Rabies in Europe: Epidemiology, Clinical Management, and Prevention.

BACKGROUND: Rabies is one of the oldest known zoonoses. In Europe, because cases are rare and medical experience is limited, there is often uncertainty about the indications and modalities of pre- and post-exposure prophylaxis (PrEP, PEP). METHODS: This review is based on studies of the epidemiology, diagnosis, treatment, and prophylaxis of rabies that were published from January 2000 to October 2025 and retrieved by a selective search in PubMed, supplemented by data from the WHO, CDC, ECDC, RKI, and FLI and the current recommendations of the German Standing Committee on Vaccination (STIKO).

Imported Human Rabies - Kentucky and Ohio, 2024.

Human rabies cases are rare in the United States; most result from domestic wildlife exposure. U.S. residents can acquire rabies abroad, typically through contact with dogs in areas where dog-maintained rabies is endemic. In November 2024, a man from Haiti was admitted to a Kentucky hospital with an 8-day history of progressive lower extremity pain and weakness. Soon after admission, he experienced hypersalivation, dysphagia, agitation, and eventually, respiratory failure requiring invasive mechanical ventilation. Ten days after admission, he was transferred to a referral hospital in Ohio, whe

Human Rabies Deaths - Minnesota and California, 2024.

Rabies is an enzootic viral disease in the continental United States and is typically transmitted through the bite of an infected mammal. Infection is almost always fatal if rabies postexposure prophylaxis (PEP) is not received before the onset of symptoms. Bats are the leading source of U.S. human rabies cases. In 2024, CDC identified two U.S. human rabies deaths in September (Minnesota) and November (California) in persons who had a recognized bat encounter but might not have been aware of the potential rabies risk. Neither patient reported the bat encounter to public health officials nor so

Rabies in a Captive Asian Elephant (Elephas maximus) in Bihar, India: First Confirmed Case With Ante- and Post-Mortem Diagnostics.

BACKGROUND: Rabies, a fatal zoonotic encephalitis, is rarely reported in elephants. OBJECTIVES: To present the first confirmed case of rabies in a captive Asian elephant in Bihar, India, using both ante-mortem and post-mortem diagnostics and discuss its implications for wildlife conservation and public health. METHODS: An 8-year-old female elephant exhibited acute neurologic signs including aggression, muscle spasms, anorexia and recumbency. Corneal smears and saliva samples were tested ante-mortem using the fluorescent antibody test (FAT) and a rapid diagnostic kit. Following the animal's d

Healthcare accessibility to rabies post-exposure prophylaxis in rural Kenya: implications for vaccine placement and travel time.

BACKGROUND: Rabid animal bites are a medical emergency, requiring immediate post-exposure prophylaxis (PEP) to prevent human deaths. This study modeled geographical accessibility to health facilities stocking rabies vaccines and assessed the impact of optimizing vaccine placement within a rural Kenyan healthcare network. METHODS: We used geocoordinates of Kenyan health facilities from an open-access inventory and identified those stocking PEP in Makueni County. Using population distribution data and a travel time friction surface, we estimated travel times to all health facilities, including

Human rabies cases in Northwestern Iran: Lessons on prevention, diagnosis, and public health response.

Rabies is a nearly universally fatal disease, yet it is preventable with timely post-exposure prophylaxis (PEP). In Iran, despite a comprehensive network of free rabies clinics, human fatalities continue to occur. This retrospective case series describes all laboratory‑confirmed human rabies deaths from Northwestern Iran in 2023 and analyzes the factors contributing to failures in post-exposure prevention. Each case involved high-risk animal exposure (fox bite, dog saliva to the face, severe dog bite) and a failure to seek immediate medical attention. Although vaccination was initiated after s

Human-animal contact and zoonotic exposure from wild and domestic animals: A cross-sectional study in wildlife-rich areas of Bolivia, Chile, and Guatemala.

INTRODUCTION: Human-animal contact (HAC) is a key driver of zoonotic disease emergence and transmission, yet population-based data from Latin America remain limited. The objective of this study was to characterize HAC patterns and estimate zoonotic exposure in wildlife-rich settings across Bolivia, Chile, and Guatemala. METHODS: A cross-sectional questionnaire was administered to 2389 participants in rural, urban, and protected areas to assess direct, indirect, and foodborne contact with domestic and wild animals. Patterns of HAC were visualized using heatmaps, and a proxy-based exposure inde

Perceptions and public health risks of the bat-human interface in households from fragmented rural landscapes in southern Chile.

