Vaccine preventable viral diseases in developing countries.

There are several viral infectious diseases with a high impact on developing countries which can be prevented by immunization with existing vaccines. The most important are poliomyelitis, measles, hepatitis B and yellow fever. Vaccines against poliomyelitis and measles used within the framework of the WHO/Expanded Programme on Immunization prevent about 1.4 million deaths from measles and 360,000 cases of paralytic polio per year in developing countries, but about 1.5 million measles' deaths and 200,000 cases of paralytic polio still occur. Hepatitis B infection and its sequelae are responsibl

Worldwide status of poliomyelitis in 1986, 1987 and 1988, and plans for its global eradication by the year 2000.

In 1986, a total of 28,951 cases of poliomyelitis were reported to WHO by 175 countries/areas. For 1987, 35,685 cases were reported by 168 countries/areas. Reporting is incomplete for 1988, based on data available as of December 1989. Trends in regional incidence rates for poliomyelitis over a 15-year period from 1974 to 1988 suggest a decline in the incidence of the disease, especially since 1982. However, the reported data underreport actual disease incidence. During 1986, 1987 and 1988, the Expanded Programme on Immunization (EPI) estimates that 200,000-250,000 cases of paralytic poliomyeli

[Prevention in pediatrics in the Nord-Cotentin. Prospective study of 255 cases].

Over a 4 month period, the medical records of 255 children with an age range of 6 months to 3 years and who had been admitted to the Cherbourg Hospital Department of Pediatrics were prospectively studied. Ninety-nine % had received BCG vaccination but only 71% had a positive skin test. Ninety-two % were vaccinated against diphtheria, tetanus, whooping cough and poliomyelitis and 61% against measles. Seventy-eight % of parents said that they had correctly administered vitamin D, at least up to 18 months of age. Seventy % of children were fed cow's milk during the second half of the first year a

[The need for preventive vaccination of children and adolescents in highly civilized industrial countries].

The advance of social and hygienic structures in industrialised countries since the beginning of the 20th century caused a significant change in the spread and pattern of many infectious diseases in these areas. Diphtheria, tetanus and tuberculosis began to disappear assisted in vaccination. The so-called "children's diseases" appeared more and more in later childhood and adolescence. This involved an increase of complications, in the first place neurological disorders. This dramatic change necessitated active immunisation against measles, mumps, rubella and poliomyelitis. The newly recommende

Vaccination strategies in developing countries.

By 1990 it is hoped that all of the world's infants will have access to immunization services and that these services will then continue indefinitely. The link between people and health services, including immunization, can only be forged and maintained by an effective system of delivery and support to all health workers. A careful choice of strategies for this delivery system and an understanding of local cultural attitudes and behaviour is vital if this link is to be effective. Health workers will have to be trained and then supported in the field by regular contact with their supervisors. T

Antibody state to poliovirus in first year university students, 1984.

Circulating antibodies to poliovirus were estimated in a group of 300 British and 84 foreign first year students who registered at the health centre of Nottingham University in 1984. Detectable antibodies to all three poliovirus serotypes were found in 212 (71%) of the British students but in only 47 (56%) of those from abroad. Most of the British students (280; 93%) had been born in 1965 or 1966, when uptake of poliomyelitis vaccine was declining. Immunisation histories showed that 10 British and 29 foreign students (3% and 35%) had no record of any immunisation; only five British and two for

The Expanded Programme on Immunization of the World Health Organization.

In the developing world today, less than 20% of children are immunized against diphtheria, pertussis, tetanus, tuberculosis, measles, and poliomyelitis. Consequently, these diseases kill 10 children and disable 10 more each minute. To address this problem, the World Health Organization created the Expanded Programme on Immunization (EPI), with the goal of reducing morbidity and mortality by making immunization services available for all children of the world by 1990. This goal will be difficult to achieve. Information systems and immunization services themselves need to be strengthened. This g

Oral route as method for immunizing against mucosal pathogens.

In the past three decades significant strides have been made in attempts at nonparenteral immunization. Appreciation of the importance of secretory immunity led to attempts to stimulate antibody production locally. The vaccines developed against respiratory pathogens as a result of this new knowledge have many practical limitations, such as the need for highly trained personnel, expensive equipment, very cooperative recipients for intranasal or aerosol administration, and a vaccine that is both adequately attenuated, immunogenic, and stable during storage. With recognition of the presence of a

Advantages and disadvantages of killed and live poliomyelitis vaccines.

Decision-making on the use of poliomyelitis vaccines in the WHO Expanded Immunization Programme, and particularly in the developing nations, needs to be based on an understanding of the epidemiology of poliomyelitis in different parts of the globe. Even with two safe and effective kinds of poliomyelitis vaccine available, poliomyelitis has by no means been eradicated from the world. In developed countries that are considered well-vaccinated, certain sectors of the population may be inadequately protected against risk of infection by indigenous or imported wild polioviruses. In developing natio

Immunisation levels---need they all decline?

Immunisation levels in West Sussex for the period 1970 to 1976 were reviewed. The national decline in immunisation against whooping cough was reflected locally in 1974, 1975, and 1976 but, in contrast to the national experience, levels of immunisation against diphtheria, tetanus, poliomyelitis, or measles did not decline. It is suggested that this difference is accounted for by the use of a computer system which keeps an "immunisation diary" for parents and for medical and nursing staff.

Over-immunization-an ever present problem.

The development of purified vaccines for the control of common infectious diseases may lead to a sense of false security and a tendency to over-immunize. Repeated antigenic challenge may cause hypersensitivity reactions which could harm the individual. For each vaccine there is an opitmum time and age for dosing; for those vaccines requiring a course of doses, there is a minimum but no maximum time interval between the doses. In Australia, active immunization is conducted on a regular basis against seven preventable infectious diseases. In this article, the vaccine control of each disease is d

Immunization for the internist.

The emphasis of immunization programs and schedules has traditionally been directed to infants and children, since most of the vaccine-preventable diseases are seen predominantly in these age groups. Immunization procedures in adults are less well defined but still of importance. Diseases for which immunizations are given before disease exposure include tetanus, diphtheria, influenza, rubella, and mumps; travelers to foreign countries may need immunizations against typhoid, cholera, yellow fever, typhus, poliomyelitis, plague, and viral hepatitis; other vaccines are available before disease ex

Simultaneous immunization with B.C.G., diphtheria-tetanus, and oral poliomyelitis vaccines in children aged 13-14.

The simultaneous administration of B.C.G. vaccine, diphtheria-tetanus toxoid aluminium hydroxide adsorbed vaccine, and oral poliovaccine was studied in 628 children aged 13-14 years between 1966 and 1969 in Newham, London. The efficacy of these vaccines was unaffected by administering them at the same time; routine simultaneous administration is considered justified when organizational difficulties prevent the attainment of high immunization rates with the vaccines given separately. No adverse reactions to B.C.G. or oral poliomyelitis vaccines took place, but 8% of children had moderately seve
