Prevalence, Serotype Distribution, and Risk Factors Associated With Streptococcus pneumoniae Carriage in Older Adults in Denmark, 2019-2022.

BACKGROUND: The burden of pneumococcal disease in older adults remains high in the postpneumococcal conjugate vaccine (PCV) era. We hypothesized that older adults may represent a carriage reservoir of Streptococcus pneumoniae. METHODS: Paired naso- and oropharyngeal (NP and OP) swabs and questionnaires were collected during 2019-2022 from adults ≥65 years of age. Pneumococcal carriage and serotype distribution were determined by phenotypical and molecular methods. RESULTS: A total of 172/1764 participants (9.8% [95% confidence interval {CI} 8.4-11.2]) were positive for carriage of S. pneumon

Optimization of heroin conjugate vaccine performance in rodent models.

Opioid use disorder (OUD) remains a global public health concern, exerting a significant economic burden on societies worldwide. To combat the rise of substance use disorder, a complementary vaccine-based intervention was developed to support and strengthen the existing treatment modalities. Hapten-protein conjugate vaccines are employed as a promising novel strategy for neutralizing nonimmunogenic small-molecule drugs such as opioids by eliciting drug-specific antibodies that sequester opioids in circulation before reaching the brain. In this study, multiple parameters to optimize the efficac

Machine-intelligent multimodal algebot for intracavitary chemotherapy.

Intracavitary drug instillation is a crucial therapeutic strategy for treating bladder cancer. However, current methods are limited in efficacy due to insufficient tumour targeting and drug penetration across tissue barriers in pathophysiological conditions. Here we devise biohybrid magnetic algae microrobots with hierarchical nanoporous structure and develop an 'algebot'-mediated, non-contact convective transport strategy to synergistically integrate targeted carrier transport, selective drug release and ultrafast tissue penetration. Our approach leverages machine-intelligent image feedback f

Sex differences in tuberculosis incidence: a pooled analysis of 11 prospective cohort studies.

BACKGROUND: Tuberculosis (TB) incidence is consistently higher in men than women globally. Whether this disparity arises from differential infection risk or sex-specific rates of progression from infection to disease remains unclear. We evaluated sex differences in the development of TB disease while accounting for baseline infection status. METHODS: We conducted a pooled individual participant data analysis of 11 prospective cohort studies (n = 22,424), conducted between 2005 and 2017 across Sub-Saharan Africa, Europe, Asia, and South America. TB infection was defined by a positive QuantiFER

Assessment of Impacts of Clinically Effective Antihypertensive Herbal Medicines on the Activities of Major Drug Metabolizing Enzymes and Transport Proteins.

INTRODUCTION: Hypertension is one of the most common non-communicable diseases, and reports indicate that its prevalence is escalating globally due to several factors. Studies in different countries have shown an upsurge in the use of Herbal Medicinal Products (HMPs), including herbal antihypertensive medicines taken by people with hypertension, and 20-80 % of hypertensive patients concurrently use these herbal medicines and conventional antihypertensive medicines or other drugs. The safety and efficacy of the concurrent use of herbal and orthodox medicines are of great concern because of the

Janus Kinase Inhibitors for Inflammatory Bowel Disease: Concise Questions and Answers on Their Use in Clinical Practice.

Janus kinase (JAK) inhibitors are being increasingly used in inflammatory bowel diseases (IBDs). In the present review we raise a series of practical questions on JAK inhibitors in IBD and provide clear and concise answers. We performed a bibliographic search to identify studies assessing the role of JAK inhibitors in IBD. The key conclusions are summarized as follows. Upadacitinib has shown favorable efficacy outcomes in ulcerative colitis relative to other JAK inhibitors, although this conclusion requires confirmation through randomized trials. Extended induction can benefit patients with pa

Immunological testing and machine learning in detecting latent tuberculosis among high-risk groups (nature review).

INTRODUCTION: Tuberculosis infection remains one of the most dangerous and difficult to diagnose diseases. To date, issues related to the early diagnosis of tuberculosis remain unresolved, which is particularly important for its detection in high-risk groups. The detection of latent tuberculosis infection (LTBI) is necessary to control the spread of tuberculosis infection. The diagnosis of LTBI is indirect and based on the detection of an immune response to mycobacterial antigens. Currently, LTBI diagnosis is recommended in high-risk groups. However, diagnosis is difficult and not always strai

Insights into the host response to 'dormant' Mycobacterium tuberculosis utilizing 'Vitamin C-induced dormant Mtb' THP-1 cell infection model.

BACKGROUND: During a latent tuberculosis infection, there exists a dynamic equilibrium between the host and the 'dormant' bacterium wherein they mutually influence each other. An understanding of the host genetic response to 'dormant' tubercle bacilli during infection is necessary for developing targeted strategies against them. RESULTS: A previously established infection model based on Vitamin C-induced Mtb dormancy was utilized in the present study to identify host responses to 'dormant' Mycobacterium tuberculosis (Mtb) infection by analyzing host transcriptomic data generated from Mtb-infe

Nonlinear association between advanced lung cancer inflammation index and latent tuberculosis infection risk: threshold effects and predictive value of a novel biomarker.

