Transcriptomic-based molecular classification of ampullary adenocarcinoma.

Ampullary adenocarcinoma (AMPAC) is a rare and heterogeneous malignancy with markedly variable clinical outcomes, underscoring the urgent need for molecularly informed classification and personalized therapeutic strategies. Current AMPAC classification relies primarily on morphological and immunohistochemical criteria, which lack prognostic accuracy and fail to capture the underlying molecular and tumor microenvironment heterogeneity. In this study, we integrated bulk RNA sequencing, single-cell RNA sequencing (scRNA-seq), and clinicopathological data to elucidate the molecular architecture of

Laparoscopic versus open pancreaticoduodenectomy for non-pancreatic periampullary adenocarcinomas: study protocol for a multicentre randomised controlled trial in China.

INTRODUCTION: Pancreaticoduodenectomy (PD) is the preferred surgical approach for resectable periampullary carcinomas. Non-pancreatic periampullary carcinomas (NPPC) demonstrate distinct biological behaviours and are generally less aggressive than pancreatic cancer. Despite over three decades of advancements, the role of laparoscopic PD (LPD) remains debated. Few studies have directly compared LPD and open PD (OPD) while accounting for tumour invasiveness, and the data specifically evaluating LPD vs OPD for NPPC are limited. Randomised controlled trials are necessary to determine whether thera

Lemmel Syndrome: A Case Report.

Lemmel syndrome is a rare and misdiagnosed cause of obstructive jaundice due to a periampullary duodenal diverticulum (PAD) in the absence of choledocholithiasis or a tumor. PADs are extraluminal outpouchings of duodenal mucosa that develop within a radius of 2-3 cm from the ampulla of Vater. They are largely asymptomatic, but rarely they can cause acute abdominal pain and obstructive jaundice due to the extrinsic compression of the common bile duct or pancreatic duct. PADs are mostly asymptomatic, but complications can occur, such as hemorrhage, perforation, diverticulitis, pancreatitis, chol

Characteristics and environmental susceptibility of first-pass meconium microbiota in neonates with congenital intestinal atresia.

PURPOSE: Whether congenital gastrointestinal atresia (atresia) specifically affects the meconium microbiota because of an altered intrauterine intestinal environment remains unclear. Therefore, we aimed to characterize the meconium microbiota of neonates with congenital anomalies, specifically atresia. METHODS: Meconium samples were collected from healthy term neonates (control), neonates with congenital malformations other than atresia (other anomalies), and those with atresia who were admitted to the growing care unit or obstetrics ward. Alpha-diversity (Shannon index and observed features)

Prognostic host phenotypes based on body composition and systemic inflammation predict survival in patients with resected pancreatic and periampullary cancer.

INTRODUCTION: Most present-day survival-models of patients with cancer do not account for tumor-host interactions. We hypothesized that host phenotypes based on systemic inflammation and body composition are prognostic of overall survival (OS) in patients with pancreatic ductal adenocarcinoma (PDAC). METHODS: We performed a post-hoc analysis of the nationwide PORSCH-trial, including all patients undergoing pancreatoduodenectomy for PDAC. Primary outcome was OS. Body composition analysis was performed using automatic segmentation (Mosamatic™) of preoperative abdominal computed tomography scans

Impact of pathological diagnosis on postoperative outcomes after pancreatoduodenectomy: A nationwide cohort study.

BACKGROUND: Benchmarking of surgical outcomes across institutions requires adjustment for established risk factors. Although pathological diagnosis is recognized as a determinant of postoperative morbidity after pancreatoduodenectomy, its independent impact on outcomes has not been systematically quantified at a national level. This study aimed to investigate the impact of pathological diagnosis on postoperative outcomes after pancreatoduodenectomy and to assess inter-hospital variation in case-mix. METHODS: Data were obtained from the nationwide Dutch Pancreatic Cancer Audit. Postoperative c

Evaluation of HER-2/neu status in metastatic lymph nodes synchronous with the primary tumor in periampullary region malignancies.

BACKGROUND: Key prognostic factors in periampullary tumors include tumor size, histopathological subtype, degree of differentiation, lymph node metastasis, perineural invasion, and lymphovascular invasion. Therefore, evaluating molecular markers alongside traditional histopathological parameters may improve the accuracy of prognostic classification. AIM: We seek to examine how HER2/neu status relates to clinicopathological features like T-stage, N-stage, and anatomical site to better define its potential as a clinical tool for managing periampullary tumors. METHODS: This study was conducted

Hodgkin lymphoma of the ampulla of Vater: A rare cause of obstructive jaundice in children.

