OBJECTIVE: Age of symptom onset is highly variable in familial frontotemporal lobar degeneration (f-FTLD). Accurate prediction of onset would inform clinical management and trial enrollment. Prior studies indicate that individualized maps of brain atrophy can predict conversion to dementia in f-FTLD. We used a Bayesian linear mixed-effect (BLME) prediction method for identifying accelerated brain volume loss to predict conversion to dementia.
METHODS: Participants included 234 asymptomatic or prodromal carriers of C9orf72, GRN, or MAPT mutations (including 21 dementia converters) with ≥3 long

INTRODUCTION: High blood pressure (BP), functional decline, and cognitive decline commonly co-occur in late life. The temporal dynamics between changes in BP and changes in functional or cognitive status remain unclear. The aim of this study is to explore these temporal dynamics in adults aged 85 years and older using dynamic time warping (DTW).
METHODS: This study used data from the Leiden 85-plus Study. BP, functional status and cognitive function were measured at baseline (age 85) and annually over 5 years. Participants with at least three measurements were included (n = 429 of 599). Funct

INTRODUCTION: The Mini-Mental State Examination (MMSE) is widely utilized in clinical settings for cognitive screening, yet its diagnostic accuracy is often influenced by demographic factors such as educational attainment. This study investigates the educational gradient in MMSE performance and evaluates whether uniform cutoff scores adequately distinguish cognitively normal (CN), mild cognitive impairment (MCI), and Alzheimer's disease (AD) patients across different educational strata.
METHODS: A total of 300 older adults (CN = 100; MCI = 100; AD = 100) were retrospectively recruited from th

UNLABELLED: Depression and obesity are highly comorbid and likely involve common risk factors and pathophysiological mechanisms, which could crosslink to gut microbiome dysfunction. Here, we performed a case-control study with a total of 105 subjects, 43 with major depressive disorder (MDD) and 62 non-depressed controls free from psychiatric comorbidities, to identify gut microbiome signatures associated with MDD and dissect its relation to body mass index (BMI) and lifestyle (diet and exercise). We performed shotgun metagenomics, followed by taxonomic and functional annotations. Using differe

This study investigates the discordance between forensic psychiatric reports and judicial decisions in Brazilian civil and administrative litigation. Using a retrospective documentary design, we analyzed a random sample of 200 cases (50 per domain) from the Brazilian Electronic Judicial Process (PJe) across four areas: social security (INSS), public service examinations, access to medications, and alleged medical error. Overall concordance was high (87.5%), but 12.5% of cases showed discordance, which clustered significantly in public service examinations (20%) and medical error disputes (20%)

BACKGROUND: Borderline personality disorder (BPD) is a severe psychiatric condition marked by disturbances in self-image, affect regulation, and interpersonal functioning. Up to 54% of individuals with BPD experience psychotic symptoms, particularly auditory verbal hallucinations (AVH). While AVH's neural correlates have been studied in schizophrenia (SZ), their structural basis in BPD remains poorly understood within a transdiagnostic framework.
METHODS: This cross-sectional study used voxel-based morphometry (VBM) on MRI data to assess gray matter volume (GMV) in BPD patients with AVH (n =

BACKGROUND: With the acceleration of global aging, cognitive impairment has become a critical public health issue. However, evidence delineating the dynamic and multifactorial trajectories of its progression remains scarce. Early identification of modifiable lifestyle and clinical risk factors is therefore essential to enable timely and effective prevention and intervention.
METHODS: This retrospective cohort study included 6,462 older adults from Baisha Town, Hepu County, Guangxi, China, who underwent health examinations between 2019 and 2024. Cognitive function was assessed using the Mini-M

BACKGROUND: Although emerging evidence links glymphatic dysfunction to Alzheimer’s disease (AD), whether this association depends on structural brain changes remains unknown. In this study, we employed Diffusion Tensor Imaging Analysis along the Perivascular Space (DTI-ALPS) to assess the glymphatic alterations along the AD continuum in a Chinese cohort and explored whether these alterations are statistically mediated by gray matter (GM) or white matter (WM) atrophy as anchored by the ALPS-index. METHODS: We assessed glymphatic function using the ALPS-index in 200 participants with normal cont

