Preclinical development of a well-defined semisynthetic glycoconjugate vaccine against Haemophilus influenzae type b.

Vaccines are the most effective method for preventing bacterial infections. Identification of the best hapten is an essential step in the development of third-generation Haemophilus influenzae type b (Hib) conjugate vaccines. A comprehensive understanding of the relationship between Hib oligosaccharide chain length and immunological activities remains elusive. A series of Hib capsular polysaccharide (CPS) oligosaccharide derivatives of different lengths were synthesized to establish carbohydrate structure-function relationships. Glycan microarray screening and immunological evaluation of oligo

Two autopsy cases of necrotizing soft-tissue infection in which the causative organism was inferred by 16S rRNA gene analysis.

Necrotizing soft-tissue infection (NSTI) is a rapidly progressing, life-threatening disease that is difficult to diagnose based on clinical findings alone. NSTI often does not present with pus at the infection site, so it may not be suspected during an autopsy. Consequently, bacterial cultures are not performed, which hinders identification of the causative organism. In this study, we attempted to amplify the 16S ribosomal RNA (16S rRNA) gene (also referred to as 16S rDNA) from blood samples collected during forensic autopsies in two cases where death was suspected to be due to NSTI, as determ

Genome-wide association study of Mycoplasma anserisalpingitidis strains for antibiotic susceptibility.

Mycoplasma anserisalpingitidis is a facultative pathogenic bacterium affecting waterfowl, predominantly geese and sporadically ducks. Understanding the molecular basis of antimicrobial resistance mechanisms is crucial in the preservation of antibiotic efficiency. This study aimed to elucidate the genetic background of antibiotic susceptibility profiles of 110 M. anserisalpingitidis strains against nine antimicrobial agents. Significant associations between k-mers and five (tylvalosin, tilmicosin, enrofloxacin, lincomycin, spectinomycin) of the nine antimicrobial agents were identified by pysee

Advanced Eumycetoma of the Right Foot Resulting in Below-Knee Amputation in Somalia: A Case Report.

INTRODUCTION: Mycetoma is a chronic, progressively destructive neglected tropical disease that presents with localised swelling, multiple sinus tracts and a discharge containing grains, and that may extend into deep tissue and bone. Somalia lies within the recognised mycetoma belt, but the national burden is poorly documented, and few patients reach specialist care early enough for the limb to be saved. CASE PRESENTATION: A 33-year-old man from Qoryooley District, Lower Shabelle, Somalia, presented with a seven-year history of progressive swelling of the right foot. Examination showed massive

Investigation of the cefazolin inoculum effect in blood culture-isolated methicillin-susceptible Staphylococcus aureus strains: A Japanese multicenter study.

BACKGROUND: Cefazolin inoculum effect (CInE) is a microbiological phenomenon where the MIC of cefazolin against methicillin-susceptible Staphylococcus aureus (MSSA) strains increases with higher bacterial volumes. METHOD: We retrospectively investigated the prevalence and characteristics of the CInE among MSSA strains isolated from blood cultures at three Japanese hospitals. The collected isolates were screened for blaZ using PCR, and the cefazolin minimum inhibitory concentration (MIC) for the blaZ-positive MSSA isolates was measured at standard and high inoculum volumes. CInE-positive MSSA

RETINAL DETACHMENT AFTER ENDOGENOUS ENDOPHTHALMITIS: A Multicenter Study.

PURPOSE: To assess factors associated with retinal detachment (RD) in eyes presenting with endogenous endophthalmitis (EE). METHODS: Multicenter retrospective study of individuals with an EE diagnosis from seven tertiary care centers. Demographics, medical history, ocular examination findings, microbial cultures (blood or intraocular), and management strategies were assessed relative to subsequent RD occurring within 6 months of EE diagnosis. Of note, this study did not distinguish between intraocular cultures without growth and eyes that were not sampled for an intraocular organism. RESULTS

Diagnostic Gaps and Management Hurdles: Central Venous Catheter-related Infections in an End-stage Renal Disease Patient in Nigeria.

Vascular access-related infections are a significant cause of morbidity and mortality among patients on maintenance hemodialysis (MHD). In developing countries like Nigeria, the reliance on central venous catheters (CVCs) as a primary vascular access point increases the risk of catheter-related bloodstream infections (CRBSI). This report highlights the clinical management of CRBSI and the systemic constraints faced in resource-limited settings. The index case was a 49-year-old woman with end-stage renal disease (ESRD) secondary to diabetes mellitus, currently on MHD via a CVC, presented with a

Dermatoses in Pregnancy: A Hospital-Based Cross-Sectional Study From Eastern India.

