Differential effects of PM2.5 constituents on tuberculosis infection and disease progression in 40,379 student close contacts.

RATIONALE: Outdoor PM2.5 has been increasingly implicated in tuberculosis, yet evidence has largely focused on total PM2.5 mass and has usually considered Mycobacterium tuberculosis (M. tuberculosis) infection and active tuberculosis separately. Whether specific PM2.5 chemical constituents differ in their associations with infection and subsequent progression to active disease remains unclear. OBJECTIVES: To assess the associations of PM2.5 and five major constituents with M. tuberculosis infection and subsequent progression to active tuberculosis within the same high-risk population. METHOD

Upper respiratory tract commensal Streptococcus oralis inhibits mycobacterial growth by producing bioactive molecules and inducing macrophage autophagy.

BACKGROUND: Tuberculosis (TB) is caused by Mycobacterium tuberculosis (Mtb) and remains a major global health challenge. Although host immunity is critical for controlling infection, the role of upper respiratory tract commensal microbiota in anti-mycobacterial defense remains unclear. This study investigated whether commensal bacteria from close contacts of patients with TB, who remained persistently tuberculin skin test- (TST) and QuantiFERON-TB (QFT) Gold-negative after long-term exposure, produce factors capable of inhibiting mycobacterial growth. METHODS: Throat swabs were collected from

Microbiota as a biomarker of tuberculosis treatment response: association with clinical history, inflammation and outcome.

Tuberculosis requires prolonged multidrug therapy. Whether this disrupts the microbiota in ways reflecting previous treatment exposure and outcome is unknown. We performed 16S rRNA gene sequencing on sputum and stool from 107 people with tuberculosis (cases) in a longitudinal cohort, sampled at baseline, month 2, end-of-treatment (month 6), and months 12 and 18, and ≤2 close contacts per case (n=180) at baseline, months 6 and 18. Serum cytokines were quantified in cases (baseline, months 6 and 18) and contacts (baseline). Treatment outcome was defined by month 2 culture-conversion and month 6

U.S. Health department roles and challenges in conducting contact evaluation for tuberculosis aircraft contact investigations.

OBJECTIVES: To describe the experiences of U.S. tuberculosis (TB) program officials participating in contact investigations after TB exposures on aircraft in order to 1) characterize barriers to participation and 2) better understand reasons for low return of outcomes data. METHODS: We surveyed 51 TB professionals representing 47 jurisdictions about their experiences participating in TB aircraft contact investigations coordinated by the U.S. Centers for Disease Control and Prevention (CDC). RESULTS: Most jurisdictions (43/47, 91%) reported that they attempt an evaluation for every contact in

Series 1: Behind the Spread: A Scoping Review of Risk Factors for Exposure to Mycobacterium tuberculosis.

BACKGROUND: Tuberculosis (TB) remains a major global health challenge, with transmission influenced by the incidence of contagious people with TB, the duration of infectivity, and the probability of contact with susceptible individuals. This review synthesizes recent evidence on established and emerging risk factors influencing TB transmission, particularly in light of global trends such as migration, urbanization, and demographic shifts, to guide future prevention and control strategies. This scoping review maps and synthesizes evidence from systematic reviews on risk factors for Mycobacteriu

Novel Genetic Locus Associated with Resistance to M. tuberculosis Infection: A Multi-Ancestry Genome-Wide Association Study.

Understanding host susceptibility to Mycobacterium tuberculosis ( Mtb) is critical for the development of new vaccines. Certain individuals "resist" becoming infected with Mtb despite intensive exposure; however, it is unknown whether there is a genetic basis for "resistance" to Mtb infection across populations. Here we conducted a genome-wide association study (GWAS) of resistance to Mtb infection by carefully characterizing exposure to TB patients among 4,058 close contacts in India, Brazil, and South Africa. 476 (12%) "resisters" remained free of Mtb infection despite substantial exposure t

Missed Opportunities in Tuberculosis (TB) Prevention in Pediatric Population: A Retrospective Observational Study From North India.