Background: Moral injury (MI) is a clinical phenomenon that can emerge in the aftermath of engaging in or witnessing an event that transgresses an individual's deeply held moral beliefs. Research on MI assessments has yielded conflicting results regarding optimal structure, and there are very few comprehensive examinations of the clinical correlates of MI in diverse community samples. Objective: We examined psychometric properties of the Moral Injury Exposure and Symptoms Scale for Civilians (MIESS-C) and investigated clinical correlates of this measure. Method: In a community sample of N = 598

BACKGROUND: Physical activity (PA) has positive effects on physical and mental health. Nevertheless, two thirds of individuals with major depressive disorders (MDD) do not meet PA recommendations. Furthermore, MDD is associated with increased cardiovascular risk and autonomic nervous system dysfunction. This depression-related dysfunction is expressed by low heart rate variability (HRV). In contrast, regular PA has been shown to improve autonomic regulation over time as indicated by higher HRV values.
OBJECTIVE: The overarching aim of this umbrella review is to examine the complex interrelati

The monitoring of daily life in nursing home residents generates diverse and heterogeneous sources of information. Artificial Intelligence (AI) is increasingly used to predict a wide range of outcomes in both research and clinical practice, including mortality and cognitive impairment (CI). A key challenge is determining which information sources (IS) provide the most accurate predictions. In this work, we present an integrative AI-based framework that combines harmonized temporal modeling, Bayesian hyperparameter optimization, XGBoost, and explainable AI (SHAP) to predict CI in nursing home r

Previous studies have reported associations between nonrestorative sleep (NRS) and various physical and mental illnesses. However, its long-term relationship with subjective health and work performance remains unclear. This study aimed to investigate the association between NRS and subjective health and work performance among Japanese workers. A longitudinal cohort study was conducted using health examination and medical claims data from fiscal years 2018 to 2023, involving 36,160 workers. NRS was assessed at baseline based on responses to the question, "Do you feel refreshed after a typical n

BACKGROUND: Despite clear diagnostic criteria established in 2017, delivering and explaining the diagnosis of persistent postural-perceptual dizziness (PPPD) can be challenging.
METHODS: We outline a step-by-step approach to clearly explain the diagnosis, underlying mechanisms and treatment options to patients with PPPD, followed by a brief overview of current treatment approaches. This approach is adapted from the Cambridge-Calgary Consultation Model and previous recommendations for functional neurological disorders but is mainly based on our expertise accrued over decades and input from pat

BACKGROUND: Postoperative cognitive dysfunction (POCD) is a detrimental complication after surgery with lasting impact on patients' daily lives. It is most common after postoperative delirium. While dopaminergic dysfunction has been suggested to play a role in delirium, little knowledge exists regarding its relevance for POCD.
OBJECTIVE: We hypothesised that POCD is associated with altered resting-state functional connectivity (FC) of the ventral tegmental area (VTA) and the substantia nigra pars compacta (SNc) in functional magnetic resonance imaging (fMRI).

Timely detection of dementia is crucial for reducing its health and societal burden. Standard tools such as the Mini-Mental State Examination (MMSE) and Cognitive Abilities Screening Instrument (CASI), although widely used, are limited by time and resource demands. This study developed and validated a machine learning-based screening tool using the Six-Question Dementia Screening Test (6Q-DS), a brief interview of six items. Data from 533 older adults at a neurology clinic in Taiwan (331 with dementia, 202 without) were analyzed with eXtreme Gradient Boosting. The 6Q-DS achieved an AUC of 0.93

BACKGROUND: Digital health technologies offer new opportunities for cognitive screening and monitoring among older adults. In Thailand, where dementia prevalence is rising, accessible web-based cognitive tools remain limited despite their potential to facilitate early detection and community-based assessment. Understanding usability and validity is critical to ensure successful implementation in real-world contexts.
OBJECTIVE: This study aimed to develop and validate a web-based application, Healthy Brain Test, for cognitive and functional assessments in dementia screening among older Thai ad
