Delayed-type hypersensitivity, contact sensitivity, and phytohemagglutinin skin-test responses of heat- and cold-stressed calves.

Three-week-old Holstein bull calves were used to investigate the effect of a 2-week chronic heat (35 C) or cold (-5 C) exposure on delayed-type hypersensitivity (DTH) reactions to purified protein derivative after sensitization with heat-killed Mycobacterium tuberculosis, contact sensitivity (CS) reactions to 1-fluoro-2,4-dinitrobenzene, and phytohemagglutinin (PHA) skin tests. Heat exposure reduced expression of DTH reactions by 42% and CS reactions by 38% at 24 hours after elicitation of the responses. The PHA-induced skin tests were not affected after 1 week of heat exposure, but this react

Tuberculin skin test conversion from exposure to contaminated pulmonary function testing apparatus.

Tuberculosis is a prevalent infectious disease that is transmitted primarily from man to man by droplet nuclei. This report describes an outbreak of tuberculosis infection in a hospital in which 8 persons--7 hospital employees and 1 patient--had documented conversion of intermediate strength tuberculin tests from negative to positive in a period of about 10 weeks. The 7 employees constituted 18% of those selected for skin testing because of close contact with a patient who had newly discovered smear-positive pulmonary tuberculosis. The one patient converter had been tested on pulmonary functio

Clinical determinants of tuberculosis screening.

Because of declining prevalence of tuberculosis in the United States, standard laboratory tests detect fewer actively infected patients. Of 6,884 cultures for Mycobacterium tuberculosis (MTB) in three years at our hospital, only 2% were positive. To select other effective screening variables, we retrospectively reviewed patients in whom cultures were ordered. Charts of 79 patients with active tuberculosis and 226 patients whose cultures were negative were reviewed for 45 signs, symptoms, and laboratory tests determined at the time of initial contact. Those variables which distinguished the MTB

How environmental mycobacteria may predetermine the protective efficacy of BCG.

A proposal is made that there are 2 mechanisms of cell mediated response to mycobacteria, both of which produce positive tuberculin tests and that one of them is more protective against mycobacterial infection than is the other. These are referred to respectively as the Listeria-type and the Koch-type of responses. Contact with environmental mycobacteria will induce one or other of these types of response and BCG vaccination will enhance it. Thus in those places where the environmental species prime for the Listeria-type of response subsequent BCG vaccination will afford good protection from b

The effect of two distinct forms of cell-mediated response to mycobacteria on the protective efficacy of BCG.

The previous paper presents evidence that prior exposure to some environmental mycobacteria enhances the protective efficacy of BCG, whereas exposure to other species opposes it, and suggests that these different species act by evoking one of two types of cell-mediated response of different protective efficacy. This paper reviews past evidence for the existence of these two types of response, and suggests that both can be demonstrated in mice. The type of response evoked in mice by environmental species correlates with their effect on the efficacy of BCG in man, and with the type of proliferat

Transmission of multiple drug-resistant tuberculosis: report of a school and community outbreak.

An outbreak of tuberculosis in 1976 was caused by mycobacteria resistant to isoniazid (INH), streptomycin (SM), and para-aminosalicylic acid (PAS). High rates of infection associated with exposure to the index case suggested that transmission of resistant organisms had occurred, and the subsequent appearance of bacteriologically proven INH-SM-PAS-resistant tuberculosis in four school contacts of the index case confirmed this fact. Retrospective investigation revealed that the school outbreak was part of an ongoing community outbreak dating back at least to 1964. Through the use of case histori

Tuberculosis surveillance of hospital personnel.

Tuberculosis continues to present a problem of exposure for both patients and personnel in the general hospital. While not frequent, it is not uncommon and often is initially unsuspected, resulting in significant delay in diagnosis and the possibility of many potential contacts. We have reviewed our experience with tuberculosis surveillance at the Ohio State University Hospitals with the following conclusions:(1) Skin testing is a cost-effective means of surveillance of personnel, but requires proper interpretation. (2)Apparently high conversion rates may result from an undocumented history of

Outbreak of tuberculosis after minimal exposure to infection.

The identification of a case of respiratory tuberculosis in a swimming-baths attendant whose sputum was smear positive was followed by intensive contact tracing of children aged 8-11 years who had visited the baths. An outbreak was discovered that otherwise might not have been detected. Out of 3764 children, 108 (2.9%) had evidence of infection: there were 16 cases of tuberculosis, of which 11 were symptomless but showed lesions on chest radiography, and a further 92 with tine test grade 3 or 4 without clinical or radiological signs. The contact of these children with the index case was appare

Inhibition of Mycobacterium intracellulare by some vitamin K and coenzyme Q analogues.

Because vitamin K is present in several species of mycobacteria, and because coenzyme Q has been found to stimulate the growth of Mycobacterium tuberculosis and atypical mycobacteria, 8 quinones that can be considered to be dual analogues of vitamin K and coenzyme Q were tested for their activity against Mycobacterium intracellulare. One compound, 6-cyclo-octylamino-5,8-quinolinequinone (CQQ) exhibited considerable activity in vitro against several strains of Mycobacterium intracellulare at a concentration of 8 micrograms per ml. It has also shown evidence of bactericidal activity against grow

A study of a standardised contact procedure in tuberculosis. Report by the Contact Study Sub-Committee of The Research Committee of the British Thoracic Association.