Background Contact investigation, particularly Household contact tracing (HHC), is a cornerstone of tuberculosis (TB) control programs globally. However, the implementation of contact tracing and TB preventive treatment (TPT) in India remains suboptimal, especially in the pediatric age group. This study aimed to determine the missed opportunities of disease identification and prevention among pediatric TB cases with a documented history of household exposure to a TB index case. Primary objective To determine the proportion of microbiologically confirmed pediatric TB cases with documented house

Pulmonary versus extrapulmonary tuberculosis among diagnosed tuberculosis cases in Bangladesh: Evidence from a systematic review and meta-analysis.

BACKGROUND: Tuberculosis remains one of the primary infectious diseases in Bangladesh that affects both adults and children. Poor diagnostic criteria and insufficient reporting, especially of pulmonary tuberculosis (PTB) and extrapulmonary tuberculosis (EPTB) in children, limit the prevalence estimation, resulting in insufficient preventive strategies. This systematic review and meta-analysis aim to estimate the pooled proportions of PTB and EPTB among pediatric and adult diagnosed tuberculosis cases in Bangladesh, and to summarize the associated factors. METHODS: We searched PubMed, Scopus,

Series 2: Invisible Threats: A Global Scoping Review of Risk Factors for Tuberculosis Infection.

BACKGROUND: Tuberculosis (TB) remains a major global health challenge, with Mycobacterium tuberculosis (M. tuberculosis) causing significant morbidity and mortality mainly in high-burden countries. Following exposure to M. tuberculosis, individuals may become infected, developing TB infection (TBI) through inhalation of the bacillus: this affects approximately one-fourth of the global population and serves as a critical reservoir for potential disease reactivation and transmission. The risk of being infected with M. tuberculosis is shaped by bacterial load of people with TB, contact patterns,

Improving Bovine Tuberculosis Surveillance Through Risk-Based Prioritization of Slaughterhouse-Triggered Trace-Back Investigations.

Slaughterhouse detection of lesions compatible with bovine tuberculosis represents a key passive surveillance component in Santa Catarina, Brazil, yet subsequent trace-back investigations often fail to identify infected farms. This study developed a quantitative framework to prioritize epidemiological investigations by estimating the probability of infection associated with each farm connected to PCR-confirmed cases. Using official movement records and historical diagnostic data, we reconstructed the lifetime contact networks of slaughtered cattle presenting confirmed Mycobacterium bovis lesio

Tuberculosis in households with infectious cases in Kampala city: Harnessing health data science for new insights on an ancient disease with persistent, unresolved problems (DS-IAFRICA-TB) Study Protocol.

Tuberculosis (TB) is prevalent in Uganda and overlaps with a high rate of HIV/TB coinfection. While nearly all hospital-based TB cases in Kampala, the capital of Uganda, show clear TB symptoms, 30% or more of undiagnosed TB cases found through active screening are asymptomatic. Additionally, the host risk factors for TB in Kampala cannot be distinguished from environmental risk factors. These TB-specific challenges are just part of the complexity, especially in areas with high HIV/AIDS burden. Data science techniques, especially Artificial Intelligence (AI) and Machine Learning (ML) algorithms

Nosocomial patient exposure to a healthcare worker with active tuberculosis in a paediatric setting: a contact investigation.

Transmission of tuberculosis (TB) from healthcare workers (HCWs) to patients is rare, especially in facilities with robust TB control programmes. We describe a TB contact investigation in which an HCW with TB disease was identified and had 233 paediatric patient contacts. Two medium-risk contacts had a positive TB test but no disease. Further research is needed to refine definitions for paediatric exposures by a HCW with TB disease in healthcare settings to reduce unnecessary testing.

Risk of progression to pulmonary tuberculosis among household contacts with chest radiographic abnormalities in South Africa.

