Change in HbA1c and weight after initiation of antipsychotic treatment in people with severe mental illness in England: an individual-level interrupted time-series analysis.

BACKGROUND: Second-generation antipsychotics contribute to excess cardiometabolic morbidity in people with severe mental illness, but the timing of early glycaemic change and whether it parallels weight gain remain unclear. We aimed to evaluate glycated haemoglobin A1c (HbA1c) and weight trajectories before and after initiation of antipsychotic treatment. METHODS: We conducted an individual-level interrupted time-series analysis in England using electronic health records (EHR) from the Clinical Practice Research Datalink Aurum database between Jan 1, 2000, and June 7, 2023. Included participa

Diagnostic Decision Support and Automation in Vestibular Diagnosis.

This article examines the current and future role of decision-support systems and automation in vestibular diagnosis across emergency, primary care, and specialty settings. Vestibular disorders are common but often misdiagnosed because of overlapping symptoms, challenging bedside examinations, and limitations of neuroimaging. Structured diagnostic approaches improve accuracy when correctly applied. Emerging technologies offer objective assessment of eye movements and may enhance diagnostic reliability. Large language models provide valuable support for clinical reasoning, education, and inform

Validity of Cognitive Performance-Based Screening Tools for Older Adults: Dual Tasks Combining Cognition and Hand Function.

BACKGROUND: Early detection of cognitive decline in older adults is essential for timely interventions to preserve independence in instrumental activities of daily living (IADLs). Existing cognitive screening tools often lack ecological validity or require extensive administration. Dual-task paradigms combining cognitive and motor demands may provide a practical and sensitive alternative. OBJECTIVE: This study aimed to validate two dual-task measures integrating cognition and hand function as cognitive screening tools for older adults. METHODS: A total of 103 community-dwelling older adults

Cinnamic Acid Attenuates Ethanol-Exacerbated Hepatocyte Pyroptosis in Type 2 Diabetes via AMPK-Mediated NLRP3/GSDMD Inhibition.

Type 2 diabetes mellitus (T2DM) is frequently accompanied by chronic ethanol (ET) consumption, which worsens metabolic dysregulation and organ damage. Cinnamic acid (CA), a natural compound with anti-diabetic and hepatoprotective effects, shows therapeutic potential, but its role in mitigating ET-exacerbated hepatocyte injury in diabetes remains unclear. This study thus investigated whether CA protects against ET-induced hepatocyte pyroptosis under diabetic conditions and explored the underlying mechanisms. In vitro, HepG2 cells cultured under high-glucose (HG) conditions were exposed to ET to

Examining the relationship between anthropometric and body composition measures with coronary heart disease events: the multi-ethnic study of atherosclerosis.

AIMS: Obesity, traditionally measured using body mass index (BMI), is considered a significant risk factor for the development of cardiovascular disease (CVD). However, there have been mixed findings regarding the association between obesity and cardiovascular events in part due to the way it is measured. Major limitations exist with the use of BMI as it cannot assess body composition or fat distribution, and thus fails to account for the separate roles of muscle and fat mass (FM). The objective is to examine the relationship between various measures of body composition including FM, fat-free

Ambulatory and Unattended, Automated Office Blood Pressure in People With and Without HIV in Tanzania: A Cohort Study of Repeatability and Regression to the Mean.

Sub-Saharan Africa (sSA) bears a high burden of hypertension and HIV, yet blood pressure measurement often relies on single-day, conventional office readings, prone to measurement error. We assessed the short-term repeatability of unattended, automated office BP (uAOBP) and ambulatory blood pressure monitoring (ABPM), and whether reproducibility differs by HIV status in Tanzania. We analyzed data from the Mwanza HIV&CVD cohort study, using measurements from enrollment and 1-month later. Bland-Altman plots visualized mean differences and limits of agreement. Reproducibility of hypertension clas

Blood Pressure Management in the Inpatient and Acute Care Setting.

Existing recommendations for blood pressure goals and treatment accumulated from decades of clinical trials and observational studies in which a resting blood pressure was measured in an outpatient setting and participants were followed for years. Consequently, existing guidelines for hypertension do not address day-to-day blood pressure control in an inpatient setting. When a patient is admitted, home blood pressure-lowering medications are often held, and the unfamiliar environment, clinical testing, and noise stress patients and heighten blood pressure. In this review, we describe the natur

Lipoprotein(a) and atherosclerosis risk across estimated glucose disposal rate strata: evaluating synergistic predictive utility.

BACKGROUND: High lipoprotein(a) [Lp(a)] levels and insulin resistance (IR) are both established risk factors for atherosclerosis. Nevertheless, the interrelationship between these two factors and their combined effects on atherosclerosis remain poorly understood. This research aimed to explore the distribution characteristics of Lp(a) across individuals at varying IR degrees and to evaluate the combined ability of Lp(a) levels and the IR surrogate marker, estimated glucose disposal rate (eGDR), in predicting atherosclerosis. METHODS: This retrospective study involved data from 10,753 particip

Beyond BMI: the predictive value of body roundness index for cardiovascular mortality and related disparities: a national cohort study of US adults.

AIMS: Obesity, particularly visceral adiposity, is a key driver of cardiovascular mortality (CVM), yet conventional measures such as body mass index (BMI) inadequately capture fat distribution. The Body Roundness Index (BRI) is a novel anthropometric measure integrating waist circumference and height to better reflect body shape and visceral fat. This study examined the association between BRI and CVM in U.S. adults and across sociodemographic subgroups. METHODS AND RESULTS: We analysed data from 31 351 adults (median age 43 yrs, 50% females) aged ≥20 years in NHANES 1999-2018, excluding thos

Prognostic value of the neutrophil percentage-to-albumin ratio for mortality in patients with hyperuricemia and gout.

BACKGROUND: The neutrophil percentage-to-albumin ratio (NPAR) is a composite biomarker reflecting systemic inflammation and nutritional imbalance. Its prognostic significance for mortality in patients with hyperuricemia (HUA) or gout remains incompletely understood, and whether this association differs between asymptomatic HUA and gout has not been examined. METHODS: We utilized data from the National Health and Nutrition Examination Survey (NHANES, 1999-2018). After applying exclusion criteria, 4,344 adults were identified, comprising 3,659 with asymptomatic hyperuricemia and 685 with gout (