Land-use change and habitat fragmentation in southern Chile have favored synanthropic bat species, promoting their contact with domestic animals and humans, representing a potential risk of exposure. This study aimed to characterize dwellings with bat colonies in rural zones of the Los Ríos and Los Lagos regions, estimate the frequency of human-bat contact, and determine the health risk perception from residents. A cross-sectional descriptive study was conducted in 69 rural dwellings. A structured survey was used to gather information on demographics, household characteristics, frequency of co

Pathogens Associated with Domestic Cats (Felis catus), Their Public Health Impact on Children, and Implications of Urban Management.

Domestic cats (Felis catus) are ubiquitous companion animals that provide substantial psychological and social benefits to children and adults alike, but they also serve as reservoirs and vectors for a wide range of zoonotic pathogens. Close physical contact between cats and children, frequent use of shared environments such as homes, playgrounds, and sandboxes, and still-developing hygiene behaviours increase opportunities for exposure to protozoa, helminths, bacteria, fungi, and ectoparasite-borne agents. This review synthesizes current evidence on key feline-associated zoonoses of pediatric

Eschars and estate fever: A case series from the Johorean oil palm heartland highlighting the changing face of scrub typhus in Malaysia.

Scrub typhus, a zoonotic bacterial infection caused by the arthropod-borne, Gram-negative, obligate intracellular bacillus Orientia tsutsugamushi and transmitted by larval chiggers, is an underdiagnosed cause of acute undifferentiated febrile illness. Its clinical spectrum ranges from a self-limited denguelike illness to rapidly progressive multiorgan dysfunction driven by smallvessel vasculitis, including pneumonitis, acute kidney injury, hepatitis, encephalitis and circulatory shock. Early recognition is frequently missed because the characteristic eschar may be absent or easily overlooked i

A cross-sectional survey to explore healthcare providers' experiences and attitudes toward HIV pre-exposure prophylaxis for women in family planning centers of Greater Paris.

Despite representing a disproportionately high percentage of new HIV diagnoses in France annually, women who have migrated from Sub-Saharan Africa (WMSSA) remain underserved by HIV prevention strategies, including Pre-Exposure Prophylaxis (PrEP). This study aimed to understand healthcare providers' experiences and attitudes toward PrEP delivery to WMSSA within family planning centers (FPCs) of the Paris region in France. We conducted a web-based cross-sectional survey from February to June 2024 to explore the knowledge, attitudes, and experiences of providers in FPCs in Paris and Seine-Saint-D

Considerations of HIV PrEP Among Heterosexually Active Women and Men: Results from a Qualitative Study in New York City.

Although 22% of new HIV diagnoses in the USA are attributed to heterosexual contact, uptake of pre-exposure prophylaxis (PrEP) remains low among heterosexually active individuals and information about PrEP considerations in this population is limited. We report on a cross-sectional qualitative study exploring attitudes toward PrEP among a diverse sample of 50 heterosexually active cisgender adults (31 women, 19 men) in New York City. Approximately, two-thirds (34) had previously heard of PrEP, but none had used it. Many learned about PrEP through marketing and social interactions, leading them

Cigarette smoking in PrEP recipients: a challenge and an opportunity.

The increase in HIV pre-exposure prophylaxis (PrEP) uptake presents both challenges and opportunities. The process of initiating and continuing PrEP brings the medical care establishment into direct and ongoing contact with a young population that is often otherwise unengaged with health-care systems. The groups at highest risk for HIV acquisition, including PrEP recipients, have high rates of tobacco use, which remains the leading cause of preventable death in the world. However, there has been little attention paid to tobacco use and cessation treatment in the context of PrEP care. PrEP guid

Lessons Learned from Community Contact Tracing of Women Disengaged from an Urban Prevention of Mother-to-Child Transmission Program in Uganda.

PURPOSE: Community tracing for disengaged pregnant and post-partum women with HIV(PPWH) shows promise, yet few studies detail the practical challenges of tracing, particularly in the Prevention of Mother-to-Child Transmission programs (PMTCT). Standard guidelines and evidence-based approaches remain lacking. We share lessons from our experiences in tracing disengaged PPWH in an urban PMTCT program in Uganda, where nurse counselors joined outreach teams to trace lost PPWH women via calls and home visits. DESCRIPTION: The parent study was a prospective case–control study conducted in Uganda at s

Prison-based HIV partner notification in Indonesia: rationale and design of a hybrid type 1 effectiveness-implementation study.

BACKGROUND: With HIV Assisted Partner Notification (APN), individuals with HIV voluntarily name and authorize trained health care workers to inform their at-risk partner of possible exposure to the virus without disclosing the identity of who had named them. Although APN is safe and effective in increasing HIV partner notification and testing, its successful implementation in prison settings to assist incarcerated people with HIV (IPWH) to inform their pre-incarceration partners of possible HIV exposure has yet to be demonstrated. Set in Indonesia where many people with HIV are diagnosed durin
