Predictive Feasibility Of Colonoscopy Without Sedation Using A Nomogram Model.

Colorectal cancer poses a global burden, and colonoscopy is an effective screening tool, with unsedated colonoscopy offering distinct advantages but facing completion challenges. A retrospective analysis of 745 patients who underwent unsedated colonoscopy between July 2021 and August 2022 was conducted, with participants divided into completion (n = 670) and incompletion (n = 75) groups. Univariate analysis revealed significant differences in gender, age, constipation history, anxiety grade, postoperative Visual Analogue Scale scores and examiner experience between groups. Cardiovascular health assessed by Life's Essential 8 score, genetic risk, colonoscopy status, and risk of colorectal cancer: an analysis of UK Biobank.

OBJECTIVE: Colorectal cancer (CRC) and cardiovascular disease (CVD) share common risk factors. This study explores the association between cardiovascular health (CVH), assessed using the Life's Essential 8 (LE8) score, and CRC risk, while examining the modifying roles of genetic predisposition and colonoscopy screening. METHODS: A total of 291 177 UK Biobank participants were included in the analysis. LE8 comprises eight indicators: diet, physical activity, nicotine exposure, sleep health, body mass index, blood lipids, blood glucose, and blood pressure. Cox proportional hazards models were u

Evaluation of Obesity as an Independent Risk Factor for Colorectal Dysplasia Development in Inflammatory Bowel Disease: A Matched Case-Control Study.

BACKGROUND: Chronic intestinal inflammation is a well-established driver of colorectal dysplasia in inflammatory bowel disease (IBD). However, the role of metabolic factors such as obesity remains poorly understood. We evaluated whether chronic obesity, measured using five-year longitudinal body mass index (BMI), is independently associated with colorectal dysplasia in patients with IBD. METHODS: We conducted a retrospective 1:1 matched case-control study at a tertiary academic center. Adult patients with ulcerative colitis (UC) or Crohn's disease (CD) who underwent surveillance colonoscopy b

Colonoscopy findings and polyp pathology in CHEK2 carriers: a single center experience.

Checkpoint kinase 2 is a moderate-penetrance cancer susceptibility gene associated with increased colorectal cancer risk. Recent guideline updates recommend average-risk colorectal cancer screening for carriers; however, real-world colonoscopy data remain limited. We performed a retrospective review of individuals with confirmed pathogenic or likely pathogenic checkpoint kinase 2 variants identified through an institutional hereditary cancer registry from 2011 to 2024. Colonoscopy and pathology reports were analyzed for demographics, cancer history, bowel preparation quality, and polyp charact

European Code Against Cancer, 5th edition - organised cancer screening programmes.

The 5th edition of the European Code Against Cancer (ECAC5) recommends sustainable, organised screening programmes for: (a) colorectal cancer using biennial quantitative faecal immunochemical test (FIT) for individuals aged 50-74 years. As an alternative strategy, once-only endoscopy may be considered within the same age range; (b) breast cancer using biennial digital mammography for women aged 50-69 years. Implementing this strategy for women aged 45-49 years and 70-74 years can be considered. Other screening strategies or additional examinations could be considered for women with high mammog

Stool DNA-based SDC2 Methylation Test for the Screening of Colorectal Neoplasia in an Asymptomatic, Average-Risk Population.

BACKGROUND/AIMS: Programmatic screening for colorectal cancer (CRC) could maximize the impact of screening in the average-risk population, but the diagnostic performance of a stool DNA-based Syndecan-2 methylation (meSDC2) test has only been reported in case-control studies or high-risk populations. This study examined the performance of a stool DNA-based meSDC2 test for CRC in an average-risk population from a real-world setting. METHODS: This retrospective, multicenter study included consecutive asymptomatic, average-risk individuals for CRC who completed a meSDC2 stool test at 18 hospitals

Risk Factors of Proximal and Distal Advanced Colorectal Conventional and Serrated Neoplasia in Adults Younger Than 50 Years of Age.

INTRODUCTION: Colorectal cancer (CRC) has increased in adults younger than 50 years of age, particularly in the distal colon. Data examining the risk factors of advanced colorectal neoplasia in young adults as stratified by anatomic location have important implications for screening and risk modification. METHODS: We conducted a cross-sectional analysis of data from New Hampshire Colonoscopy Registry participants younger than 50 years of age. We developed an average-risk equivalent sample by excluding patients with IBD, genetic syndromes, and those with indications predictive of colorectal ne

Higher Sessile Serrated Lesion Detection Rates Calculated Using All Examinations Are Associated With Lower Risk for Postcolonoscopy Colorectal Cancer: Data From the New Hampshire Colonoscopy Registry.

