Prevalence of self-reported pain in patients 5 years after rectal cancer treatment: results from the prospective QoLiRECT cohort study.

BACKGROUND AND PURPOSE: Pain as a long-term outcome following rectal cancer treatment has received limited attention in previous studies. This prospective cohort study examined pain prevalence up to 5 years post-surgery in patients treated with curative intent. Secondary aims included evaluating pain intensity and its interference with daily life, and comparing pain experiences with a reference population. In addition, potential risk factors for pain were explored. Patient/material and methods: The study is based on the multicenter prospective cohort study QoLiRECT including patients from 16 s

Colonoscopy in elderly patients: diagnostic yield, pre-referral assessment and quality indicators in two Swedish cohorts.

OBJECTIVES: Colonoscopy in elderly, frail patients requires weighing diagnostic yield against procedure-related risk, but real-world data are limited. We describe outcomes from two Swedish cohorts undergoing non-screening colonoscopy in two regions. METHODS: We combined aggregated results from two retrospective cohort studies. The Ersta cohort included all patients aged ≥75 years undergoing colonoscopy in 2022 and compared inpatient versus outpatient examinations. The Örebro cohort included all colonoscopies in patients aged ≥80 years during 2016-2018. We extracted data on indications, pre-re

MRI versus histopathology in EMVI detection for rectal cancer: prognostic relevance and survival outcomes.

OBJECTIVES: To evaluate the diagnostic accuracy of magnetic resonance imaging (MRI) compared to histopathology for detecting extramural venous invasion (EMVI) in rectal cancer, and to assess their prognostic value for long-term recurrence-free survival (RFS) and overall survival (OS). MATERIALS AND METHODS: In a prospective cohort of 150 patients with biopsy-confirmed rectal adenocarcinoma, EMVI was assessed via high-resolution 1.5-T MRI and histopathological examination. Interobserver agreement, clinical correlations, and survival outcomes were analyzed. Kaplan-Meier and Cox regression analy

Clinical value of wide-angle colonoscopy combined with narrow-band imaging in detecting sessile serrated lesions during colorectal cancer screening: A single-center retrospective observational study.

Sessile serrated lesions (SSLs) represent a clinically significant challenge in colorectal cancer screening due to their flat morphology and association with interval cancers. Advanced endoscopic techniques such as wide-angle colonoscopy (WAC) and narrow-band imaging (NBI) may enhance detection rates of SLs, but evidence regarding their combined efficacy remains limited. This study aimed to evaluate the effectiveness of WAC combined with NBI in detecting SSLs compared to NBI alone and standard white-light endoscopy (WLE) during colorectal cancer (CRC) screening. In this single-center retrospec

Computer-aided detection in surveillance colonoscopy: a population-based randomized trial.

BACKGROUND: Computer-aided detection (CADe) increases adenoma detection in colonoscopy, but its effectiveness in postpolypectomy surveillance remains uncertain. This study compared the adenoma detection rate (ADR) of CADe-assisted versus standard high definition colonoscopy during surveillance. METHODS: We conducted a population-based, randomized controlled trial in the Galician colorectal cancer screening program. Colonoscopy days were randomized in a 1 : 1 ratio to high definition colonoscopy with or without CADe. The primary end point was the ADR. Prespecified subgroup analyses evaluated e

Macroscopic and microscopic features indicating serosal invasion of colonic adenocarcinoma.

OBJECTIVES: Colonic adenocarcinoma poses a significant global health burden. Accurate detection of serosal invasion (pT4) is crucial for prognosis, yet inconsistent grossing protocols create diagnostic challenges. We aimed to evaluate macroscopic predictors of serosal invasion, determine the minimum tumor blocks required for accurate staging, and assess pT4 frequency at Siriraj Hospital. MATERIALS AND METHODS: We performed a retrospective review of a cohort comprising 218 patients diagnosed with colonic adenocarcinoma. The assessment was conducted by gastrointestinal pathologists and a traine

Appendiceal mucocele on imaging masking invasive goblet cell adenocarcinoma.

A woman in her late 60s presented with right lower quadrant abdominal pain suggestive of acute appendicitis. Cross-sectional imaging demonstrated a dilated, fluid-filled appendix with mural enhancement, interpreted as concerning for appendiceal mucocele. Laparoscopic appendectomy was performed. Histopathologic examination revealed goblet cell adenocarcinoma invading into the subserosa (pT3) with involvement of the proximal resection margin. Completion right hemicolectomy was subsequently undertaken for definitive oncologic management and lymph node staging and revealed no residual carcinoma, w

Intestinal Endometriosis Mimicking Ulcerative Colitis-Associated Neoplasia: A Case Report.