BACKGROUND: High-risk subgroups among household contacts of persons with tuberculosis (TB) might benefit from additional interventions. However, the significance of an abnormal baseline chest radiograph (CXR) suggestive of TB, despite negative sputum microbiology, is uncertain. METHODS: Adults (≥18 years) with recent household TB exposure were enrolled at three South African sites (April 2021-September 2022). All participants underwent symptom screening, CXR, and sputum Xpert Ultra and MGIT culture. Pulmonary TB diagnosis was microbiologically-confirmed. Participants were followed for symptom

Climate change and tropical medicine in the Amazon: contributions from a research center, gaps and future priorities.

The Amazon Basin is undergoing rapid climatic and environmental changes, with direct impacts on infectious disease transmission and public health. Rising temperatures, altered rainfall patterns, and more frequent extreme events are reshaping vector and pathogen dynamics. Additionally, deforestation and land-use change increase human and animal exposure. These combined factors intensify the risk of disease transmission in the region. This narrative review synthesizes evidence generated by researchers from a reference center in tropical medicine in the Amazon, integrating epidemiological analyse

Prevalence and Associated Clinical Factors of Tuberculosis Infection and Disease Among Pediatric Household Contacts: A Facility-Based Cross-Sectional Study.

INTRODUCTION: Household contact screening is a key strategy for early detection of tuberculosis (TB) disease and infection in children exposed to microbiologically confirmed pulmonary TB. The National Tuberculosis Elimination Program data forms the vital foundation for national policy decisions. Complementary facility-based evaluations remain valuable for capturing region-specific nuances, particularly in geographically challenging areas. This study aimed to estimate the prevalence of TB disease and TB infection among pediatric household contacts and assess associated demographic and clinical

Factors associated with drug-resistant tuberculosis among patients at a tertiary referral hospital in Tanzania.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) undermines global TB control, particularly in high-burden African settings, with high HIV co-infection rates. While Tanzania is among the 30 high-burden TB countries, data on resistance patterns at tertiary referral hospitals remain limited. We aimed to determine the prevalence and factors associated with drug-resistant TB among patients at Muhimbili National Hospital (MNH) in Tanzania. METHODS: We conducted a retrospective cross-sectional study of 170 TB patients managed at MNH between July 2024 and July 2025.Clinical and demographic data

Response to active tuberculosis within a maternal newborn unit-military treatment facility, Alaska, April-August 2025.

Tuberculosis (TB) disease exposure within maternity units is a significant concern due to increased risk of infection among pregnant and post-partum women and their newborns. During April-August 2025, personnel from several military health authorities were deployed to assist a military treatment facility in Alaska after a labor and delivery nurse was diagnosed with pulmonary TB disease. A total of 282 individuals were identified through contact tracing. Staff contacted the affected families, answered questions, and facilitated evaluation and treatment. 263 (92.9%) of the 283 contacts were test

Context-dependent risk stratification using a tuberculin skin test induration threshold of ≥ 15 mm in school tuberculosis screening in Hangzhou, China: a historical cohort study.

BACKGROUND: Larger tuberculin skin test (TST) reactions are associated with higher tuberculosis (TB) risk, but the operational value of the ≥ 15 mm threshold among adolescents who already have a TST induration ≥ 10 mm during routine school screening is less well characterised in high-BCG coverage settings. We assessed whether TST induration ≥ 15 mm provided incremental risk stratification information for secondary school students and interpreted this threshold across routine screening, outbreak contact and previously cured TB contexts. METHODS: We conducted a historical cohort study using sch

Public transport: the missing link in tuberculosis control in Somalia.

Tuberculosis (TB) remains a major public health challenge in Somalia, where conflict, displacement, delayed diagnosis, and fragile healthcare systems sustain ongoing transmission. While existing TB control efforts largely emphasize household and healthcare-associated exposure, the role of overcrowded public transport systems has received limited attention. In rapidly urbanizing Somali cities, particularly Mogadishu, minibuses and shared taxis represent enclosed, poorly ventilated, high-turnover environments that may facilitate airborne transmission of Mycobacterium tuberculosis. Evidence from
