Mucinous cystic neoplasm of the liver with biliary prolapse mimicking intraductal papillary neoplasm of the bile duct: a case report.

Mucinous cystic neoplasm of the liver (MCN-L) is a rare cyst-forming epithelial neoplasm that typically lacks bile duct communication. Intraductal protrusion is uncommon and can mimic cystic intraductal papillary neoplasm of the bile duct (IPNB). A 39-year-old woman undergoing fertility treatment presented with recurrent hyperbilirubinemia and predominantly cholestatic liver test abnormalities. MRI demonstrated a 10.5-cm multilocular cystic lesion in segment 4 of the liver with downstream biliary dilatation, and internal septa were visible within the dilated bile duct on T2-weighted single-sho

Extent of liver resection for incidental gallbladder cancer: Anatomic versus nonanatomic approaches in T2-T3 disease.

BACKGROUND: Incidental gallbladder cancer (IGBC) is increasingly diagnosed after cholecystectomy performed for presumed benign disease. For T2 and selected T3 tumors, completion radical re-resection with liver resection and regional lymphadenectomy is recommended, but the optimal hepatic extent-nonanatomic wedge resection versus anatomic segment IVb/V resection-remains debated. METHODS: This narrative review summarizes evidence from IGBC-specific series and registries, international multicenter cohorts, meta-analyses, and contemporary consensus guidelines comparing wedge resection and segment

Primary Extrahepatic Biliary Mucinous Cystic Neoplasm Suspected Preoperatively and Treated by Robotic Pancreaticoduodenectomy: A Case Report.

INTRODUCTION: Biliary mucinous cystic neoplasm (MCN) is a cystic tumor characterized by ovarian-like stroma and is recognized as a neoplasm with the potential for invasive carcinoma. While MCNs predominantly occur in the pancreas, primary occurrence in the extrahepatic bile duct is exceedingly rare. Most reported cases are diagnosed postoperatively due to the lack of specific clinical symptoms, the difficulty in preoperative differentiation from other cystic diseases, such as intraductal papillary neoplasm of the bile duct, and the strict requirement of pathological evaluation for definitive d

Human biliary atresia extrahepatic cholangiocyte organoids express increased ER and oxidative stress, altered drug metabolism and cell polarity changes.

Biliary atresia (BA), the leading cause of pediatric liver transplantation, is characterized by neonatal jaundice and progressive extrahepatic bile duct obstruction, yet its pathogenesis remains elusive. To uncover extrahepatic cholangiocyte injury, we generated patient-derived extrahepatic cholangiocyte organoids (EHCOs) from bile duct remnants of BA and controls. We performed discovery bulk RNA-seq followed by targeted molecular and functional validation. Transcriptomic profiling and pathway/perturbation analyses revealed dysregulation of ER stress pathways, xenobiotics/drug metabolism, and

Pancreaticoduodenectomy with Preservation of a Prior Hepaticojejunostomy for Ampullary Carcinoma: A Case Report.

INTRODUCTION: Pancreaticoduodenectomy (PD) can be technically challenging in patients who previously underwent biliary reconstruction, particularly hepaticojejunostomy. The optimal strategy for managing a prior hepaticojejunostomy during PD has not been well established. CASE PRESENTATION: A woman in her 70s developed ampullary carcinoma 10 years after diversion surgery for pancreaticobiliary maljunction. She had previously undergone laparoscopic extrahepatic bile duct resection with hepaticojejunostomy. Subtotal stomach-preserving pancreaticoduodenectomy was planned. Intraoperatively, dense

Multidisciplinary treatment with durvalumab-containing regimens for mixed neuroendocrine-non-neuroendocrine neoplasm of the extrahepatic bile duct.

Mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) are rare tumors comprising both neuroendocrine and non-neuroendocrine components, with each component constituting at least 30% of the tumor. MiNEN of the extrahepatic bile duct is particularly uncommon and is often associated with a poor prognosis. We report the case of a 63-year-old male with jaundice, who was diagnosed with cholangiocarcinoma at the confluence of the cystic duct. Imaging and endoscopic cholangiography revealed perihilar-to-distal bile duct involvement, accompanied by bulky lymph node metastases. Endoscopic biliary d

A Biliary Behemoth: A Report of a Rare Case of a Giant Choledochal Cyst.

