Trained immunity-based mucosal immunotherapies for the prevention of respiratory infections.

The devastating impact of respiratory infections demonstrates the critical need for novel prophylactic vaccines. In this opinion article, we advocate for bacterial immunotherapies as a complementary tool in our fight against respiratory infections. These immunotherapies can activate a wide spectrum of immunological mechanisms, with trained immunity (TI) being particularly significant. This phenomenon has led to the concept of trained immunity-based vaccines (TIbVs), which represent a novel approach in vaccinology. We discuss examples of TIbVs, including the tuberculosis vaccine Bacille Calmett

Genetic insights in infectious diseases: Insights from a case report and implications for personalized medicine.

The relationship between genetics and infectious diseases is important in shaping our understanding of disease susceptibility, progression, and treatment. Recent research shows the impact of genetic variations, such as heme-oxygenase promoter length, on diseases like malaria and sepsis, revealing both protective and inconclusive effects. Studies on vaccine responses highlight genetic markers like human leukocyte antigens, emphasizing the potential for personalized immunization strategies. The ongoing battle against drug-resistant tuberculosis (TB) illustrates the complexity of genomic variants

Molecular mechanisms of immune evasion by host protein glycosylation of a bacterial immunogen used in nucleic acid vaccines.

Nucleic acid vaccines, including mRNA platforms, represent a transformative approach by inducing adaptive immunity against in situ-expressed antigens. Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains a leading global health threat. The only licensed TB vaccine, Bacillus Calmette-Guérin (BCG), offers limited protection against pulmonary TB in adults and adolescents. Underscoring the urgent need for more effective vaccination strategies. However, clinical trials targeting the Mtb Ag85 complex proteins as nucleic acid vaccines have failed to confer protective immunity. We hy

Quantifying the impact of a broadly protective sarbecovirus vaccine in a future SARS-X pandemic.

COVID-19 has underscored the need for more timely access to vaccines during future pandemics. This has motivated development of broad-spectrum vaccines providing protection against entire viral families, which could be stockpiled and deployed rapidly following detection. Using mathematical modelling, we assess the utility of a broadly protective sarbecovirus vaccine during a hypothetical SARS-X outbreak, for a range of implementation strategies including ring-vaccination, spatial-targeting and mass vaccination of high-risk groups. Broadly protective sarbecovirus vaccine ring- or spatial strate

Computational epitope profiling and AI-driven protein engineering enable rational design of multi-epitope vaccines against Mycobacterium tuberculosis.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains a major global health threat, accounting for approximately 1.5 million deaths annually. The rise of antibiotic-resistant strains further complicates treatment efforts. While vaccination is a cornerstone of disease control, the only licensed TB vaccine, Bacille Calmette-Guérin (BCG), shows limited efficacy in adults. There is thus a critical need for more effective vaccines. Multi-epitope vaccines, which incorporate key epitopes from multiple antigens, offer a promising strategy by eliciting both humoral and cellular immunit

Targeting Mycolic Acid Biosynthesis with Cyclic Sulfamates: A New Strategy against Mycobacterium tuberculosis.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), is the deadliest infectious disease globally. Current TB regimens involving multidrug cocktails for ≥4 months with significant side effects leave much to be desired, with the first- and second-line drugs inhibiting only a limited number of bacterial targets. Thus, potent antimycobacterial agents with novel targets and mechanisms of action are urgently needed to overcome these limitations and the emergence of multidrug-resistant strains. To address this need, we tested a panel of cyclic sulfamate (CS) compounds and identified novel

A novel influenza vector-based vaccine expressing ESAT-6 and TB10.4 confers immunity and protection against Bovine tuberculosis in guinea pigs and calves.

BACKGROUND AND AIM: Bovine tuberculosis (bTB), caused by Mycobacterium bovis, remains a significant zoonotic and economic threat globally. Despite the long-standing use of the Bacillus Calmette-Guérin (BCG) vaccine, its inconsistent efficacy and interference with surveillance tests underscore the need for alternative approaches. This study evaluated the safety, immunogenicity, and protective efficacy of a novel influenza vector-based vaccine expressing M. bovis antigens ESAT-6 and TB10.4, formulated with or without an adjuvant. MATERIALS AND METHODS: Recombinant influenza A viruses expressing

Neuro-immunobiology and treatment assessment in a mouse model of anti-NMDAR encephalitis.

Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a disorder mediated by autoantibodies against the GluN1 subunit of NMDAR. It occurs with severe neuropsychiatric symptoms that often improve with immunotherapy. Clinical studies and animal models based on patients' antibody transfer or NMDAR immunization suggest that the autoantibodies play a major pathogenic role. Yet, there is an important need for models offering an all-inclusive neuro-immunobiology of the disease together with a clinical course long enough to facilitate the assessment of potential new treatments. To develop such a

Vaccination in pregnancy to protect the newborn.