Introduction Numerous skin changes occur in pregnancy, the majority of which are physiological in nature and linked to the hormonal, metabolic, vascular, or immunologic changes. However, some pruritic eruptions cause anxiety during pregnancy and may affect the maternal and fetal outcome. This study aims to evaluate the prevalence, clinical patterns, and association with gravida and trimester of dermatoses among pregnant women attending an antenatal clinic for routine obstetric care. Methods This descriptive cross-sectional study was conducted at the Department of Dermatology, Venereology and L

Neuroimaging Patterns of Recreational Drug Use in the Emergency Setting.

Recreational substance use is a global public health problem associated with preventable morbidity and mortality. Commonly consumed substances include alcohol, cannabinoids, opioids, benzodiazepines, amphetamines, cocaine, and inhalants. Given its prevalence in the emergency department, radiologists can expect to see neuroimaging manifestations of recreational drug use. Central nervous system imaging findings associated with recreational drug use vary widely depending on the drug of choice, the route of consumption, and the presence of impurities. Findings can be acute or chronic and include c

BACKGROUND: Preoperative urinary tract infections in urolithiasis patients represent a significant clinical challenge, with infection rates of 7–28% increasing hospitalization duration and complicating surgical management. Rapid diagnostic platforms are needed to replace traditional culture-based methods that delay targeted therapy. METHODS: This study evaluated 158 urolithiasis patients’ urine specimens using both the HB&L rapid detection system and traditional culture methods. The HB&L system utilized 500 µL of urine incubated for 4.5 h, while conventional culture used 10 µL inoculated onto

Management of Pyogenic Flexor Tenosynovitis.

» Pyogenic flexor tenosynovitis (PFT) is an uncommon closed-space infection of the hand that can result in severe stiffness and other sequela. » When PFT is recognized and treated with antibiotics before advanced disease, surgery is often unnecessary to achieve a cure. » Physical examination features are helpful in differentiating PFT from other finger infections, particularly when considered together in aggregate. » Laboratory values such as systemic inflammatory markers are usually normal in early cases of PFT and are more likely to be elevated in other diagnoses like cellulitis. » Imaging f

Exuberant Dermatitis Herpetiformis With Oral Involvement Mimicking Behçet Disease and Disseminated Herpetic Infection: The Crucial Role of Direct Immunofluorescence.

Dermatitis herpetiformis (DH) is a chronic autoimmune blistering dermatosis associated with gluten-sensitive enteropathy and characterized by intensely pruritic papulovesicular eruptions, typically distributed symmetrically on extensor surfaces. Although DH classically presents with grouped vesicles and excoriated papules, atypical and exuberant manifestations may represent a substantial diagnostic challenge, frequently mimicking infectious, inflammatory, and autoimmune blistering disorders. We report the case of a 30-year-old woman presenting with a one-year history of progressive, intensely

An Atypical Presentation of Ecthyma Gangrenosum in a Patient With Waldenström Macroglobulinemia.

Ecthyma gangrenosum (EG) is a relatively rare cutaneous infection that is most commonly caused by Pseudomonas aeruginosa (P. aeruginosa). EG predominantly affects immunocompromised individuals and can be categorized into bacteremic and non-bacteremic types. Despite this distinction and clinical evidence supporting the existence of the non-bacteremic type of EG, the long-held belief that bacteremia must be present for the diagnosis of EG persists. This report describes a case of a 78-year-old Caucasian male with Waldenström macroglobulinemia (WM) undergoing chemotherapy, who presented with a no

The diagnostic and therapeutic value of FDG PET/CT for infectious diseases in FUO/IUO patients.

BACKGROUND AND OBJECTIVE: The diagnosis of infectious diseases is often a challenging problem faced by clinicians, especially in the case of complex infections manifested as fever of unknown origin or inflammation of unknown origin (FUO/IUO). Currently, 18F-fluorodeoxyglucose (FDG) Positron emission tomography/Computed tomography (PET/CT) has been increasingly used in the inflammatory diseases. This study aimed to explore the diagnostic and therapeutic value of FDG PET/CT for infectious diseases in patients with FUO/IUO. METHODOLOGY: A retrospective analysis was conducted on the clinical and i

Orbital cellulitis with posterior scleritis in a 13-year-old girl: A case report.

RATIONALE: Clinical signs and symptoms of posterior scleritis vary and are easily confused with other diseases. Inflammation spreads anteriorly, involving the upper lid structures, causing lid swelling and simulating cellulitis. However, we report a case of orbital cellulitis with posterior scleritis and complications, with the aim of helping clinicians identify and treat early to prevent irreversible loss of vision. PATIENT CONCERNS: A 13-year-old girl presented with pain in her right eye and an acute deterioration in vision. DIAGNOSES: The case was diagnosed as orbital cellulitis with post
