Whole Exome Sequencing Helps Diagnose Familial Anophthalmia in Zimbabwe: A Call from the Field to Fund Clinical Genomics for Planetary Health.

Anophthalmia is the most severe ocular malformation inherited in an autosomal, X-linked, recessive, or dominant form. We report here the use of whole exome sequencing (WES) to help the clinical diagnosis of familial anophthalmia in Harare, Zimbabwe. A mother presented her two sons, who are half-brothers, at the Eye, Ear, Nose, and Throat Institute, Ophthalmology Unit in Harare, Zimbabwe. Upon clinical examination, half-brothers were diagnosed with clinical bilateral anophthalmia. The mother requested a genetic diagnosis for her two sons. To segregate the phenotype with genotype, whole blood wa

Novel KMT2D pathogenic variant causing Kabuki Syndrome with associated macular abnormalities and retinopathy of prematurity.

BACKGROUND: Kabuki Syndrome (KS) is a rare multiple congenital anomaly syndrome originally described in 1981 by Japanese clinicians. KS belongs to the family of chromatinopathies, a group of disorders characterized by abnormalities in chromatin regulation due to germline mutations in the KMT2D or KDM6A genes. KS is characterized by five cardinal manifestations: (1) postnatal growth deficiency, (2) skeletal anomalies, (3) dermatoglyphic anomalies - including persistent fetal pads, (4) mild-to-moderate intellectual disability, and (5) typical facial features. PURPOSE: Ocular abnormalities have

Paediatric Presentations of Early-Onset Glaucoma and Stickler Syndrome: A Case Series.

INTRODUCTION: Stickler syndromes are multisystem connective tissue disorders characterised by a range of ocular and systemic features, including an increased risk of early-onset glaucoma. This report describes two paediatric patients initially assessed before the age of one who were ultimately diagnosed with both Stickler syndrome and infantile-onset glaucoma. Despite different genotypes, both shared similar phenotypes, most notably a strikingly unilateral glaucoma presentation. CASE PRESENTATIONS: Both patients presented with unilateral ocular hypertension and buphthalmus, followed by surger

Comparison of Event-based Analysis Versus Trend-based Analysis in the Detection of Glaucoma Progression by Optical Coherence Tomography 3-Dimensional Rim Measurements.

PRÉCIS: Compared with trend-based analysis, event-based analysis detects OCT structural progression in more patients and at an earlier time point. Using event-based analysis, MDB rim thickness detects progression more often than RNFL thickness. PURPOSE: To determine whether event-based or trend-based analysis best detects glaucoma progression using spectral domain optical coherence tomography (OCT) retinal nerve fiber layer (RNFL) thickness and minimum distance band (MDB) neuroretinal rim measurements. METHODS: Over 5 years, 124 open angle glaucoma patients had yearly dilated eye examination

Extended Survival in a Dog With Disseminated Protothecosis.

A 5-year-old Basenji Mix achieved extended survival with treatment for disseminated protothecosis. The dog originally presented with iridal hyperemia and ocular ultrasonographic examination revealed a mass effect. Histopathology revealed ocular protothecosis and staging confirmed disseminated protothecosis. Treatment with itraconazole and amphotericin B (AmB) infusions resulted in remission. Four years later, restaging revealed recurrence of infection in colonic biopsy samples. An extended AmB protocol was restarted for a cumulative dose of 84 mg/kg and the dog received isavuconazole based on

ACR Appropriateness Criteria® Vision Loss.

Vision loss is a common medical problem that can arise secondary to a variety of pathologies including trauma, ischemia, infection or inflammation, demyelination, and mass lesions anywhere along the visual pathway. Most nontraumatic pathology to the globe is diagnosed by ophthalmologic examination and does not require imaging. When needed, the choice of imaging study is directed at the known or suspected cause and the expected anatomic location of the insult along the visual pathway. This document focuses on the appropriateness of imaging in adults where vision loss is the primary presenting s

Central Corneal Thickness and Glaucoma Risk: The Importance of Corneal Pachymetry in Screening Adults Over 50 and Glaucoma Suspects.

Glaucoma is a leading cause of preventable blindness, yet nearly half of those affected are unaware of their diagnosis. Individuals with glaucoma may present with "normal" or "lower" intraocular pressure (IOP) compared to typical glaucoma thresholds, due in large part to many of these individuals having thin corneas. Conversely, many with elevated IOP and thicker corneas may not necessarily have glaucoma. In this article, we review the importance of central corneal thickness (CCT) and corneal pachymetry devices in eye care. Additionally, we review the role of corneal thickness as a risk factor

Cognitive Level Enhancement through Vision Exams and Refraction (CLEVER): study protocol for a randomised controlled trial.

BACKGROUND: Longitudinal observational studies have found an association between vision impairment and accelerated decline in cognition. However, no randomised trials have assessed the possible benefit of vision correction on cognitive change. We present the protocol for a three-year randomised controlled trial designed to assess the impact of spectacles for distance and/or near vision correction on cognitive change among community-dwelling elderly participants in India. METHODS: Cognitive Level Enhancement through Vision Exams and Refraction (CLEVER) is a single-centre, open-label, parallel-

Ocular Surface Granuloma Faciale: A Clinicopathologic Study of Two Novel Conjunctival Cases with a Review of the Literature Regarding Ophthalmic Involvement.

Granuloma faciale (GF) is a rare, idiopathic, localized vasculitis, usually without associated systemic autoimmune disease. Primarily involving the skin, GF can also occur on the ocular surface, mimicking neoplasia and other inflammatory processes. In this report, the authors report 2 novel cases of GF in the conjunctiva, and analyze the 4 published ocular cases to better appreciate the clinical and histopathologic features of GF involving the ocular surface, and to compare the findings with those in the skin. A pink-to-yellowish lesion of the superior bulbar conjunctiva in a 16-year-old femal