Infectious diseases pose a particular risk to newborns and there is a global need to protect this vulnerable group. Because of the challenges of developing vaccines that are effective in newborns, only the hepatitis B and tuberculosis vaccines are given in the first 28 days of life, and even those vaccines are mainly only offered to high-risk groups. Maternal antibodies cross the placenta and can afford some protection to the newborn, so an alternative strategy is vaccination in pregnancy. This approach has been successfully used to protect newborns against tetanus and pertussis, and vaccines

Glucose Metabolic Reprogramming of Macrophages Against Mycobacterium tuberculosis Infection.

Tuberculosis (TB) is a global infectious disease caused by Mycobacterium tuberculosis (Mtb). Serving as the primary effector cells, macrophages play a crucial role in host immune responses against Mtb. During Mtb infection, macrophages undergo extensive metabolic reprogramming, notably glycolysis, the pentose phosphate pathway (PPP) and the tricarboxylic acid (TCA) cycle, to adapt to the challenges posed by the pathogen, with glucose metabolic rewiring being particularly critical. This review focuses on the dynamic reprogramming of glucose metabolism in macrophages during Mtb infection, highli

Macrophage targeted cellular microparticles synergize anti-tuberculosis therapy combining CircRNA-mediated host immune defense and antibiotic killing.

Tuberculosis, caused by Mycobacterium tuberculosis (Mtb) infection, remains one of the most deadly infectious diseases worldwide. There is an urgent need for the development of innovative strategies improving the antibiotic efficiency and enhancing host immunological defenses for more effective TB treatment. CircRNA TRAPPC6B (circTRAPPC6B) can enhance autophagy and M1 polarization of Mtb infected macrophages to facilitate intracellular Mtb clearance, arising the promise for more effective anti-TB treatment. Here, we innovated macrophage targeted cellular microparticles encapsulating circTRAPPC

Viral control is required for intravenous BCG-mediated protection against tuberculosis in simian immunodeficiency virus-infected macaques.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), is a major global health challenge, especially for people living with HIV (PLWH) not on antiretroviral therapy. We previously showed that intravenous Bacillus Calmette-Guérin (IV-BCG) provides robust protection in SIV+ Mauritian cynomolgus macaques (MCM). Here, we evaluate whether the immunogenicity and efficacy of IV-BCG in SIV+ MCM was impaired by eliminating BCG with drugs. SIV+ macaques were treated with an anti-BCG regimen of isoniazid, rifampicin, and ethambutol (HRE), starting either 1- or 3-weeks post-BCG vaccination. Five

Global, regional, and national trends in routine childhood vaccination coverage from 1980 to 2023 with forecasts to 2030: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Since its inception in 1974, the Essential Programme on Immunization (EPI) has achieved remarkable success, averting the deaths of an estimated 154 million children worldwide through routine childhood vaccination. However, more recent decades have seen persistent coverage inequities and stagnating progress, which have been further amplified by the COVID-19 pandemic. In 2019, WHO set ambitious goals for improving vaccine coverage globally through the Immunization Agenda 2030 (IA2030). Now halfway through the decade, understanding past and recent coverage trends can help inform and r

[TNF-α blockers: Its indications and potential toxicity in pulmonology].

INTRODUCTION: As a key cytokine in inflammation and granuloma formation TNF-α plays a central role in several pulmonary diseases, including sarcoidosis, tuberculosis and the acute respiratory distress syndrome. Although widely used in treatment of chronic inflammatory diseases, anti-TNF-α biotherapies have few indications in pulmonology. STATE OF THE ART: In therapeutic trials, the efficacy of anti-TNF-α drugs in treating lung diseases has yet to be proven, and they are not currently recommended as first-line treatments. Possible immunization against the treatment could be counteracted by the

Editor's Choice Platform for identifying human glycan-specific antibodies against bacterial pathogens using synthetic glycan fragments.

Bacterial infections represent a substantial global health challenge, impacting both human and veterinary health. The ongoing evolution of antibiotic-resistant pathogens, coupled with limited new antibiotic discoveries, urges the need for alternative strategies to treat and prevent these infections. Passive immunization with monoclonal antibodies (mAbs) is gaining interest as a promising alternative. Here, we report an experimental pipeline for generating human mAbs from healthy donor B cells using synthetic mimics of complex bacterial glycans. We identified functional mAbs recognizing discret

Hepatitis B virus resistance to nucleos(t)ide analogue therapy: WHO consultation on questions, challenges, and a roadmap for the field.

In this Review, we summarise outputs from a multidisciplinary consultation convened by WHO between July 11 and 13, 2023, to discuss hepatitis B virus (HBV) drug resistance (HBVDR). Treatment of chronic HBV infection with highly effective nucleos(t)ide analogue agents, tenofovir and entecavir, is a crucial intervention that supports the global goal of elimination of HBV infection as a public health threat. The risk of HBVDR as a threat to treatment outcomes is currently considered low from a public health perspective; however, drug resistance can influence individual outcomes, particularly amon
