Outcomes Following Pancreatoduodenectomy With Arterial Resection: A NSQIP Analysis Comparing Trends in Patient Selection and Outcomes to Venous or no Vascular Resection Over 5 Years.

Evaluation of arterial resection (AR) during pancreatoduodenectomy (PD) remains limited. This study compares PD with AR to PD with venous resection (VR) and no vascular resection (NVR). Patient demographics, outcomes, and changes over 5 years are evaluated. In this study, NSQIP data was analyzed comparing patients undergoing PD with AR to NVR and VR. Demographics and 30-day post-operative outcomes were compared, and multivariable modeling evaluated factors associated with serious complications and mortality. Propensity score matched (PSM) analysis compared AR to NVR, AR to VR, and VR to NVR. W

TNF/TNFR1 axis restrains inflammation-suppressive myeloid subclusters to drive acute pancreatitis.

Acute pancreatitis (AP) is a pancreatic inflammatory disorder that can progress to fatal systemic inflammation and multiorgan failure. Although tumor necrosis factor (TNF) signaling is a key regulator of inflammation, its contribution to AP progression and its therapeutic potential remain incompletely understood. We induced AP in wild-type and Tnfr1-/- mice using caerulein, pancreatic duct ligation, or L-arginine. Single-cell RNA sequencing (scRNA-seq) and bulk RNA sequencing were used to characterize pancreatic immune-cell profiles, and pomalidomide, infliximab, and necrostatin-1 were evaluat

Preoperative CT Suspicion of Main Pancreatic Duct Injury in Isolated Blunt Pancreatic Trauma: A Case Report With Operative Correlation.

An 18-year-old male had persistent tachycardia after blunt abdominal trauma. CT signs raised suspicion of main pancreatic duct disruption in the tail (suspected AAST Grade III). Exploratory laparotomy confirmed ductal transection, managed by spleen-preserving distal pancreatectomy. Six-month follow-up showed no fistula or clinical pancreatic insufficiency.

Robotic-assisted transduodenal ampullectomy for a neuroendocrine tumour: A step-by-step surgical video.

BACKGROUND: When endoscopic resection of ampullary neuroendocrine tumors (NETs) is not feasible due to tumour size or location, transduodenal ampullectomy can offer a parenchymal-sparing alternative to a formal pancreaticoduodenectomy (Whipple procedure) [1,2]. We present a robotic-assisted transduodenal ampullectomy (RATA) in a 71-year-old male with an incidental 14 mm ampullary NET found during routine screening. Preoperative 68Ga-DOTATATE PET-CT demonstrated intense somatostatin-receptor avidity (SUVmax 66.8) and absent FDG uptake. The MDT recommended surgical excision due to size and Ki-67

Pancreatic head solid-pseudopapillary neoplasm in a man treated by robot-assisted laparoscopic partial pancreatectomy: a case report.

BACKGROUND: Solid-pseudopapillary neoplasm (SPN) of the pancreas is a rare tumor with low malignant potential, most commonly occurring in the pancreatic body or tail of young women. Atypical presentations, particularly in male patients and cases involving the pancreatic head, may pose diagnostic and therapeutic challenges. This report highlights surgical management considerations in a patient with inconclusive preoperative findings. CASE PRESENTATION: We report the case of a 37-year-old man with a large lesion initially interpreted as a retroperitoneal mass closely associated with the pancrea

Navigating the Pancreatic Duct: Up-to-Date Radiologic Expedition through Intraluminal Pathologic Entities.

Several congenital, traumatic, infectious, inflammatory, and neoplastic processes, as well as iatrogenic interventions, can alter intraluminal contents of the pancreatic duct, leading to adverse outcomes. Variant ductal anatomy may result in clinically significant complications such as duodenal obstruction, acute and chronic pancreatitis, and increased risk of developing gallbladder or biliary malignancies. The presence of calculi, blood, pus, and other unintended iatrogenic material in the pancreatic duct can be associated with a range of complications. Neoplastic involvement of the pancreati

Intraoperative Pancreatoscopy Demonstrates Feasibility of Pancreatic Preservation During Robotic Pancreatoduodenectomy for IPMN in Elderly Patients.

BACKGROUND: Main duct intraductal papillary mucinous neoplasm (MD-IPMN) carries a substantial risk of malignant transformation and often requires surgical resection.1,2 Preservation of the remnant pancreas remains an important goal, particularly in elderly patients.3,4 Therefore, determining the optimal extent of resection requires a careful balance between oncological safety and preservation of pancreatic function.5,6 The utility of intraoperative pancreatoscopy during robotic pancreatoduodenectomy (RPD) for this purpose has not been well established.7 METHODS: We present two elderly patients

Pancreatic hydatid cyst with a characteristic "water-lily" sign.

BACKGROUND: Cystic echinococcosis rarely involves the pancreas and may mimic common cystic neoplasms. Recognition of characteristic imaging signs is central to diagnosis; the "water-lily" sign (floating detached endocyst) is a rare and highly characteristic finding in favor of transitional-stage hydatid cyst and, when recognized, can prevent inappropriate cyst puncture and guide stage- and anatomy-based management. CASE: A 34-year-old woman from Iran, an endemic region for cystic echinococcosis, presented with intermittent epigastric/right upper quadrant pain. At an outside institution, endos

Usefulness of Endoscopic Ultrasound-Guided Pancreatogastrostomy and the Related Trans-Endosonographically Created Route Procedure for the Management of Refractory Pancreatic Stones (With Video).

