Cerebral Schistosoma japonicum infection confirmed by histopathology and PCR: Diagnostic and therapeutic challenges in multiple parasitic coinfections.

Cerebral schistosomiasis, a rare manifestation of schistosomiasis, is challenging to diagnose due to non-specificity in the clinical and radiologic findings and cross-reactivity in serologic testing with other helminths. Coinfection with multiple parasites further complicates diagnosis and treatment. A previously healthy 32-year-old Filipino man presented with a seizure. Brain magnetic resonance imaging (MRI) revealed multiple clustered enhancing nodules with surrounding vasogenic edema in the left frontal lobe. Serologic testing was positive for anti-Schistosoma japonicum, Strongyloides, and

Behind the splenomegaly: a parasitic twist in siblings with beta thalassemia trait.

BACKGROUND: Thalassemia syndromes are characterized by varying degrees of ineffective hematopoiesis and increased hemolysis. β-thalassemia trait typically presents as mild microcytic anemia and rarely with significant clinical findings such as splenomegaly. However, coexisting conditions in patients from endemic regions can obscure or complicate the clinical picture. Schistosomiasis, a parasitic infection endemic to Sub-Saharan Africa, is a well-recognized cause of splenomegaly and eosinophilia. CASE PRESENTATION: A pair of Congolese siblings with β-thalassemia trait presented with moderate sp

Na-GST-1 adsorbed on Alhydrogel co-administered with different Toll-like receptor agonists in hookworm-naive adults using a controlled human infection model in the USA: a phase 2, double-blind, randomised controlled trial.

BACKGROUND: Necator americanus glutathione S-transferase-1 (Na-GST-1) performs a crucial enzymatic step in hookworm blood feeding and is a leading target for vaccine development. Phase 1 trials in Brazil, Gabon, and the USA of recombinant Na-GST-1 adsorbed on Alhydrogel (Na-GST-1/Al), co-administered with or without AP 10-701 (Toll-like receptor [TLR] 4 agonist) or CpG 10104 (TLR9 agonist), showed the vaccine was well tolerated, safe, and immunogenic. A controlled human hookworm infection (CHHI) model was previously developed to evaluate the effect of candidate hookworm vaccines on infection.

Giant primary splenic hydatid cyst: diagnostic and therapeutic challenges.

Echinococcosis, or hydatid disease, is a zoonotic parasitic infection caused by the larval stage of Echinococcus granulosus Although the liver and lungs are the most commonly affected organs, primary isolated splenic hydatid cysts are rare, accounting for less than 2% of all cases even in endemic regions. Diagnosis can be challenging due to non-specific clinical presentations. We report a female in her late 20s from Rampur district in Uttar Pradesh who presented with dull, intermittent pain in the left upper abdomen for 3 years. Imaging revealed a well-defined cystic lesion within the splenic p

[Pediatric Alveolar Echinococcosis with Pulmonary and Hepatic Involvement and Literature Review: A Rare Case].

Alveolar echinococcosis (AE) caused by Echinococcus multilocularis, is a rare but potentially fatal zoonotic infection. AE most commonly presents with multiple cysts in the liver, may spread to other organs through infiltration or metastasis and can mimic malignancy with fatal consequences if not diagnosed early. Its occurrence in childhood is extremely rare and simultaneous hepatic and pulmonary involvement has been reported only in a few cases. This report aimed to discuss a pediatric AE case with concurrent hepatic and pulmonary involvement, initially misdiagnosed as malignancy, in the ligh

BACKGROUND: Malarial retinopathy refers to a constellation of changes seen in severe or complicated malaria cases. These include: retinal whitening, vessel changes-whitening, tramlining, retinal hemorrhages, and papilledema. There are very few Indian studies on this entity. Since retina can be easily visualized by direct ophthalmoscopy, this study was done to determine prevalence of malarial retinopathy among malaria cases and to determine relationship between malarial retinopathy and severity of the disease. MATERIALS AND METHODS: The study was done at Indoor and Outdoor Departments of Tropi

Worm in the Water: Diagnosing Urinary Ascariasis in an Elderly Patient Through Routine Microscopy.

Ascariasis is primarily an intestinal parasitic infection, with ectopic migration to the genitourinary tract, representing an uncommon clinical entity. Such presentations are infrequently reported and may pose a diagnostic challenge, particularly in the absence of gastrointestinal symptoms. We report a case of an 88-year-old female from a suburban region of Nepal, who presented with bilateral iliac pain and dysuria of three days' duration. Physical examination was notable for mild lower abdominal tenderness, with stable hemodynamic parameters. Laboratory investigations revealed leukocytosis wi

Towards effective and efficient machine learning models for schistosomiasis diagnosis in microscopic images.

Schistosomiasis is an infectious disease caused by the parasite Schistosoma mansoni, and it is a significant health concern in many underdeveloped tropical regions. The traditional method of diagnosing schistosomiasis involves fecal examinations under a microscope for parasite eggs, a process that is time consuming, error-prone, and requires specialized training. Artificial intelligence encompasses a broad range of techniques, including both machine learning and its subset, deep learning, which have been successful in solving such problems. Therefore, this study aimed to propose an automated s

Ultrasonographic findings in Schistosoma mekongi infection. A minireview.

Schistosoma (S.) mekongi is causing intestinal and hepatosplenic schistosomiasis in some South East Asian countries located along the Mekong river. Ultrasonography is the point of care method of choice for detecting and staging hepatosplenic morbidity due to schistosomiasis. We review all publications on ultrasound examinations in S. mekongi infected individuals. An extensive data base search revealed only six eligible publications on 1454 patients. Findings included (peri-)portal fibrosis, splenomegaly and signs of portal hypertension. Portal fibrosis was sometimes difficult to differentiate

Evaluation of molecular and serological testing for imported urogenital schistosomiasis screening in a referral tropical medicine centre in Barcelona, Spain.

BACKGROUND: Schistosomiasis, a major neglected tropical disease, is caused by Schistosoma spp. It is estimated that more than 200 million people are affected worldwide, mostly in Africa. The gold standard diagnosis of urogenital schistosomiasis (UGS) is the microscopic visualisation of Schistosoma haematobium eggs in concentrated urine; however, its sensitivity is low. This study aimed to evaluate the effectiveness of molecular and serological testing for imported UGS screening in asymptomatic sub-Saharan migrants in a non-endemic setting. METHODS: A retrospective cross-sectional study betwee
