Retroperitoneal Hibernoma Presenting as a Gastrointestinal Surgical Diagnostic Challenge: A Rare Benign Lipomatous Tumor and Literature Review.

BACKGROUND: Hibernoma is an uncommon benign adipocytic neoplasm showing differentiation toward brown fat. Although it usually occurs in anatomical sites where residual brown adipose tissue persists, retroperitoneal involvement is particularly uncommon, with fewer than 60 cases reported. CASE PRESENTATION: A 54-year-old man was found to have a retroperitoneal mass incidentally during a routine physical examination. CT and MRI demonstrated a 15-cm homogeneous fat-containing lesion without radiological evidence of local invasion. Complete surgical excision was performed. Histopathological examin

Hidden in Plain Sight: The Diagnostic Odyssey of Glomus Tumors.

Glomus tumors are benign mesenchymal neoplasms arising from specialized neurovascular dermal structures known as glomus bodies, which participate in thermoregulation by modulating cutaneous blood flow. While these tumors are predominantly located in the subungual and periungual regions of the distal extremities, they may arise at extradigital sites, where atypical presentations frequently delay diagnosis. Despite the availability of a well-established clinical triad, these tumors are notoriously difficult to diagnose promptly. We present three surgically confirmed cases of glomus tumor, each r

Novel Liquid Biopsy in Gastrointestinal Cancers.

Gastrointestinal (GI) cancers represent a significant global health burden, being among the leading causes of cancer-related deaths. The prognosis for patients remains unsatisfactory, largely because most cancers are detected at advanced stages. Traditional diagnostic methods, such as radiological and histopathological examinations and serum tumor markers like AFP, CEA, CA-125, and CA-199, possess limitations in sensitivity and specificity, particularly for early screening. A major drawback of tissue biopsy is its inability to fully capture the inherent heterogeneity within tumors, as mutation

Primary Neuroendocrine Carcinoma of the Breast: A Diagnostic Challenge and Case Report.

BACKGROUND: Primary neuroendocrine carcinoma of the breast (NECB) represents an exceedingly rare entity, comprising less than 0.5% of all breast carcinomas and approximately 1% of all neuroendocrine neoplasms. The 2019 WHO classification defines NECB as tumors expressing neuroendocrine markers in > 90% of tumor cells with morphological features resembling neuroendocrine tumors of other anatomical sites. CASE PRESENTATION: A 61-year-old postmenopausal female presented with a palpable left breast mass. Clinical examination revealed a 2 × 3 cm firm, poorly circumscribed mass at the 9 o'clock pos

Artificial Intelligence Design for Race-Based Prostate Cancer Stage Classification With Multilayer Perceptron: Feature Selection Optimization Approach.

BACKGROUND: Prostate cancer progression exhibits significant variability influenced by biological and racial factors. DNA methylation profiling has shown potential in early cancer detection, but its integration with machine learning across racially diverse populations remains limited. OBJECTIVE: This study aimed to develop a prostate cancer stage classifier for the majority White cohort using DNA methylation data and a multilayer perceptron (MLP) model in order to classify prostate cancer stages into early (stages I-II) and late (stages III-IV) stages and assess its performance when applied t

PRIMA: randomized prospective multicenter non-inferiority study for primary diagnosis of clinically significant PRostate cancer by PSA and MR IMAging-study protocol for a randomized diagnostic accuracy trial.

BACKGROUND: Diagnostic pathways based on PSA, digital rectal examination (DRE), and systematic biopsy (SB) may miss clinically significant prostate cancer (csPCa) and lead to overdiagnosis of indolent disease. Multiparametric MRI (mpMRI) and MRI-targeted biopsy (TB) improve detection of csPCa; however, the additional diagnostic value of routine SB in biopsy-naïve men with suspicious MRI findings remains controversial. METHODS: PRIMA is a randomized, prospective, multicenter non-inferiority diagnostic accuracy trial in eight German hospitals. Biopsy-naïve men aged 50-75 years with PSA ≥ 3 ng/m

The spectrum of tonsillar involvement in chronic lymphocytic leukemia (CLL).

Tonsillar enlargement in chronic lymphocytic leukemia (CLL) either with or without symptoms has been rarely reported. We describe 22 cases of CLL with tonsillar involvement and demonstrate this is a CLL disease site that may be under-recognized in routine practice. Tonsillar involvement can be divided into three categories: (1) symptomatic CLL tonsillar involvement with upper airway obstruction (UAO) requiring therapy managed with: (a) tonsillectomy without systemic therapy when tonsillar enlargement is disproportionate to overall disease state, (b) tonsillectomy and systemic CLL therapy, and

A 58-Year-Old Woman's Odyssey With Bilateral Nodular Opacities Comes to a Diagnosis.

Benign metastasizing leiomyoma (BML) is an uncommon entity in which histologically benign smooth muscle tumors appear at distant sites, most frequently the lungs, in women with a history of uterine leiomyoma treated surgically. Despite its benign pathology, BML often presents radiologically in a pattern that mimics metastatic malignancy, resulting in substantial diagnostic uncertainty. Awareness of this condition is essential for avoiding unnecessary or overly aggressive interventions. We describe the case of a 58-year-old woman and active smoker with a significant 40-pack-year history who was

Beneath Sepsis, Lupus-Like Autoimmunity, and COVID-19: Diagnostic Eclipse in Fatal Intravascular Large B-cell Lymphoma.