A standardised procedure in contacts of patients with all types of newly diagnosed tuberculosis was undertaken by the British Thoracic Association Research Committee to assess the present relevance of contact examination in Britain. The results suggest that the tracing and examination of contacts remains a worthwhile procedure, resulting in the detection of significant numbers of previously unsuspected cases of tuberculosis in the contact population. The yield of new cases of tuberculosis is very similar for the Asian and non-Asian (mainly British) communities, namely 3.4% and 3.6% respectivel

Miliary tuberculosis in two infants after nursery exposure: epidemiologic, clinical, and laboratory findings.

Two infants born 4 days apart, and cared for in the same nursery, developed miliary tuberculosis at 2.5 and 5.5 months of age, respectively. A search for a source case among household contacts failed to reveal anyone with active tuberculosis. An epidemiologic study of nursery personnel revealed a nurse's aide with cavitary tuberculosis. Examination of 1,647 infants (98.9 per cent of all infants cared for in the nursery during the 10.5-month period preceding the discovery of the source case) failed to reveal any other infected children. Recommendations are made for tuberculosis control programs

Failure of isoniazid prophylaxis after exposure to isoniazid-resistant tuberculosis.

We report the failure of isoniazid chemoprophylaxis to prevent the development of active pulmonary tuberculosis and tuberculin conversion in contacts of patients infected with isoniazid-resistant strains of Mycobacterium tuberculosis. Although failure of isoniazid chemoprophylaxis has been reported rarely in contracts of isoniazid-resistant tuberculosis, the cases reported here as well as previous observations by others suggest that isoniazid chemoprophylaxis for contracts of patients with isoniazid-resistant tuberculosis may not be effective and that other agents must be considered. Tuberculosis notwithstanding BCG vaccination.

In a series of 176 patients, 40 children (23%) developed tuberculosis notwithstanding BCG vaccination. The type and severity of the disease in this group were similar to those of the larger series of which this group was a part. However, BCG vaccination appeared to diminish the incidence of dissemination, especially tuberculous meningitis, and the number of deaths. The 40% incidence of tuberculous contacts probably indicates heavy exposure to infection. BCG offers a modicum of protection which the vulnerable should not be denied, but its use should not lead to complacency--the potency of vacci

Preventive treatment of tuberculin-negative contacts.

Five children who had been in close contact with highly infectious tuberculous individuals presented recently to the tuberculosis service of The Montreal Children's Hospital. Four had developed serious pulmonary tuberculosis and one tuberculous meningitis, all within the three months which followed a post-contact negative tuberculin test. The management of tuberculin-negative children recently exposed to active infectious tuberculosis by repeated skin tests and chest radiography alone is inadequate for their protection. These children are at high risk of developing disease by the time their tub

Attack rate of tuberculosis in a 5-year period among close family contacts of tuberculous patients under domiciliary treatment with isoniazid plus PAS or isoniazid alone.

This report from the Tuberculosis Chemotherapy Centre, Madras, considers the risk, over a 5-year period, to close family contacts of sputum-positive patients treated at home for 1 year with a standard regimen of isoniazid plus PAS or one of 3 regimens of isoniazid alone. The attack rate of tuberculosis in the contacts did not appear to be influenced by the treatment received by the patients in the first year or by the duration in the 5-year period for which the patients had (1) positive sputum smears, (2) positive cultures, or (3) isoniazid-resistant cultures. Further, over half the cases of t

In vitro effect of rifampin on mycobacteria.

Rifampin inhibited 20 strains of Mycobacterium tuberculosis in concentrations of 0.005 to 0.02 mug/ml in 7H-9 broth with Tween 80 and killed all or nearly all of the inoculum in four to eight times greater concentrations. In the same medium without Tween 80, as well as on 7H-10 agar, about 16 to 64 times these amounts were required to produce the same effect. Rifampin was also active against M. kansasii and some of the nonchromogenic mycobacteria. The incidence of mycobacterial cells resistant to rifampin within the cultures studied was in the range of one to four per 10(8) to 10(9) colony-for

A controlled study of the influence of segregation of tuberculous patients for one year on the attack rate of tuberculosis in a 5-year period in close family contacts in South India.

This report is the last of a series of nine publications from the Tuberculosis Chemotherapy Centre, Madras, concerning various aspects of an investigation of the role of ambulatory chemotherapy for pulmonary tuberculosis. It presents the attack rates of tuberculosis over a 5-year period of follow-up of close family contacts of patients, all of whom were treated for one year with isoniazid plus PAS, half (selected at random) in sanatorium and half at home. The incidence of active tuberculosis and of tuberculous infections was no greater in the contacts of patients treated at home than in the co

Influence of segregation to tuberculous patients for one year on the attack rate of tuberculosis in a 2-year period in close family contacts in South India.

The authors present a second report from the Tuberculosis Chemotherapy Centre, Madras, on the incidence of tuberculosis in close family contacts of tuberculous patients. The patients initially received a year's chemotherapy either at home or in sanatorium in a controlled comparison of the merits of domiciliary as opposed to institutional treatment. The first report presented data relating to the prevalence and the attack rate of tuberculosis among the contacts during the first year of treatment of the index cases; this second report presents the attack rate for the 2-year period since the star
