Ischemic duodenal injury due to systemic lupus erythematosus: A case report.

BACKGROUND: Systemic lupus erythematosus (SLE) is a chronic autoimmune disease involving multiple organ systems. Lupus enteritis is a rare but potentially life-threatening gastrointestinal manifestation characterized by immune complex-mediated inflammation that may cause edema, ischemia, or intestinal perforation. Because abdominal symptoms often overlap with those of infection or drug toxicity, an accurate diagnosis requires a high index of suspicion supported by characteristic radiologic findings. CASE SUMMARY: A 47-year-old female with a known history of SLE and membranous glomerulonephrit

Adjuvant Chemotherapy After Curative Intent Resection of Ampullary Cancer (ADAPTA): Study Protocol for a Pan-European Prospective Multicenter Double Single Arm Trial.

OBJECTIVES: Ampullary adenocarcinoma (AAC) is rare. Five-year survival rates of 30% to 70% are seen after resection. This broad range of survival could be explained by the morphologic heterogeneity in the 3 subtypes of AAC (pancreatobiliary, intestinal, and mixed subtype), which complicates the prediction of individual prognosis and clinical decision making with regard to adjuvant therapy. To date, there are no prospective studies to elucidate whether adjuvant chemotherapy improves survival in these patients. METHODS: The ADAPTA study is a phase II prospective single-arm multicenter cohort st

Impact of Tumor size in Predicting Outcome in Patients with Ampullary Cancer.

PURPOSE: Periampullary, pancreatic ductal, distal bile duct and duodenal carcinomas have overlapping gross presentation and morphology. Exact site of origin is often difficult to establish conclusively, yet they have differing AJCC TNM staging systems. This makes grossing Whipple’s specimens daunting in centers with limited volume, increasing the chance of inaccurate staging. The aim of this study is to validate an alternative size-based staging system for ampullary cancers, using pancreatic ductal adenocarcinoma staging characteristics, which can simplify pathological assessment of Whipple’s

Unusual Ampullary Presentation of Pediatric Burkitt Lymphoma: Case Report and Literature Review.

INTRODUCTION: Burkitt lymphoma (BL) is a highly aggressive B-cell non-Hodgkin lymphoma that typically presents with abdominal masses; ampullary involvement with obstructive jaundice and gastrointestinal bleeding is exceptionally rare in children. Early biliary decompression can be crucial to permit timely chemotherapy. CASE PRESENTATION: A 14-year-old boy presented with 1 week of worsening abdominal pain, non-bilious vomiting, constipation, jaundice, and melena. Examination showed pallor, icterus, firm hepatomegaly, and a palpable epigastric mass. Laboratory testing revealed cholestatic liver

Complex Hemosuccus Pancreaticus from Dual Pseudoaneurysms: A Rare Case of Gastroduodenal and Inferior Pancreaticoduodenal Artery Involvement.

Hemosuccus pancreaticus (HP) is a rare yet critical cause of upper gastrointestinal bleeding, often challenging to diagnose due to its intermittent presentation. We present a complex case of HP in a 63-year-old woman with hematemesis and melena caused by dual pseudoaneurysms in the gastroduodenal and inferior pancreaticoduodenal arteries. Contrast-enhanced computed tomography (CT) and upper GI endoscopy confirmed active bleeding from the ampulla of Vater. Successful management was achieved with retrograde coil embolization. This case highlights the diagnostic and therapeutic challenges in HP a

Synchronous Intra-ampullary Papillary-Tubular Neoplasm and Gallbladder Cancer.

Intra-ampullary papillary-tubular neoplasms (IAPNs) are rare precursor lesions of the ampulla of Vater, with a high propensity for progression to invasive carcinoma. Synchronous primary malignancies of the biliary tract are uncommon, and their occurrence in the absence of congenital anomalies such as pancreaticobiliary maljunction (PBM) has not been well documented. We report a case of an IAPN coexisting with a synchronous gallbladder carcinoma in a patient without identifiable predisposing anomalies. A 70-year-old woman with a history of interstitial pneumonia, acute pancreatitis, and mixed c

Germline Pathogenic Variants in Homologous Recombination Pathway Genes Are Frequent in Pancreatobiliary Ampullary Carcinoma.

PURPOSE: Ampullary carcinoma (AMPCA) is a rare cancer classified into subtypes depending on the histologic appearance and epithelium of origin. To gain insights into the germline genetic factors driving predisposition to AMPCA, the analysis focused on the role of homologous recombination (HR) genes in its oncogenesis. METHODS: We analyzed germline testing results from 26,159 patients with cancer undergoing clinical tumor-normal sequencing from May 2015 to November 2022, of which 112 individuals had AMPCA. Germline and somatic alteration profiles were analyzed for the different histologic subt

Non-traumatic Intramural Duodenal Hematoma With Acute Pancreatitis in a Nonagenarian: A Case Report.