Endoscopic management of symptomatic pancreatic stones (P-Stones) with main pancreatic duct (MPD) strictures due to chronic pancreatitis remains challenging. For refractory P-Stones, high-quality evidence for endoscopic treatment remains limited, and endoscopic ultrasound-guided pancreatogastrostomy (EUS-PGS) is considered a potential salvage option. This single-center retrospective study enrolled 16 consecutive patients with symptomatic refractory P-Stones who underwent EUS-PGS between June 2017 and June 2025, after failed transpapillary access or, in cases of surgically altered anatomy (SAA)

New insights into β-cell vulnerability in type 1 diabetes-Lessons from nature.

Type 1 diabetes results from an autoimmune-mediated destruction of pancreatic β-cells, yet considerable heterogeneity exists regarding disease onset, progression and severity. Emerging evidence suggests that this variability reflects not only differences in immune responses, but also differences in the architecture and immune landscape of the endocrine pancreas. Our recent Steve Morgan Foundation Grand Challenge studies have provided important 'lessons from nature', revealing that the endocrine tissue in the human pancreas exists as a continuum of endocrine objects (EOs), ranging from isolated

Neutrophil-to-Lymphocyte Ratio Is Associated with Disease Progression in a Surveillance Cohort of Patients with Branch-Duct IPMN.

Background: A Branch-duct intraductal papillary mucinous neoplasms (BD-IPMNs) are common pancreatic precursor lesions requiring varying duration of surveillance because of their potential for malignant transformation. Current risk stratification relies predominantly on imaging-based criteria defined by the Kyoto guidelines, while reliable blood-based biomarkers are lacking. The neutrophil-to-lymphocyte ratio (NLR) has been associated with invasive IPMN in surgical resected cases, but its role in routine BD-IPMN surveillance has not been investigated. This study investigated the association bet

Quantitative CT-Derived Imaging Features Improve Preoperative Risk Stratification of Intraductal Papillary Mucinous Neoplasms.

BACKGROUND: Accurate preoperative identification of high-grade dysplasia (HGD) or malignancy in intraductal papillary mucinous neoplasms (IPMNs) remains challenging using conventional guideline-based imaging criteria. Quantitative computed tomography (CT)-based features may detect subtle structural changes preceding overt radiologic findings. This study aimed to evaluate whether the pancreatic index (PI) and additional three-dimensional CT-derived quantitative features improve prediction of HGD or malignancy in IPMNs. METHODS: This single-center retrospective study included patients who under

Pancreatic Ascites Complicated by Salmonella Enteritidis Peritonitis: A Diagnostic Challenge and the Role of Surgical Source Control.

Pancreatic ascites is an uncommon complication of pancreatic disorders resulting from persistent leakage of pancreatic juice into the peritoneal cavity, most commonly due to disruption of the main pancreatic duct or rupture of a pancreatic fluid collection. Bacterial peritonitis complicating pancreatic ascites is rarely reported. We report the case of a 40-year-old man who presented with massive pancreatic ascites associated with a marked elevation of serum lipase. Analysis of the ascitic fluid strongly supported its pancreatic origin, and conservative management with enteral nutrition and a s

Differentiating pancreatic acinar cell carcinoma from solid pseudopapillary neoplasm using contrast-enhanced computed tomography.

BACKGROUND: Solid pseudopapillary neoplasm (SPN) and pancreatic acinar cell carcinoma (PACC) represent two uncommon pancreatic tumors with distinct biological behaviors and management strategies. Both entities can present with overlapping appearances on contrast-enhanced computed tomography (CECT), posing a diagnostic challenge preoperatively. Therefore, reliable imaging differentiation between these tumors is clinically important. This study aimed to evaluate computed tomography (CT) imaging features for differentiating SPN from PACC and to develop a combined model to improve preoperative dia

Rapid Radiological Disappearance of Pancreatic Duct Stones Following Oral Trimethadione.

A 59-year-old man with chronic pancreatitis, surgically altered anatomy, and an implantable cardioverter-defibrillator presented with acute exacerbation caused by cast-like obstructive pancreatic duct stones. Endoscopic therapy was considered technically difficult, and extracorporeal shock wave lithotripsy was avoided. While delayed surgical drainage was being considered, off-label oral trimethadione was started after institutional ethics approval. Computed tomography 2 weeks later showed disappearance of the stones and improvement of ductal dilatation. No adverse events occurred, and surgery

Difficult biliary cannulation during ERCP: techniques and an evidence-informed practical algorithm.

Difficult biliary cannulation remains the pivotal technical barrier in therapeutic endoscopic retrograde cholangiopancreatography (ERCP) and is a major modifiable driver of post-ERCP pancreatitis (PEP). Contemporary practice has moved from prolonged standard attempts toward earlier recognition of difficult access, guidewire-based primary cannulation, targeted rescue techniques, and routine pancreatitis prophylaxis. This review summarizes current evidence and proposes a practical, evidence-informed algorithm for native papilla ERCP. Difficult cannulation is best recognized using objective proce
