Polygenic Determinants of Arterial Stiffness: Implications for Hypertension.

BACKGROUND: Components of arterial stiffness (structural stiffness from aging and load-dependent stiffness from high blood pressure [BP]) are markers for cardiovascular disease risk. We evaluated relationships between polygenic risk scores (PRSs) for BP and arterial stiffness. METHODS: Participants (n=5804) are from MESA (Multi-Ethnic Study of Atherosclerosis) with measures of carotid ultrasound and genomic DNA. Carotid pulse wave velocity (PWV) was calculated from carotid ultrasound. Structural stiffness was calculated by adjusting carotid PWV to a 120/80 mm Hg BP with participant-specific m

Rupture of Left Ventricular Aneurysm in Infant: Case Report.

Rupture of ventricular aneurysm is an unusual pathology in children. This case describes a preterm infant who had been in the hospital since birth due to necrotizing enterocolitis, which was complicated by narrowing of the ascending colon that required surgery. Throughout the entire period, the infant exhibited signs of infection, first caused by Staphylococcus haemolyticus and subsequently by Staphylococcus epidermidis. Several echocardiographic examinations did not reveal any significant changes; however, the final echocardiography revealed a structure connected to the left ventricular cavit

Diagnostic challenges in posterior circulation ischemic stroke presenting with acute dizziness and vertigo.

Posterior circulation stroke (PCS) is a common and clinically important cause of acute dizziness and vertigo, accounting for approximately 20-25% of all acute ischemic strokes. Because PCS frequently presents with non-specific, fluctuating, or isolated vestibular symptoms, it is about three times more likely to be initially missed in emergency care than anterior circulation stroke (ACS), with important consequences for acute treatment, secondary prevention, and patient outcomes. Diagnostic evaluation is further complicated by the low sensitivity of prehospital stroke scales for posterior fossa

Uncorrected Tetralogy of Fallot Complicated by Brain Abscess and Postoperative Intracranial Haemorrhage: A Case Report.

Cerebral abscess is a rare but potentially fatal complication of cyanotic congenital heart disease, in which chronic hypoxia, polycythaemia and right-to-left shunting allow bacteraemia to bypass pulmonary filtration. A woman in her 20s with uncorrected tetralogy of Fallot presented with severe headache, right-sided weakness and seizures. Examination showed hemiparesis, hypoaesthesia and hyperreflexia; blood tests revealed secondary polycythaemia and leucocytosis. No dental, sinus, otological or respiratory source was identified. Unenhanced computed tomography demonstrated a left parietal space

Dietary vitamin B1 intake and stroke risk in U.S. adults: Insights from a nationally representative health examination survey.

Stroke imposes a substantial global burden of mortality and long-term disability, and its occurrence is driven by diverse vascular risk factors. As an essential micronutrient, vitamin B1 (thiamin) has been implicated in cardiovascular protection, yet whether habitual dietary intake of this vitamin relates to stroke occurrence remains incompletely understood. The present study therefore examined the association between dietary vitamin B1 intake and stroke risk using nationally representative data from the National Health and Nutrition Examination Survey. This cross-sectional analysis included 1

The imperative for improved antihypertensives.

Hypertension remains a leading modifiable risk factor for cardiovascular morbidity and mortality worldwide, yet blood pressure (BP) control rates remain suboptimal despite the availability of multiple antihypertensive drug classes. This persistent gap reflects a combination of patient-related factors, including poor adherence, physician-related factors such as therapeutic inertia, health system limitations, and intrinsic shortcomings of current pharmacological therapies. Existing antihypertensive agents are effective at the population level but are limited by interindividual variability in res

Comparison of heart rate variability in normotensive and newly detected hypertensive adults in a tertiary care hospital, Mysuru, Karnataka.

Background and objectives Autonomic dysfunction plays a key role in blood pressure regulation, and heart rate variability (HRV) is a sensitive non-invasive marker of autonomic function. This study aimed to compare HRV among newly detected hypertensive and normotensive adults and to examine the association of HRV parameters with blood pressure levels. Methods This hospital-based case-control study included 52 participants (26 hypertensives and 26 normotensives). Time-domain HRV parameters, standard deviation of all normal-to-normal RR intervals (SDNN), and root mean square of successive differe

The challenge of introducing aldosterone synthase inhibitors into multidisciplinary practice.

Novel therapeutic agents are required to improve hypertension control globally, which remains one of the leading modifiable risk factors for premature cardiovascular mortality. Aldosterone synthase inhibitors (ASIs) are a novel class of agents that suppress aldosterone production and have demonstrated clinically meaningful blood pressure reductions in phase 2 and phase 3 trials in patients with uncontrolled and resistant hypertension. As these therapies approach clinical adoption, important questions remain regarding effects on cardiovascular and kidney outcomes, safety profile, and optimal po

From low renin to the primary aldosteronism spectrum: aldosterone synthase inhibitors and the endocrine management of hypertension.

BACKGROUND: Hypertension remains a major contributor to global cardiovascular morbidity despite decades of available therapy. AIM: This review advances an endocrinological framework in which chronic hypertension, including resistant hypertension, is understood as a consequence of dysregulated hormonal control of renal sodium handling, mediated by excess renin-angiotensin II activity, inappropriate aldosterone excess, or both. RESULTS: Within this framework, the traditional binary distinction between "essential" and "secondary" hypertension gives way to a mechanistic continuum characterized b

Aldosterone synthase inhibitors: biological rationale, mechanism of action, and results of clinical trials.

Resistant and uncontrolled hypertension remain major unmet needs, with aldosterone dysregulation or frank excess increasingly recognized as a central driver. To this end, aldosterone synthase inhibitors (ASIs) represent a novel therapeutic approach targeting CYP11B2, the key enzyme responsible for aldosterone biosynthesis. By reducing aldosterone production at its source, ASIs may overcome limitations associated with conventional renin-angiotensin-aldosterone system (RAAS) blockade and attenuate both genomic and non-genomic effects of aldosterone. Advances in drug development have led to secon

AI-Assisted Carotid Ultrasound for Nonexpert Use in Primary Care Cardiovascular Prevention: User-Centered Iterative Development Study.

BACKGROUND: Carotid ultrasound is traditionally confined to specialist settings because examination quality and diagnostic reliability are highly dependent on the operator's technical skill and experience. Recent advances in AI-assisted ultrasound may enable task shifting to primary care staff and support the integration of personalized visual risk communication into routine preventive care. Visualization of subclinical atherosclerosis has been shown to strengthen cardiovascular risk communication and support preventive engagement. However, limited evidence exists on how such technology can be

Novel Sensors and Data-Driven Approach to Support Prevention, Diagnosis, and Treatment Planning of Cerebrovascular Accidents: Protocol for a Multimodal Data Collection Study With Novel Sensors.

BACKGROUND: Stroke is the second leading cause of death worldwide. Cerebrovascular diseases (CVDs), including strokes and transient ischemic attacks (TIAs), cause long-term disabilities and economic burdens. Strokes can be ischemic, caused by blood clots; or hemorrhagic, caused by bleeding in the brain. Immediate diagnosis and treatment are crucial. Proper management of TIAs is essential due to the high risk of subsequent strokes. Currently, wearable sensors, AI-based prediction, and automatic video analysis are not used in CVD diagnosis and recovery estimation. OBJECTIVE: This paper describe