Intramural duodenal hematoma (IDH) is a relatively rare condition typically managed conservatively. Although IDH frequently causes duodenal obstruction, the development of acute pancreatitis (AP) due to ampulla of Vater compression is uncommon. We report the case of non-traumatic IDH in a 90-year-old man receiving anticoagulant therapy, complicated by AP and persistent duodenal obstruction. Imaging studies revealed an intramural hematoma in the descending portion of the duodenum, without evidence of active bleeding. Despite five weeks of conservative management, the obstruction did not improve

Polypoid and Cystic Presentation of Type 1 Autoimmune Pancreatitis at the Duodenal Papilla: A Diagnostic Pitfall Mimicking Intraductal Papillary Mucinous Neoplasm.

We report a rare case of type 1 autoimmune pancreatitis (AIP) forming a polypoid mass at the major duodenal papilla in continuity with a cystic lesion of the pancreatic head, initially misdiagnosed as an intraductal papillary mucinous neoplasm (IPMN). A man in his 70s underwent pancreaticoduodenectomy after imaging revealed a mural nodule within a cystic lesion. Gross examination revealed a papillary polypoid lesion with continuity to a firm nodule in the pancreatic head. Histologically, the lesion exhibited glandular proliferation with pyloric and Brunner's gland-like differentiation in the f

Molecular Analysis of Biliary Tract Neoplasms Associated with Familial Adenomatous Polyposis.

OBJECTIVES: Familial adenomatous polyposis (FAP) is an inherited disorder characterized by multiple colorectal polyposis and is frequently associated with a variety of extracolonic lesions. However, neoplastic lesions of the biliary tract system, excluding those of the ampulla of Vater, are extremely rare in FAP, and the impact of APC alterations on their tumorigenesis remains unclear. We aimed to clarify the relationship between germline variants and somatic variants in the APC gene in biliary tract neoplasms (BTNs) of FAP patients. METHODS: A total of 115 genetically confirmed FAP cases tre

[Pancreaticoduodenectomy for Ampullary Carcinoma in a Patient with Dynapenia - A Case Report].

A 79-year-old female patient with dynapenia was diagnosed with duodenal ampullary cancer (cT1a, cN0, cM0, cStage Ⅰ) and underwent pancreaticoduodenectomy. Postoperatively, she experienced complications, a decline in her ability to perform activities of daily living and required long-term hospitalization. Dynapenia is defined as an age-related decline in muscle strength that occurs before the loss of muscle mass. In contrast, sarcopenia refers to the age-related loss of muscle or lean mass. Both conditions contribute to the decline of active and healthy aging. Dynapenia plays a significant role

Establishment and characterization of a novel human ampullary carcinoma cell line derived from a Chinese patient.

Ampullary carcinoma is an uncommon malignant neoplasm located in Vater's ampulla, necessitating comprehensive exploration of its pathogenesis and therapeutic alternatives. Currently, the available models of ampullary carcinoma are severely restricted, impeding the advancement of fundamental research. Consequently, further models of ampullary carcinoma are required to enhance pertinent fundamental research. This study reports the development of a novel Chinese-origin human ampullary carcinoma cell line, named DPC-X2, utilizing fresh primary tumor tissue from ampullary carcinoma. DPC-X2 has been

Clip Combined With Rubber Band vs Clip-Assisted Endoscopic Retrograde Cholangiopancreatography for Difficult Biliary Cannulation in Periampullary Diverticulum: A Propensity Score-Matched Analysis.

INTRODUCTION: Successful biliary cannulation is critical to the success of endoscopic retrograde cholangiopancreatography (ERCP). While various devices and auxiliary techniques have been developed to manage difficult cannulation in patients with periampullary diverticulum (PAD), however, comparative studies on different traction methods remain understudied. The aim of this study was to compare the safety and efficacy of clip-assisted ERCP (C-ERCP) vs clip combined with rubber band-assisted ERCP (CRB-ERCP) for difficult biliary cannulation in patients with PAD. METHODS: This study was a single

Ampullary carcinoma detected incidentally during ERCP: a case report and literature review.

Early ampullary tumors are challenging to diagnose due to their complex surrounding structures and deep anatomical location. We report a rare case in which enhanced computed tomography (CT), magnetic resonance imaging (MRI), magnetic resonance cholangiopancreatography (MRCP) and endoscopic ultrasound (EUS) all failed to detect the lesion, with the diagnosis ultimately established through endoscopic retrograde cholangiopancreatography (ERCP). The patient subsequently underwent timely surgical resection. This case highlights the irreplaceable value of ERCP in detecting early and occult ampullary

Robotic versus open pancreatoduodenectomy (SPAIN Trial): protocol for a multicenter randomized controlled trial assessing safety, efficacy and cost-effectiveness.

Minimally invasive pancreatoduodenectomy (PD), particularly through a robotic approach (RPD), is gaining increasing worldwide acceptance. However, high-quality evidence remains limited and very few randomized controlled trials have directly compared RPD with open PD (OPD). The SPAIN Trial (Spanish Pancreatoduodenectomy: Open vs. Robotic Approach) is designed to generate national evidence on the safety, efficacy, and economic value of RPD compared with the open technique. This is a national, multicentre, randomized, prospective, non-inferiority trial comparing RPD and OPD in high-volume Spanish