INTRODUCTION: Intestinal endometriosis is a benign condition characterized by the presence of endometrial tissue within the bowel wall, often mimicking colorectal malignancy. Its coexistence with ulcerative colitis (UC) is rare and can complicate the diagnosis. CASE PRESENTATION: A 35-year-old woman with long-standing UC presented with severe diarrhea and rectal bleeding. Colonoscopy revealed severe disease activity with multiple deep ulcerations and a newly developed stricture in the sigmoid colon. Pathological examinations, including bite-on-bite biopsies, showed no evidence of malignancy.

Quality Indicators of Endoscopists for Both Index and Surveillance Colonoscopy are Associated With Risk of Metachronous Colorectal Neoplasia.

INTRODUCTION: An association between higher adenoma detection rate (ADR) at index screening colonoscopy and lower risk of metachronous advanced neoplasia (AN, defined as colorectal cancer [CRC] or advanced adenoma [AA]) has been reported. However, the relationship between ADR at both index and surveillance colonoscopy and subsequent AN is unknown. We examined the association between ADR and withdrawal time (WT) at index and surveillance colonoscopy and risk of metachronous AN at surveillance colonoscopy. METHODS: We used GIQuIC, a repository of colonoscopies across the United States. Each pat

Research progress in the diagnosis and treatment of pseudomyxoma peritonei.

Pseudomyxoma Peritonei (PMP) is a rare and complex clinical syndrome characterized by the extensive implantation and metastasis of tumor cells from different tissues within the abdominal cavity, accompanied by the continuous secretion of large amounts of jelly-like mucin. PMP primarily originates from appendiceal mucinous tumors, and in rare cases, can also arise from organs or tissues such as the ovary, colon, gallbladder, pancreas, urachus, small intestine, stomach, intestinal duplications, and teratomas. PMP has diverse clinical manifestations and lacks specificity, making diagnosis difficu

An Isolated Colonic Neurofibroma Without Systemic Neurofibromatosis: A Rare Case Report and Literature Review.

BACKGROUND Solitary neurofibromas of the colon are extremely rare and are most often reported in patients with neurofibromatosis type 1 (NF1). Although usually benign, these lesions can rarely undergo malignant transformation, particularly when associated with NF1. Therefore, isolated cases merit careful evaluation and follow-up. CASE REPORT A man in his 70s with a history of benign prostatic hyperplasia presented with recurrent abdominal pain, constipation, and unintentional weight loss of 5 kg over 1 month. Physical examination revealed a firm, non-mobile mass in the right lower quadrant, wi

Early gastric cancer with malignant invasion into the duodenum: A case report.

RATIONALE: Well-differentiated early gastric cancer (EGC) typically exhibits limited invasive potential, and duodenal involvement is exceedingly rare. This report presents a unique case of a well-differentiated early gastric cancer with extensive duodenal invasion. Magnifying endoscopy is critical for elucidating the growth and infiltration patterns of such lesions. Furthermore, this study provides a detailed magnified endoscopic comparison between the gastric and duodenal mucosa. PATIENT CONCERNS: A 62-year-old female patient presented with 20 days of epigastric discomfort, but no nausea, vo

Pneumatosis cystoides intestinalis mimicking colonic subepithelial tumors with spontaneous resolution after relocation from a high-altitude region: a case report.

BACKGROUND: Pneumatosis cystoides intestinalis(PCI) is a relatively rare condition with a pathogenesis that remains incompletely understood. It is typically managed through conservative medical treatment, with surgical intervention considered when necessary. We report a case of PCI in which the lesions resolved spontaneously after diagnostic endoscopic submucosal dissection (ESD). CASE PRESENTATION: A 51-year-old female, who had resided long-term at high altitude, presented with persistent abdominal pain and increased bowel movements. Routine blood tests and biochemical profiles showed no sig

Colonic metastasis of dermatofibrosarcoma protuberans detected by colonoscopy.

Colonic metastasis from dermatofibrosarcoma protuberans (DFSP) is exceedingly rare, and few reports have described its characteristic endoscopic features. Here, we report the case of a 66 year-old woman with a history of DFSP who presented with a colonic lesion detected during colonoscopy. The lesion appeared as a firm, subpedunculated, elevated mass in the descending colon. At the initial examination, the lesion was covered with intact epithelium; however, at a follow-up examination 6 weeks later, the surface epithelium had sloughed off. Endoscopic ultrasonography revealed a predominantly sub

Gastric Adenomas and Mimickers: A Review.

Gastric adenomas are epithelial premalignant neoplasms which may appear similar on endoscopic examination; however, histologically, they are classified into four distinct subtypes according to the World Health Organization (WHO) Classification of Tumors of the Digestive System: intestinal adenoma, foveolar-type adenoma, oxyntic gland adenoma, and pyloric gland adenoma. Each subtype has characteristic histologic features that are essential for accurate diagnosis, although distinction can be challenging due to overlapping morphologic patterns. Moreover, these entities differ in their potential f
