Diagnosis of giardiasis relies largely on the microscopical detection of trophozoites or cysts in feces. However, this method is labour- and time-intensive and depends highly on the skill of an experienced microscopist. In order to identify the prevalence of Giardia lamblia in travellers returning from overseas, we evaluated sensitivity and specificity of a commercially available ELISA kit for the detection of Giardia-lamblia-antigen in stool. Nine hundred seventy-eight stool samples from 795 patients were examined by microscopy (iron-hematoxyilin-stain, SAF-concentration) and ELISA. Altogethe

Based on seroprevalence studies, approximately 30% of the cats in the United States have been infected by T gondii. Cats are the only species that pass the environmentally resistant oocyst in feces. Sporulated oocysts are infectious to humans and the organism can cause significant disease in immunocompromised people and transplacentally-infected babies. Clinical illness including liver disease, lung disease, central nervous system disease, fever, and uveitis occur in some infected cats. Veterinarians need to be able to identify T gondii infection in cats because of potential public health risk

OBJECTIVES: To summarize recent information about the "new" gastrointestinal protozoal pathogens (cryptosporidia, microsporidia, isospora, and cyclospora) and to help practicing clinicians integrate this information into their clinical databases by emphasizing the similarities among these organisms.
DATA SOURCES: Relevant English-language articles published between 1988 and 1995 were identified through a MEDLINE search done using the names of the intestinal spore-forming protozoa. Articles cited in the bibliographies of these and other articles were searched manually.

Eosinophilia is defined as an absolute count of > 500 eosinophils per mm3 of peripheral blood. Eosinophilia is associated with many disorders, limiting its usefulness as a diagnostic tool in screening expatriates for parasite infections. In addition, only tissue-invasive helminthic parasites cause eosinophilia, which limits its general application as a screening tool for parasitic infections. Because eosinophilia may resolve spontaneously over time, the life cycle of parasites must be considered when evaluating eosinophilic patients, and repeated stool examinations or appropriate serology may

Skin infections are an important health problem in Vanuatu and other Pacific island countries. In this study, 18,223 people, representing 90% of the population of the southern island of Tanna in Vanuatu, were examined. Scabies was found in 16% of those screened, with 24% of children under 10 years affected. Other skin infections were predominantly furuncles, abscesses, impetigo and cellulitis and were found in 12% overall and in 16% of children under 10 years of age. These infections were found to be more common in larger households and in the vicinity of the volcano Yasur. Strategies for prev

The three major vaginal infections are yeast vulvovaginitis, Trichomonas vaginalis vaginitis, and bacterial vaginosis. In terms of signs and symptoms, these disorders overlap substantially with one another and with other infections. Therefore, the diagnosis of candidiasis and trichomoniasis requires isolation of the responsible pathogen. For the diagnosis of bacterial vaginosis, all other potential causes of vaginal infection must be excluded and specified laboratory criteria must be met. Clinical trials must be carefully designed to control for coexisting pathogens, for potential efficacy of

On the occasion of the thirty-five years of systematic research of toxoplasmosis, a review of the present knowledge of the role of toxoplasmosis in pregnancy is presented. Diagnostic procedures and the interpretation of the results of laboratory examinations are particularly stressed. The main conclusion is that in a population exposed to the infection in the way as was in the cases studied, the prevention of congenital toxoplasmosis can be achieved through a serologic follow-up of all pregnant women starting from the moment when pregnancy is established. The purpose of such screening is to id

As llamas become more common in North America, veterinarians will be called on ever more frequently to deal with their dermatologic problems. Adherence to the basic tenets of the thorough dermatologic work up, including history, physical examination, skin scrapings, cytological examinations, fungal culture, skin biopsies, and assessing response to judiciously chosen trial therapies will offer the best opportunity of arriving at proper diagnoses. Special attention must be paid to the fact that llama skin bears some histological differences that may prove confusing to the uninitiated. A mild deg

Clinical toxoplasmosis was diagnosed in 15 cats by correlating serologic evidence of infection and clinical signs to either response to therapy or histopathologic demonstration of the organism. Ophthalmic manifestations, primarily involving the anterior segment, were common. Other common physical examination abnormalities included muscle hyperesthesia, fever, and weight loss. Response to therapy was variable, but administration of clindamycin hydrochloride resulted in resolution of all clinical signs not involving the eyes in surviving animals. This drug, alone or in combination with corticost
