The Basophilic Fringe: A Case Report of Intestinal Spirochetosis in a Patient With Chronic Diarrhea.

Intestinal spirochetosis is a rare infection of the colonic mucosa that often presents with nonspecific gastrointestinal symptoms despite largely normal endoscopic findings. Diagnosis depends on characteristic histologic features supported by immunohistochemical staining. Here, we present the case of a 57-year-old man with HIV, chronic abdominal pain, bloating, and long-standing watery diarrhea. His colonoscopy was largely unremarkable except for rectal ulcers. Histological examination of random colonic biopsies revealed a basophilic, brush-like fringe along the epithelial surface consistent w

Kaposi Sarcoma in an HIV-Negative Patient Following Temozolomide Chemoradiation Treatment for High-Grade Glioma.

Kaposi sarcoma is a rare vascular neoplasm caused by human herpesvirus 8 (HHV8), commonly associated with human immunodeficiency virus (HIV) infection or iatrogenic immunosuppression. This report describes the case of a 44-year-old male who recently completed chemoradiation therapy for a high-grade midline glioma who presented with a right upper eyelid lesion. Wedge resection of the lesion was performed. Microscopic examination revealed a nodular proliferation of atypical medium-sized spindled cells with numerous vascular spaces and scattered mitotic figures. Immunohistochemical staining confi

Prevalence of major structural congenital abnormalities detected on infant surface examination in Botswana, 2014-2022: a birth outcomes surveillance study.

BACKGROUND: Population-level research focused on major congenital abnormalities (MCAs) in sub-Saharan Africa is needed. The Tsepamo study estimated the prevalence of MCAs evaluable on neonatal surface examinations in Botswana. METHODS: Data were collected from up to 18 government hospitals throughout Botswana. Surface examinations were conducted by trained nurses after birth, and photographs and/or descriptions were used to identify and classify MCAs. Birth defects were categorised as MCAs if they were of clinical, surgical or cosmetic importance. We estimated MCA prevalence for all live and

Neuroimaging Patterns of Recreational Drug Use in the Emergency Setting.

Recreational substance use is a global public health problem associated with preventable morbidity and mortality. Commonly consumed substances include alcohol, cannabinoids, opioids, benzodiazepines, amphetamines, cocaine, and inhalants. Given its prevalence in the emergency department, radiologists can expect to see neuroimaging manifestations of recreational drug use. Central nervous system imaging findings associated with recreational drug use vary widely depending on the drug of choice, the route of consumption, and the presence of impurities. Findings can be acute or chronic and include c

Diagnostic Accuracy of Sonographic Signs of Extrapulmonary Tuberculosis and Treatment Response Monitoring in HIV-Positive and -Negative Populations.

Background Diagnosis and monitoring of extrapulmonary tuberculosis (EPTB) remains challenging. Ultrasound such as the extended focused assessment for HIV-associated tuberculosis (eFASH) protocol might improve diagnosis and monitoring of treatment responses. This study determined the diagnostic accuracy of eFASH for EPTB and its value in monitoring EPTB treatment response compared with clinical signs and symptoms. Methods We performed a post-hoc analysis of a trial assessing eFASH impact on management of adults with suspected EPTB. Participants who had baseline and follow-up ultrasound examinat

Intervention to optimise body mass index in adolescents and address the triple burden of malnutrition-the Ntshembo (Hope) trial in rural and urban South Africa study: a study protocol for a randomised controlled trial.

INTRODUCTION: South Africa faces a complex health burden with burgeoning non-communicable diseases against a background of prevalent infection. The triple burden of malnutrition, comprising undernutrition alongside overweight/obesity and micronutrient deficiencies, is widespread and imposes risks for non-communicable diseases along the life course, especially among adolescent girls. We hypothesise that, by optimising nutrition and body mass index (BMI) of at-risk adolescent girls, we can realise a triple return on investment: improved nutritional status, reduced metabolic risk, and moderated p

Clinical assessment and biomarkers for the detection of bacterial community-acquired pneumonia in children and adults: a protocol for systematic review of diagnostic accuracy.

BACKGROUND: Community-acquired pneumonia (CAP) is defined as an infection of the pulmonary parenchyma acquired in the community and outside the hospital setting. CAP is generally diagnosed based on clinical parameters and chest radiography, with biomarkers increasingly used to support diagnostic decisions. Our review aims to synthesise evidence on the diagnosis of bacterial CAP using clinical assessments and biomarkers and to guide evidence-based national antibiotic guidelines for bacterial CAP in India. Our primary objective is to determine the diagnostic accuracy of clinical criteria (sympto

An Fc receptor and IgA functional signature identifies TB disease in children living with HIV.

