The experience of running a specialized photoroentgenographic station of the Institute is discussed. The total of 4,052 examinations (healthy subjects for prophylaxis and patients with gastrointestinal complaints) were carried out. The percentage of early gastric cancer diagnosed by means of prophylactic photoroentgenography was significantly higher than that yielded by standard out-patient examination procedures. Such centers should be set up primarily at large polyclinics, regional hospitals, etc.

An extremely rare case of malignant mixed tumor of the liver in adults is reported. A 60-year-old man was admitted because of abdominal fullness. Abdominal CT-scan demonstrated a low density lesion surrounded by high density ring area in the liver. Liver scintigram and selective hepatic angiography revealed a malignant tumor mass which was indistinguishable from other malignant masses. Postmortem examination disclosed that histology of the liver tumor was Edmondson's grade II hepatocellular carcinoma and chondrosarcoma. Cytological examination of exfoliated cells in the bile or irrigated fluid from a percutaneous transhepatic choledochal drainage (PTCD) tube and other jaundice-reducing fistula was performed in 59 patients with obstructive jaundice. The work is based on the experience with the exploration and treatment of 310 patients with the functional obstruction of the cardia which are found to have two pathogenetic forms-cardiospasm and cardiac achalasia. The article analyzes results of the examination and surgical treatment of 24 patients with carcinoma of the gastric stump developing at different terms after resection of the stomach for gastroduodenal ulcers. Questions of pathogenesis, clinical picture and diagnosis of the disease are discussed. Special attention is called to low percentage of radical operations for gastric stump carcinoma. An important role of dispensary observation with the use of gastroscopy for patients subjected to resection of the stomach for peptic ulcers is emphasized.

We performed a mass survey using a thin forward-viewing gastroscope (Olympus GIF-P2) as a secondary examination of 449 persons from 3457 employees who had undergone the first step of the screening with photofluorography. Five patients with hyperplastic polyps of the stomach were found in the course of 573 double contrast examinations over a period of six and a half months. The radiological findings were similar in all cases, consisting of multiple small round mucosal elevation with sharp margins, situated in the body of the stomach. They can probably extend into the fundus and the pylorus. Histological examination has indicated that these polyps are the result of an inflammatory process.

In a follow-up study of a consecutive series of 1000 patients, who underwent Billroth II resection for duodenal ulcer 22-30 years earlier, the mucosal changes of the gastric stump could be studied by endoscopy and biopsy in 196 cases. Difficulties in evaluating the gastric cardia are well known and may often be resolved with the double-contrast technique. Special views to show the cardia en face were obtained in 200 double-contrast examinations. Four basic patterns were recognized. While there is no generally applicable method of test reduction, ways in which the problem can be tackled can usually be suggested. These include the examination of error rates of tests and the use of current statistical methods. When the cost-effectiveness of some more expensive test has to be measured, a model of the decision problem needs to be developed, with an attempted estimation of the values or utilities of the states of health that result from treatment.

The investigation covers 96 patients from Sofia, with cancer of the digestive system, with unsatisfactory early diagnosis -- 3.6 per cent and operability -- 36 per cent.