Hodgkin lymphoma (HL) has a wide spectrum of presentation. Most cases affect lymph nodes (nodal), while extranodal involvement is rare. Whereas the gastrointestinal tract is enriched with lymphoid tissues, the ampulla of Vater is not rich in lymphoid tissue. Involvement of the ampulla of Vater with HL has rarely been reported in adults and has never been reported in children. Its presentation mimics other causes of obstructive jaundice, and immunodeficiency increases the risk of lymphoma. There have been reports of cutaneous T-cell lymphomas and one case of HL following the use of dupilumab, a

Endoscopic Papillectomy for Ampullary Tumors in Older Patients: Balancing Oncologic Benefit and Geriatric Risk.

As population aging accelerates, patients aged ≥ 75 years represent a growing proportion of those diagnosed with ampullary tumors. Endoscopic papillectomy (EP) is an established minimally invasive treatment for ampullary adenoma; however, treatment selection in older patients requires careful consideration of oncologic benefit, procedural risk, geriatric vulnerability, and life expectancy. This review outlines key geriatric assessment tools and evaluates the safety, prognosis, and treatment selection of endoscopic and surgical approaches for ampullary tumors in older patients. With appropriate

Long-term outcomes and recurrence patterns of endoscopic papillectomy for ampullary tumors: a 15-year experience from a large tertiary center in China.

BACKGROUND AND AIMS: Endoscopic papillectomy (EP) is an established treatment for ampullary tumors. This study aimed to analyze recurrence patterns and risk factors in a large cohort with long-term follow-up. METHODS: Patients who underwent EP for ampullary tumors at a large tertiary center from January 2010 to June 2025 were retrospectively reviewed. Cumulative recurrence rates were estimated using Kaplan-Meier analysis, and Gaussian kernel density estimation was employed to explore dynamic recurrence patterns. Cox proportional hazards model was used to identify risk factors for recurrence.

Rapidly Progressive Neuroendocrine Carcinoma of the Ampulla of Vater Detected During Intraductal Papillary Mucinous Neoplasm Surveillance.

A 77-year-old woman undergoing surveillance for an intraductal papillary mucinous neoplasm (IPMN) was found to have new enlargement of the major papilla on esophagogastroduodenoscopy. Contrast-enhanced computed tomography and endoscopic ultrasound (EUS) revealed an ampullary mass with multiple hepatic lesions. Because no tumor exposure was evident on the duodenal mucosal surface, endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB) was performed. Histopathological and immunohistochemical findings, including a high Ki-67 labeling index, confirmed the diagnosis of neuroendocrine carcinoma (

Recurrence Patterns of Resected Ampullary Adenocarcinoma and Implications for Multimodal Therapy: A Hidden Genome-Derived Analysis from the Ampullary Cancer Collaborative.

BACKGROUND: Ampullary adenocarcinoma is characterized by significant clinical heterogeneity that may preclude rational application of adjuvant therapies. The Hidden Genome Classifier defines distinct prognostic subtypes of ampullary adenocarcinoma. The impact of genomic subtype on recurrence patterns is unknown and may provide novel biomarkers to optimize treatment selection. METHODS: The previously validated Hidden Genome Classifier was applied to an international, six-institution cohort of 218 patients with resected ampullary adenocarcinoma. Genomic classification was correlated with patter

Successful pancreaticoduodenectomy for ampullary carcinoma in a patient with diabetes and stable chronic lymphocytic leukemia: a case report emphasizing multidisciplinary optimization.

BACKGROUND: Pancreaticoduodenectomy (PD) for ampullary carcinoma carries significant risks, and the presence of hematologic comorbidities adds further complexity. While stable Chronic Lymphocytic Leukemia (CLL) is not a contraindication, it complicates perioperative risk stratification and infection control. CASE PRESENTATION: A 66-year-old male with type 2 diabetes mellitus (DM) and stable, untreated Rai Stage I chronic lymphocytic leukemia (CLL) presented with a three-week history of painless obstructive jaundice. Imaging and biopsy confirmed ampullary carcinoma. INTERVENTION: Following mu

Large-Cell Neuroendocrine Carcinoma of the Ampulla of Vater: A Systematic Literature Review of a Rare and Aggressive Entity.

Neuroendocrine carcinomas of the ampulla of Vater are rare but highly aggressive, poorly differentiated neoplasms associated with poor outcomes. This contrasts with ampullary adenocarcinoma, which is generally associated with more favourable survival following curative resection, and with well-differentiated periampullary neuroendocrine tumours, which typically demonstrate more indolent behaviour and better prognosis. A Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)-compliant systematic review was conducted across PubMed/MEDLINE, Embase, Scopus, Web of Science, and