BACKGROUND: Tuberculosis (TB) is a leading cause of morbidity and mortality among children living with HIV (CLHIV). TB disease is difficult to detect with current clinical microbiological, immunological, and radiographic tools. Serological assays that determine levels of Mycobacterium tuberculosis reactive antibodies inconsistently identify TB but antibody Fc receptor engagement and effector functions are promising biomarkers of TB disease. The objectives of this study are to identify antigen specific antibody signatures in CLHIV with TB disease that both 1) respond to treatment and 2) disting

Advanced-Stage Anal Squamous Cell Carcinoma Detected During Colonoscopy in a Rural HIV-Positive Patient: A Case Illustrating Fragmentation of Care and Barriers to Screening.

Squamous cell carcinoma (SCC) of the anal canal is an uncommon malignancy with increasing incidence, particularly among people living with human immunodeficiency virus (HIV). In rural populations, limited access to specialized screening and fragmented care contribute to delayed diagnosis and advanced-stage presentation. A 58-year-old male patient with a 25-year history of HIV presented with one month of persistent rectal pain, intermittent bleeding, and mass sensation. Physical examination revealed a 5 × 6 cm ulcerated lesion near the anal margin. The patient reported inconsistent adherence to

Invasive gastric candidiasis presenting as gastric perforation in an immunocompetent adult: a rare case report.

INTRODUCTION: Gastric perforation is most commonly caused by peptic ulcer disease, malignancy, or trauma. Isolated gastric candidiasis leading to perforation is an exceptionally rare event, almost exclusively reported in severely immunocompromised hosts. We present a case of peritonitis secondary to Candida-mediated gastric perforation in a patient with no identifiable immunodeficiency. CASE PRESENTATION: A 25-year-old previously healthy man presented with a 2-day history of acute-onset, severe epigastric pain, and signs of peritonitis. He reported no history of antibiotic exposure, smoking,

Orbital and Meningeal Metastases Secondary to Breast Adenocarcinoma: A Case Report.

Orbital metastasis and meningeal carcinomatosis (MC) are uncommon complications of breast cancer and typically indicate advanced disease with poor prognosis. While breast cancer is the most frequent primary tumor associated with orbital metastases in women, the simultaneous occurrence of orbital metastasis and MC is exceedingly rare. We present the case of a 51-year-old female patient diagnosed with stage IV breast adenocarcinoma, previously diagnosed with controlled HIV infection, who presented with posterior cervical pain, progressive bilateral visual loss, and left blepharoptosis. Aetiologies of pericardial effusion among medical patients at Nelson Mandela Academic Hospital, Mthatha, South Africa: A cross-sectional study.

BACKGROUND: Pericardial effusion aetiology varies across geographical locations and epidemiological contexts. In the developing world, tuberculosis (TB), catalysed by human immunodeficiency virus (HIV), is the leading aetiology, while in the developed world, it is idiopathic. The TB epidemic and paucity of recent, regional context aetiological profiling may overshadow other aetiologies in the empirical treatment guidelines. METHODS: A retrospective cross-sectional study of clinical records was conducted to investigate the aetiologies of pericardial effusion in adult patients at Nelson Mandela

Eggshell calcification of the urinary bladder in chronic Schistosoma haematobium infection: A radiologic-pathologic case report.

Chronic urinary schistosomiasis is an important cause of hematuria and genitourinary morbidity in individuals from endemic regions. In advanced disease, computed tomography (CT) may show circumferential calcification of the bladder and distal ureters, a classic pattern that can suggest the diagnosis even when laboratory tests are negative. A 38-year-old man from Mozambique, living in Portugal with human immunodeficiency virus and hepatitis B virus infections, presented with 2 months of painless gross hematuria. He denied dysuria, fever, flank pain, weight loss, prior urinary tract instrumentat

Therapeutic challenges in relapsing cutaneous and visceral leishmaniasis caused by Leishmania (Mundinia) martiniquensis in patients with advanced HIV disease from Southern Thailand.

BACKGROUND: Autochthonous leishmaniasis has become increasingly recognized in Thailand, with Leishmania (Mundinia) martiniquensis identified as the predominant species, particularly among immunocompromised individuals. Infected immunosuppressed patients often present with complex clinical features, which can delay diagnosis and complicate treatment. Given the limited clinical data available and emerging reports of resistant infections, improved awareness, prompt diagnosis, and optimized management strategies are urgently needed to address this underrecognized pathogen in Thailand.