Choledochal cysts are very uncommon congenital anomalies of the biliary tract, most commonly diagnosed in childhood. Adult presentation is uncommon and often atypical. Giant choledochal cysts, defined as cysts measuring more than 10 cm, are exceptionally rare and present significant diagnostic and surgical challenges. A 19-year-old female presented with a two-month history of dull-aching abdominal pain, progressive right upper quadrant abdominal swelling, and features of obstructive jaundice. Physical examination revealed deep jaundice and a large, non-tender abdominal mass. Imaging studies, i

A Machine Learning Model for Predicting Posthepatectomy Liver Failure After Hepatectomy With Extrahepatic Bile Duct Resection for Perihilar Cholangiocarcinoma: With and Without Indocyanine Green.

AIM: Posthepatectomy liver failure (PHLF) is a serious complication of major hepatectomy and is closely related to perioperative mortality. Whether or not indocyanine green (ICG) is used to measure liver function varies by country. Using machine learning, we aimed to develop two highly accurate, user-friendly models for predicting PHLF depending on whether ICG data were used. METHODS: This was a retrospective, three-center study. SHapley Additive exPlanations (SHAP) evaluated the feature importance of Random Forest (RF) analysis. SHAP quantitatively assessed the impact of each feature on mode

Clinicopathological Significance of Leucine-rich Repeat-containing 15 in Distal Extrahepatic Bile Duct Carcinoma.

Leucine-rich repeat-containing 15 (LRRC15) is a membrane protein whose overexpression has been linked to poorer survival outcomes in various malignant neoplasms. However, its role in distal extrahepatic bile duct carcinoma (DBDC) remains unclear. LRRC15 protein expression in tumor cells and stroma was evaluated with 196 surgically resected DBDCs, and compared with clinicopathological variables, including patients' overall survival (OS) and recurrence-free survival (RFS). LRRC15 protein expression was identified in stroma of 21 DBDCs (11%) and tumor cells in 67 cases (34%). On the basis of the

Establishment of extrahepatic bile duct cancer organoids using suspension culture and their application in clinical drug screening.

The present study aims to explore the feasibility of establishing human extrahepatic cholangiocarcinoma (eCCA) organoids via suspension culture, to biologically characterize their biological features and to evaluate their drug sensitivity, thereby providing a preclinical model for personalized precision therapy. Surgically resected tumor tissues were obtained from patients with eCCA. Tumor cells were isolated by mechanical dissociation and enzymatic digestion, and were cultured in a matrix-free suspension system for three-dimensional organoid formation and continuous passaging. Laparoscopic/robotic embedding hepaticojejunostomy with FHDO for biliary tract malformations with a small-diameter hepatic duct opening: a retrospective study.

BACKGROUND: Small hepatic duct opening diameter is an important cause of postoperative anastomotic stricture (PAS) following excision of cyst or extrahepatic bile duct for children with choledochal cyst (CC) or non-dilated pancreaticobiliary maljunction (PBM). We compared an embedded hepaticojejunostomy with flipping the end of hepatic duct outward (FHDO; modified anastomosis) to embedded hepaticojejunostomy (embedded anastomosis) and evaluated its feasibility in preventing PAS. METHODS: This retrospective study included CC or non-dilated PBM children with a small-diameter hepatic duct openin

Intraoperative indeterminate frozen-section margin leading to an unintended R1 resection in perihilar cholangiocarcinoma: a case report and decision-making framework.

Intraoperative frozen-section (FS) margin assessment is essential for guiding precise resection in perihilar cholangiocarcinoma (pCCA). However, freezing artifacts, superficial sampling, drainage-related reactive epithelial changes, and subtle cytologic atypia in well-differentiated adenocarcinoma can make intraoperative interpretation difficult. We report a 70-year-old woman with Bismuth-Corlette type II pCCA who underwent radical extrahepatic bile duct resection after PTBD-mediated biliary decompression. Intraoperative FS of the distal bile duct margin showed atypical glands of undetermined
