We present a case of bladder transitional cell carcinoma with sigmoidovesical fistula due to diverticulitis. The patient was a 65-year-old male who had been suffering from recurrent abdominal pain with high fever for the past 6 months. He noticed pneumaturia and cloudy urine several times. He visited a doctor complaining of macroscopic hematuria and high fever, and was treated under the diagnosis of acute prostatitis. This retrospective study describes the accuracy of magnetic resonance (MR) imaging in determination of local disease extent in 34 examinations performed for evaluation of primary or recurrent bladder carcinoma. The 34 examinations were carried out in 29 patients (17 men, 12 women, aged 34-89 years). All studies were followed within 30 days by surgical and pathologic evaluation. Statistics on urological waiting lists suggest that there is considerable regional variation in delay before treatment. This clearly depends on many factors but is unlikely to indicate significant variation in morbidity either at presentation or during subsequent admission for surgery. Managers and politicians should be extremely wary if encouraged to base major changes in distribution of funding and equipment on a casual examination of waiting list figures. It is doubtful if delay in diagnosis or treatment seriously influences morbidity and mortality from urological illness.

We evaluated preoperative and postoperative bladder function in 31 consecutive patients who underwent definitive operative correction of lipomyelomeningocele. Of 12 patients less than 1.5 years old at operation, bladder function was normal preoperatively and postoperatively in 5 (42%) and normalized postoperatively in 4 (33%). In 5 patients (42%) abnormal urodynamic findings were the only sign of neurological abnormality. Twenty patients with bladder carcinoma (14 transitional cell carcinoma and six squamous cell carcinoma) and ten controls of (four with bilharzial lesions, three without and three with metaplasia and dysplasia) were subjected to immunocytochemical and immunohistochemical studies. Unilateral ectopic ureter was diagnosed by endoscopic examination in an 18-month-old filly examined because of chronic urine dribbling. Intramuscular administration of azosulfamide discolored the urine and enhanced visualization of the ectopic ureter. Endoscopy proved valuable in determining that only one ureter entered the urinary bladder and in locating the ectopic ureter in the ventral portion of the vagina.

We report a case of benign polyp with prostatic-type epithelium of the urinary bladder. A 58-year-old male presented with gross hematuria. Cystoscopic examination revealed a 2-mm polypoid lesion in the mid trigone. Immunohistochemical demonstration of prostatic acid phosphatase and prostatic specific antigen in the urothelial cells as well as the prostatic-type epithelial cells suggested the histogenesis of this polyp to be metaplasia of the urothelium.

A case of primary non-Hodgkin's lymphoma of the terminal ileum with enterovesical fistula is reported. A 50-year-old Malay man presented with haematuria, dysuria and per-rectal bleeding. Intravenous urogram, double contrast enema and an MDP bone scintigram showed a fistulous communication between the bladder and distal ileum. At laparotomy, a large tumour attaching the terminal ileum to the dome of the bladder was found. Histopathological examination of resected small bowel revealed a diffuse histiocytic non-Hodgkin's lymphoma of the small bowel. The bladder mucosa was shown to be normal.

The results of screening for urinary tract tumors provided by the Republican Endoscopic Center are discussed. Groups at high risk for cancer were formed on the evidence of microhematuria. Cases of hematuria were referred to the endoscopic center for complex endoscopic, sonographic and morphological examination. Out of 575 patients at high risk, renal pathology was detected in 39.7% (tumors in 3.1% included) and urethral and bladder pathology--in 19.1% (benign and malignant tumors of the bladder in 4.4% included).

We report a case of an extragonadal germ cell tumor from the retrovesical region associated with Klinefelter's syndrome. The patient presented with symptoms of macroscopic hematuria and micturition pain. An x-ray and digital examination were suspicious for prostatic carcinoma. However, needle biopsy of the tumor revealed embryonal cell carcinoma. Serum alpha-fetoprotein was 10,700 ng. per ml.
