-old male who was referred after a papillary mass was detected in an upper GI endoscopy. MRCP showed bile duct dilation and a 9 mm nodule in the papilla of vater. Endoscopic ultrasonography identified a hypoechoic lesion. Snare papillary resection of the enlarged papilla was performed. The snare tip was positioned at the tumor apex, gradually expanded to encircle the base, and tightened to ensure complete tumor inclusion. A papillectomy was performed, followed by pancreatic duct stenting after guidew

'Posterior (SMA-first) Approach' Versus 'Standard Approach' in Terms of Resection Margin and Long-Term Survival After Pancreaticoduodenectomy for Periampullary and Pancreatic Carcinoma: A Randomized Trial.

INTRODUCTION: SMA-first-approach in pancreaticooduodenectomy (PD) was found to be associated with better oncological outcomes as compared to the standard approach both in our pilot study and a few retrospective studies. Hence, we performed a randomized controlled trial to compare these two approaches in terms of resection margins, lymph node-retrieval, and long-term survival. METHODS: Between January 2014 to December 2018, all the consecutive patients undergoing PD for periampullary cancer at the Department of Gastrointestinal Surgery, AIIMS, New Delhi were assessed for randomization. Primary

Acute Hemiparesis as the Initial Manifestation Of Chiari I-Associated Syringomyelia: A Stroke Mimic.

INTRODUCTION: Syringomyelia is a rare neurological disorder characterised by the formation of a fluid-filled cystic cavity (syrinx) within the spinal cord, most commonly resulting from disruption of cerebrospinal fluid flow. It typically presents as slowly progressive dissociated sensory loss in a cape-like distribution involving bilateral shoulders, upper back, neck and arms, and is often accompanied by muscle weakness. CASE DESCRIPTION: A 44-year-old female presented with sudden-onset posterior headache followed by right-sided blurred vision, right-sided hemiparesis along with numbness and

Difficulty in cannulation and risk of complications after endoscopic retrograde cholangiopancreatography according to the type of papilla of Vater in a tertiary center.

INTRODUCTION: cannulation of the papilla of Vater is a critical step in endoscopic retrograde cholangiopancreatography (ERCP), with direct implications for procedural success and the risk of adverse events. Although endoscopist experience and anatomical variations are known contributors, the role of papillary morphology remains insufficiently explored in Latin America. This study aimed to assess the association between papilla type and cannulation difficulty or complications. MATERIALS AND METHODS: adults with a native major papilla undergoing ERCP at a tertiary center in Bogotá were prospect

Data augmentation method for computer-aided diagnosis using specular reflection.

UNLABELLED: Drug-resistant Plasmodium falciparum has been on the rise, mainly due to point mutations in the pfDHFR or dihydrofolate reductase gene. This has heightened urgency to discover and develop new antimalarial drugs. This research utilized Pharmacophore Modeling, Virtual Screening, Molecular Docking and Molecular Dynamics Simulation techniques to identify potential pfDHFR inhibitors from virtual databases. Ligand-based pharmacophore hypothesis (H1) (r = 0.949, r2 = 0.883) was formulated using a training set of 28 compounds that demonstrated pfDHFR inhibitory activity across four orders

Advanced ampullary cancer: post-hoc analysis of the ABC-01, ABC-02, and ABC-03 clinical trials.

Ampullary carcinoma (AC) is a rare malignancy. It is often classified within biliary tract cancers (BTC) but lacks dedicated treatment guidelines. This post-hoc analysis evaluated outcomes of patients with advanced AC enrolled in the ABC-01, ABC-02, and ABC-03 clinical trials to provide reference data for future studies. All patients with advanced AC formed the "Descriptive cohort," while those AC treated with cisplatin-gemcitabine (CisGem) comprised the "CisGem-treated cohort." Among 534 trial participants, 28 (5.24%) had AC, and 17 received CisGem. The median age was 63.93 years, and 75.00%

The Risk of Developing Periampullary Tumors After Cholecystectomy: A Systematic Review and Meta-analysis.