Intravascular large B-cell lymphoma (IVLBCL) is a rare extranodal neoplasm in which malignant B cells proliferate predominantly within small blood vessels. Because lymphadenopathy, mass lesions, circulating malignant cells, and specific radiological findings may be absent, IVLBCL can closely mimic other medical conditions, resulting in delayed diagnosis. This case is notable for a prolonged culture-negative sepsis-like presentation with persistent unexplained hypoxemia due to predominant pulmonary microvascular involvement, further obscured by lupus-like autoimmune features and subsequent COVI

A case report of multifocal gastrointestinal myeloid sarcoma without leukemic involvement: an unusual presentation posing a diagnostic challenge.

Myeloid sarcoma is a rare hematologic malignancy defined by the extramedullary proliferation of myeloid blasts. It may occur without concurrent acute myeloid leukemia, creating a significant diagnostic challenge, particularly when it involves uncommon sites such as the gastrointestinal tract. We report the case of a 43-year-old woman with no relevant comorbidities who presented with a two-year history of epigastric pain and heartburn, later complicated by hematemesis. Imaging revealed a large gastric mass with associated hepatic focal lesions, initially raising suspicion for gastric adenocarci

Primary cerebral melanoma of the frontal lobe: a case report and review of the literature.

INTRODUCTION: Primary cerebral melanoma is an exceedingly rare intracranial neoplasm, accounting for a minuscule fraction of all brain tumors and melanomas. Originating from melanocytes within the central nervous system, its diagnosis is one of exclusion, requiring thorough investigation to rule out metastatic disease from cutaneous, uveal, or mucosal primary sites. CASE PRESENTATION: We present a 58-year-old male who presented with a recent onset of headaches and right lower limb weakness. Neuroimaging revealed a large, solitary, hemorrhagic mass measuring 5 × 5 cm in the left frontal lobe w

Craniofacial extramedullary plasmacytoma with hard palate and bilateral cervical lymph node involvement: a diagnostic challenge and literature review.

BACKGROUND: Extramedullary plasmacytoma (EMP) is a rare plasma cell tumor primarily affecting extramedullary sites without involving the bone marrow. While 80–90% of cases occur in the head and neck regions, craniofacial EMP involving the hard palate and spreading to bilateral cervical lymph nodes is exceptionally rare. CASE PRESENTATION: A 59-year-old female presented with a one-year history of a nasal mass on the right anterior side. Her medical history showed basal cell carcinoma and oropharyngeal tumors. Physical examination revealed a mulberry-like mass in the right nasal vestibule. Compu

Tonsillar T-cell lymphoma with hepatic and renal metastases in a fattening domestic pig (Sus scrofa domesticus).

BACKGROUND: In pigs, lymphoma is documented only sporadically in literature. Compared with other livestock species, such as cattle, lymphoma in pigs has a significantly lower incidence. The low number of reported cases may be due in part to underdiagnosis related to the production environment in which these animals are raised, together with the sporadic and non–virus-associated nature of the disease. Although rare, lymphomas represent the most frequent form of hematopoietic tumor in this species with different sites of development. To date, there are no clear reports of primary tonsillar lymph

Primary nasopharyngeal Kaposi's sarcoma without cutaneous manifestations: a case report.

BACKGROUND: Kaposi's sarcoma (KS) is a neoplastic lymphovascular disorder. It typically appears on the skin of the upper and lower extremities, but can also occur on the mucosal surfaces of the head and neck. The most common site reported for head and neck injuries is within the oral cavity, particularly on the palate, and other mucosal sites, such as the nasopharyngeal area. Moreover, involvement of the nasopharynx without skin involvement is even rarer. CASE PRESENTATION: Here, we present a rare case of HIV associated/epidemic KS in a 30-year-old man of African origin, where the primary man

Optimizing Biopsy Decisions in Benign Longitudinal Melanonychia: A Dermoscopic-Pathologic Study of Nail Matrix Nevus vs. Melanocytic Activation.

BACKGROUND: Longitudinal melanonychia (LM) is a common clinical manifestation of nail melanocytic lesions and poses diagnostic challenges in distinguishing benign melanocytic activation (MA) from melanocytic hyperplasia such as nail matrix nevus(NMN). OBJECTIVES: This study aimed to identify clinical, dermoscopic, and histopathologic features that distinguish NMN from MA in LM and facilitate biopsy decision-making. METHODS: We retrospectively reviewed 33 patients with LM (21 MA and 12 NMN) who underwent nail biopsy at our institution between 2019 and 2021. All cases underwent preoperative cl

Differentiating Pulmonary Nodule Malignancy Using Exhaled Volatile Organic Compounds: A Prospective Observational Study.

BACKGROUND: Advances in imaging technology have enhanced the detection of pulmonary nodules. However, determining malignancy often requires invasive procedures or repeated radiation exposure, underscoring the need for safer, noninvasive diagnostic alternatives. Analyzing exhaled volatile organic compounds (VOCs) shows promise, yet its effectiveness in assessing the malignancy of pulmonary nodules remains underexplored. METHODS: Employing a prospective study design from June 2023 to January 2024 at the Affiliated Hospital of Yangzhou University, we assessed the malignancy of pulmonary nodules
