An understanding of the neural control of lower oesophageal sphincter (LOS) relaxation is clinically relevant because transient LOS relaxations (TLOSRs) are a mechanism of acid reflux into the oesophagus. Preganglionic motor neurones innervating the LOS are localized in the dorsal motor nucleus of the vagus (DMV). Based on a single study in cats, it is now widely accepted that these neurones are functionally organized into two separate populations, such that stimulation of the caudal and rostral DMV evokes LOS relaxation and contraction, respectively. Our goal was to map the functional LOS res

BACKGROUND: Tuberculosis (TB) remains a leading cause of morbidity and mortality among children in low- and middle-income countries, particularly in sub-Saharan Africa. Paediatric TB is often underdiagnosed due to nonspecific symptoms, limited access to diagnostic tools, and its tendency to mimic other common childhood illnesses. In Zambia, where TB and HIV are co-endemic, understanding the burden of childhood TB at the sub-national level is critical for tailoring effective interventions. This study aimed to determine the prevalence and identify key predictors of TB among children aged 0–14 ye

OBJECTIVES: More than 20% of rheumatoid arthritis (RA) patients have comorbid fibromyalgia (FM+), which may elevate DAS28-ESR (disease activity score 28-erythrocyte sedimentation rate) and other indices, resulting in challenges to assess inflammatory disease activity. Although several reports indicate that elevated patient global assessment (PATGL) may elevate DAS28 in the absence of inflammatory activity, less information is available concerning the other three components, tender joint count (TJC), swollen joint count (SJC), and erythrocyte sedimentation rate (ESR), to possibly elevate DAS28

BACKGROUND: Tuberculosis (TB) is the leading global cause of death from a single infectious agent. Recent reductions in global health funding have threatened TB control, making comprehensive assessment of TB, HIV-related TB, and drug-resistant TB burdens before these disruptions essential for shaping effective responses. The WHO End TB Strategy sets targets of a 95% reduction in TB deaths and a 90% reduction in TB incidence between 2015 and 2035. Using results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023, this study aims to assess the burden of TB and multidr

BACKGROUND: Understanding the size, gender composition, and cadre mix of the health workforce and how it has evolved across time and locations can inform policies for planning, recruitment, training, and retention of human resources for health (HRH), and improvement of access to the health-care workforce among populations. Using comparable and standardised data sources, we aim to describe the composition and density of health workers by sex among 20 cadres for 204 countries and territories over 1990-2023 and quantify workforce shortfalls relative to universal health coverage (UHC) attainment.

BACKGROUND: Despite a general improvement in the HIV burden over the past two decades, gendered social determinants including intimate partner violence (IPV) continue to affect women's vulnerability to HIV, which could be further exacerbated as resources dwindle over the coming years. Our study aimed to quantify recent trends in the HIV burden, the magnitude of the HIV burden associated with IPV, and the potential impact of declining financial support globally.

BACKGROUND: Febrile diseases such as Malaria, Typhoid fever, HIV/AIDS, Tuberculosis, respiratory tract infections, and urinary tract infections remain major public health concerns in low-to-middle-income countries (LMICs). Their prevalence is driven by interacting socioeconomic, environmental, behavioral, and biological factors such as vector exposure, poor sanitation, overcrowding, and high-risk behaviors including intravenous drug use and smoking. This study investigates the key risk factors influencing the prevalence and diagnosis of febrile diseases in Southern Nigeria. METHODS: A cross-se

BACKGROUND: Second-hand smoke (SHS) exposure remains a major source of morbidity and mortality among non-smokers. This study presents the first dedicated Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) publication to systematically quantify SHS prevalence and evaluate its effect as a global risk factor across a broader range of outcomes.
METHODS: Using the GBD 2023 comparative risk assessment framework, we estimated SHS exposure prevalence and attributable disease burden across 204 countries and territories from 1990 to 2023, stratified by age and sex. Exposure estimates wer

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: Infectious diseases (ID) is a crucial specialty in medicine, yet applications for fellowship have declined even as the United States faces an imminent shortage of ID physicians. Career interests often develop in medical school, but little is known about which interests and experiences are associated with interest in ID.
METHODS: To evaluate interest in ID among medical students and identify factors associated with interest and disinterest in ID careers, we developed a 26-question survey to gauge interest in ID. All 16 medical schools in Texas were contacted and invited to particip

PROBLEM/CONDITION: Approximately 800 foodborne illness outbreaks occur in the United States each year. These outbreaks include approximately 15,000 illnesses, 800 hospitalizations, and 20 deaths. Although illnesses from outbreaks account for a small portion of all foodborne illnesses, outbreak investigations reveal how these illnesses originate by offering crucial data through epidemiologic, environmental health, and laboratory analyses and aid in outbreak mitigation and prevention.
PERIOD COVERED: 2014-2022.
DESCRIPTION OF SYSTEM: The Foodborne Disease Outbreak Surveillance System (FDOSS),

BACKGROUND: In sub-Saharan Africa, highly mobile men such as fishermen have a low uptake of HIV testing, prevention, and treatment. This study aimed to examine whether a HIV status-neutral, social network-based intervention could improve testing and linkage to prevention and treatment among fishermen in Kenya.
METHODS: The Owete cluster-randomised trial mapped the male social networks of fishermen in three communities along Lake Victoria in Siaya County, Kenya, and identified distinct social networks (clusters) with a highly connected, network-central man (promoter) in each network. Participa

Human papillomavirus (HPV) is the most prevalent viral infection globally, transmitted primarily through sexual or intimate skin-to-skin contact. Certain HPV types can cause anogenital warts and has the potential to cause cervical cancer, other anogenital, and oropharyngeal cancers. Adjuvanted, non-live, HPV recombinant vaccines, including the bivalent, quadrivalent, and 9-valent vaccines, are widely recommended for adolescents and young adults to prevent HPV infection and lower the incidence of HPV-related cancers. However, recommendations for adults aged 26 years or older have been lacking d

Brucellosis poses a significant health threat to the population, particularly to vulnerable populations, including infants. In this investigation, we retrospectively analyzed the infection source and potential transmission route in a three-month-old infant with febrile seizure. Bacteriology methods, epidemiological survey, Rose Bengal plate test (RBPT), and standard tube agglutination test (SAT) were used to diagnose the disease, and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS) was applied to identify the strain. The study revealed that the infant

BACKGROUND: Brucellosis is a zoonotic disease with significant economic and public health implications. Infections in humans are always transmitted through direct or indirect contact with infected animals or their products. A substantial proportion of the Indian population is occupationally exposed to animals, increasing the risk of zoonotic infections such as brucellosis.
OBJECTIVE: To determine the seroprevalence of brucellosis among veterinary health personnel in Ernakulam district and to analyze the profile of brucellosis cases reported at a tertiary care center over five years.

Excessive exposure to ultraviolet (UV) radiation is a major risk factor for skin cancer, yet sun protection behaviors vary widely across different populations. This study examines the association between sun protection behaviors and educational attainment among Brazilian adults. A cross-sectional study was conducted using data from the Brazilian Surveillance System for Risk and Protective Factors for Chronic Diseases by Telephone Survey (Vigitel) from 2007 to 2010. Sun protection behaviors were assessed based on self-reported habitual avoidance of sun exposure, use of sunscreen, hats/umbrellas