BACKGROUND: The link between cholecystectomy and the risk of periampullary tumors (PTs) is uncertain. The purpose of this systematic review and meta-analysis was to examine the available evidence on this association. METHODS: A systematic literature search was conducted in PubMed, Embase, and Web of Science for relevant studies published between 1996 and 2024. We considered studies that reported relative risks (RRs) for PTs after cholecystectomy. The Newcastle-Ottawa Scale was used to assess the risk of bias. RESULTS: The analysis includes 5 studies (n=107,476). A forest plot of individual s

AI-powered hierarchical classification of ampullary neoplasms: a deep learning approach using white-light and narrow-band imaging.

BACKGROUND: Endoscopic diagnosis of Ampulla of Vater (AoV) lesions remains challenging owing to complex morphology and limited representative images, particularly for high-risk dysplastic lesions. This study aimed to develop a hierarchical deep learning framework for the stepwise classification of ampullary lesions using white-light (WL) and narrow-band endoscopic images (NBI). METHODS: The framework employs three sequential binary classifications: (1) normal vs. abnormal, (2) adenoma vs. cancer, and (3) high-grade dysplasia (HGD) vs. low-grade dysplasia (LGD) within adenomas. Each stage uses

Gastroduodenal Intussusception With Gastric Outlet Obstruction and Periampullary Compression Due to a Gastrointestinal Stromal Tumor in the Stomach.

BACKGROUND Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract. Their presentation varies from bleeding to perforation, but they rarely manifest as gastroduodenal intussusception or obstructive jaundice. We report a rare case of a gastric GIST that caused intussusception leading to gastric outlet obstruction with biliary obstruction and acute pancreatitis. CASE REPORT A 71-year-old woman presented with melena, vomiting, and generalized weakness. Evaluation revealed severe anemia and elevated liver and pancreatic enzymes. Imaging and e

A Case of Severe Acute Pancreatitis Following Endoscopic Biopsy of the Ampulla of Vater: A Rare Adverse Event of Esophagogastroduodenoscopy.

Histological biopsy is essential for diagnosing ampullary tumors; however, it can occasionally result in severe adverse events. A 49-year-old male underwent esophagogastroduodenoscopic screening, which revealed an ampulla of Vater with enlargement of the oral protrusion. An endoscopic biopsy was performed; several hours later, the patient developed severe acute pancreatitis requiring hospitalization. The biopsy result was benign, and no gallstones, ductal abnormalities, or other etiologies were identified on endoscopic ultrasonography or magnetic resonance cholangiopancreatography, and the bio

Association of computed tomography-derived body composition with surgical and oncologic outcomes in periampullary adenocarcinoma.

BACKGROUND: Although the clinical efficacy of body composition assessment has been explored in many other cancer types, few studies have focused on periampullary cancer. Furthermore, despite the global rise in minimally invasive pancreaticoduodenectomy (PD), its safety and feasibility in patients with sarcopenic obesity remain unclear. We aimed to investigate the impact of body composition assessment on outcomes after PD and to evaluate the safety of minimally invasive PD in patients with sarcopenic obesity. METHODS: Between 2015 and 2023, we included patients who underwent PD performed by su

Rare, Yet Targetable: New Perspectives on Ampullary Carcinomas.

Ampullary carcinoma (AC) is a rare gastrointestinal malignancy with dual intestinal and pancreatobiliary differentiation, complicating diagnosis, staging, and treatment. This review synthesizes current epidemiology, pathology, and multi-omic data to outline a pragmatic care pathway: lineage-first at presentation, mutation-fast at progression. Histology remains the primary classifier: the intestinal subtype generally aligns with colorectal regimens, whereas pancreatobiliary and mixed subtypes favor pancreaticobiliary therapy. In selected fit patients, modified FOLFIRINOX may address mixed pheno

Gel Immersion vs. Water Immersion Endoscopic Ultrasound for Visualization of the Duodenal Papilla and Periampullary Lesions: A Dual-Center Retrospective Study With Blinded Evaluation of Archived Videos.

BACKGROUND AND STUDY AIMS: Accurate visualization of the duodenal papilla is essential during endoscopic ultrasound (EUS). Water immersion (WI)-EUS improves papillary visibility by expanding the duodenal lumen, but maintaining sufficient water can be difficult. Gel immersion (GI)-EUS offers more stable visualization, though comparative data remain limited. PATIENTS AND METHODS: This dual-center retrospective study included 200 patients who underwent GI-EUS (n = 101) or WI-EUS (n = 99) for ampullary evaluation. Archived videos were randomized and independently assessed by two blinded pancreato