INTRODUCTION: Calculating detection rates using data from colonoscopies for all indications, rather than screening examinations, is simpler and can mitigate gaming by endoscopists. We hypothesized that calculating sessile serrated lesion detection rates (SSLDR-A) using all examinations may also be a quality metric for predicting postcolonoscopy colorectal cancer (PCCRC) risk. METHODS: The cohort included New Hampshire Colonoscopy Registry 115,762 patients with an index colonoscopy. The primary outcome was PCCRC, defined as colorectal cancer (CRC) diagnosed ≥6 months after the index colonoscop

Association Between Red Blood Cell Distribution Width-To-Albumin Ratio and Breast, Colorectal, Lung, and Prostate Cancers.

INTRODUCTION: Cancer remains a leading cause of global morbidity and mortality, underscoring the need for accessible biomarkers for early detection and risk stratification. The red blood cell distribution width-to-albumin ratio (RAR) integrates inflammation and nutritional markers often dysregulated in cancer. We evaluated RAR's association with a recent diagnosis (≤ 1 year) of breast, colorectal, lung, and prostate cancer in a representative US population. METHODS: Using National Health and Nutrition Examination Survey data (2005-2018), we conducted a cross-sectional analysis of adults with

Management of Colorectal Sessile Serrated Lesions in Elderly Patients.

Sessile serrated lesions (SSLs) are established precursor lesions of colorectal cancer arising through the serrated neoplasia pathway and are estimated to account for a substantial proportion of sporadic colorectal cancers. Accumulating evidence has clarified their distinctive endoscopic, histopathological, and molecular characteristics, including frequent BRAF mutations, CpG island methylator phenotype (CIMP), and progression toward microsatellite instability-high carcinoma. Although the natural history of nondysplastic SSLs is generally slow, lesions that acquire dysplasia may demonstrate mo

Development and validation of a machine learning model for colorectal cancer status classification using NHANES data: A cross-sectional study.

Colorectal cancer (CRC) is a leading cause of cancer-related morbidity and mortality worldwide. Tools based on routinely collected variables may help identify prevalent CRC status and support clinical evaluation. Traditional approaches often rely on limited predictors and may not capture the multidimensional nature of CRC. Data were obtained from the National Health and Nutrition Examination Survey 1999-2018. Among 53,881 participants, 420 reported physician-diagnosed CRC (MCQ220). A 1:10 stratified case-control sample was constructed (420 cases, 4200 controls) and randomly split into training

Prevalence of preoperative sarcopenia according to EWGSOP2 in patients with colorectal cancer and its impact on surgical outcomes: a prospective observational study.

BACKGROUND: Sarcopenia may adversely affect postoperative outcomes, but most studies in colorectal cancer (CRC) have relied solely on computed tomography (CT)-derived muscle mass. We evaluated the association between a comprehensive assessment based on the European Working Group on Sarcopenia in Older People 2 (EWGSOP2) criteria and short-term postoperative outcomes after curative CRC surgery. METHODS: In this prospective observational study, 70 consecutive patients who underwent curative CRC resection between July 2024 and July 2025 were assessed for muscle strength (handgrip strength), phys

Impact of Narrow-Band Imaging on Sessile Serrated Lesion Miss Rate: A Multicenter Randomized Tandem Colonoscopy Trial.

BACKGROUND & AIMS: Sessile serrated lesions (SSLs) are precursors to post-colonoscopy colorectal cancer but are frequently missed because of their subtle morphology. Previous studies have reported the effectiveness of narrow-band imaging (NBI) for detecting colorectal adenomas, but its value for detecting SSLs remains unclear. This study aimed to determine whether NBI reduces the SSL miss rate (SSLMR). METHODS: This multicenter, randomized tandem trial was conducted in a colorectal cancer screening population across 15 endoscopy centers. The participants were randomly assigned to NBI-first or

Clinical characteristics of colorectal flat adenoma and the risk factors associated with missed flat adenoma: A retrospective cohort study.

Colorectal flat adenoma is particularly closely linked to interval cancer of the proximal colon, and therefore its clinical importance has been increasing. However, there is a lack of research on colorectal flat adenoma. This study aims to examine the clinical characteristics of colorectal flat adenoma and the risk factors related to missed flat adenoma. This retrospective cohort study was conducted on the medical records of patients who had colonoscopies performed at this hospital (single-center, Korea) from January 2018 to December 2023. The colorectal adenomas were categorized into flat and