BACKGROUND: Nutritional and inflammatory status influence latent tuberculosis infection (LTBI) susceptibility, but the role of composite biomarkers like advanced lung cancer inflammation index (ALI) is unclear. This study aimed to explore the association of the ALI with LTBI. METHODS: This cross-sectional study analyzed the National Health and Nutrition Examination Survey (NHANES) data of 3,010 adult participants in the USA from 2011 to 2012, of whom 382 were diagnosed with LTBI. Multivariate logistic regression, subgroup analysis, and interaction assessment were performed to investigate the

Diagnostic accuracy and safety of Diaskintest® compared with the tuberculin skin test for detecting tuberculosis infection in BCG-vaccinated Brazilian adults.

BACKGROUND: Evidence on the accuracy and safety of the ESAT-6/CFP-10-based Diaskintest® in BCG-vaccinated populations outside Eastern Europe remains limited. In Brazil, recurrent shortages of purified protein derivative (PPD) have challenged the implementation of tuberculin skin testing, underscoring the need to evaluate alternative tools for tuberculosis infection (TBI) screening. This trial compared the diagnostic performance and safety of Diaskintest® with the tuberculin skin test (TST) using PPD Rt-23 in Brazilian adults, a predominantly BCG-vaccinated population. METHOD: A double-blind r

Reappraising TB preventive treatment in India: programmatic and ethical implications of subclinical tuberculosis in household contacts.

India's pursuit of Tuberculosis (TB) elimination is contingent on the rapid universal scale-up of TB Preventive Treatment (TPT) for household contacts. However, current strategies largely neglect the asymptomatic active (subclinical) TB stage in terms of standardized diagnosis and optimized management. Consequently, administering TPT to individuals with unrecognized subclinical TB constitutes inadequate therapy that provides no patient benefit, enables community transmission, and risks minimal chances of iatrogenic drug resistance-violating the fundamental ethical principle of non-maleficence.

Detection for New Biomarkers of Tuberculosis Infection Activity Using Machine Learning Methods.

BACKGROUND/OBJECTIVES: Latent tuberculosis infection (LTBI) represents a critical reservoir for subsequent development of active tuberculosis (ATB) and poses significant challenges for early diagnosis and disease prevention. Traditional immunological assays, such as interferon-gamma release assays (IGRAs), are limited in their ability to reliably distinguish LTBI from ATB. Recent advances in high-throughput omics technologies and machine learning (ML) approaches offer new opportunities for precise, biomarker-based differential diagnostics. METHODS: Transcriptomic and proteomic profiling of ho

Pediatric Tuberculosis: Unraveling Immunity, Clinical Complexities, and Resource-Driven Disparities in the Pursuit of Prevention.

Pediatric tuberculosis (TB) remains a critically underrecognized contributor to global childhood morbidity and mortality, with the highest burden concentrated in low-resource settings. Although children comprise a minority of overall TB cases, mortality is disproportionately high, particularly among those under five years of age, driven largely by delayed diagnosis, inadequate linkage to care, and limited access to effective treatment. The continued rise of pediatric multidrug-resistant TB (MDR-TB), especially in regions with low sociodemographic development, further highlights persistent gaps

Future direction of tuberculin skin test: Integration of telemedicine for test reading.

OBJECTIVE: The Tuberculin Skin Test (TST) remains an important tool for the diagnosis of tuberculosis infection (TBI), including in patients receiving immunomodulatory therapies. However, conventional in-person TST reading requires repeated hospital visits, which may pose logistical challenges and limit accessibility to care. This study aimed to evaluate the reliability of patient self-assessment of TST reactions under real-time telemedicine supervision. METHODS: This prospective observational study included 100 adult patients with an indication for TST. After purified protein derivative (PPD

Germ-line exon 21 EGFR V831H mutation in advanced NSCLC resistance to almonertinib: a case report.

BACKGROUND: Germ-line EGFR mutations are rare, and their clinical significance, particularly regarding response to tyrosine kinase inhibitors (TKIs), remains poorly defined. The EGFR V831H (also known as R831H) mutation is an exceptionally rare variant with constitutive activity, and data on its therapeutic sensitivity are scarce. METHODS: We present a detailed case report of a patient with advanced non-small cell lung cancer (NSCLC) harboring a germ-line EGFR V831H mutation. Diagnosis involved imaging, histopathology, and comprehensive genomic profiling of tumor tissue. Germ-line origin was

Willingness to Receive Latent Tuberculosis Infection Treatment Among Migrants in Tijuana, Baja California, Mexico.

Over 80% of tuberculosis cases in the United States are due to reactivation of untreated latent tuberculosis infection (LTBI), most of whom were foreign born. LTBI treatment can greatly lower the risk of progression to active TB disease, and shorter treatment regimens increase the potential of migrants and other persons in vulnerable conditions, to complete treatment. We conducted across-sectional study to assess willingness to complete a one-month course of LTBI treatment among internal and international migrants living in Tijuana, BC, Mexico, and to identify factors associated with treatment
