With the exception of the vaccine against hepatitis B the principles involved in the production of the viral vaccines in use today have not changed since Jenner (1) first developed the vaccine which was so important in the control and eventual eradication of smallpox in 1977. All have relied on the presentation of the live attenuated or inactivated virus to the host, either orally or by injection, so that it elicited the formation of antibody and primed memory cells for a rapid response to the subsequent invasion of the virus. With the discovery that protective immunity could be obtained with

Neonates born to HBSAg-positive mothers are at increased risk of HBV, mainly because of perinatal exposure. Since 1985 the HBV Regional prevention and control program of Latium recommends prophylaxis both with HBIG and HBV vaccine at birth for all infants born to HBaAg-positive mothers. From 1-1-80 to 1-5-88 89 neonates (52 males, 37 female) from Local Health Units (U.S.L.) FR 2, 3, 4, 5, 6, 7, 8, 9 have been examined at the Centre for Vaccination against HBV of Frosinone. Prophylaxis was accomplished both by HBIG 1 ml. I.M.) at birth and HBV vaccine (5.5 ml. I.M. of HBVAX) at birth, and at 1

The current DHSS guidelines on immunisation against hepatitis B in mental handicap hospitals recommend vaccination for personnel at risk directly involved in patient care (who may have direct contact with patients or their body fluids) and for new admissions into institutions where the incidence is known to be high. We report a serological survey of hepatitis B markers in over 99% of the residents of a large mental handicap hospital. Of 720 patients tested only one carried hepatitis surface antigen. This patient was anti-HBe positive. Only 4.5% of the residents carried any hepatitis marker. Th

In Japan, a nationwide prevention program against mother-to-infant infection by hepatitis B virus (HBV) started in 1985. This program consists of double screenings of pregnant women and prophylactic treatment to the infants born to both hepatitis B surface antigen (HBsAg) and hepatitis B e antigen (HBeAg) positive mothers. These infants are treated with two injections of hepatitis B immune globulin (HBIG) and at least three injections of plasma derived hepatitis B vaccine. We sent questionnaires about the numbers of each procedure or examination during nine months of investigation period to ea

Immunity status to measles, mumps, rubella and type B hepatitis (HB) viruses was studied in a cohort of 83 teen-agers (13-14 years). The vaccination against these agents is optional in Italy. Out of 83 subjects none had been vaccinated against measles, mumps and HB viruses, while 31 had had rubella vaccination. The percentage of seronegative adolescents was 2% for measles, 25% for mumps and 10% for rubella among unvaccinated teen-agers. These data suggest that even adolescent could be a target for rubella and mumps vaccination. On the contrary HBV does not appear to circulate extensively in th

By 1990 it is hoped that all of the world's infants will have access to immunization services and that these services will then continue indefinitely. The link between people and health services, including immunization, can only be forged and maintained by an effective system of delivery and support to all health workers. A careful choice of strategies for this delivery system and an understanding of local cultural attitudes and behaviour is vital if this link is to be effective. Health workers will have to be trained and then supported in the field by regular contact with their supervisors. T

Thirty members of the staff and fifty-one patients of Parma hemodialysis unit, serum negative for HBV markers, having completed the vaccination (HB-vax vaccine) against hepatitis B, were followed for two years after booster dose. After the third injection, the response to HB vaccine was significantly higher (p less than .0005) in staff members, than in patients. In the staff group, 83.3% were responders at mean titers of anti-HBs of 2703 mUI/ml; only 51% of patients developed anti-HBs at mean titres of 287 mUI/ml. As to either sex or age, no significant differences were observed in the two gro

Since the hepatitis B vaccine was licensed in 1981, emergency physicians have had the opportunity to be immunized against the hepatitis B virus (HBV). The factors that affect the decision to undergo vaccination include the risk of acquiring an HBV infection in emergency practice, the prevaccination immune status of emergency physicians who have knowledge of their hepatitis serum marker profile, and the efficacy and safety of the vaccine. Emergency physicians have a significant risk of acquiring an HBV infection and are recommended by the Centers for Disease Control to receive vaccination. Howe

Despite the profound developments in recombinant DNA technology there is only one marketed recombinant vaccine (for human viral hepatitis B). The development of others proceeds with great difficulty. Molecular vaccines against veterinary parasites are at the utmost pole of complexity in the spectrum of potential vaccines since these parasites are complex eukaryotic organisms, often dwelling at mucosal surfaces where anamnestic responses are problematic, where the immunogenicity of the parasite components is poorly understood and where the effector mechanisms of immunity are unresolved. The high rate of infection with hepatitis B virus in certain defined populations in industralized countries and among the general population in many non-industrialized countries stresses the need for hepatitis B vaccines. Hepatitis B, one of at least six different forms of viral hepatitis, may progress to chronic liver disease, including chronic persistent hepatitis, chronic active hepatitis, cirrhosis and primary liver cancer. Primary liver cancer is one of the ten most common cancers in the world today. Immunization against hepatitis B is required therefore for groups at high risk of infecti

Personnel in the VA dental facilities were screened for the detection of viral hepatitis and identification of factors implicating infectivity. A total of 963 personnel from 126 dental facilities throughout the United States voluntarily participated in the study. The rate of seroconversion for any hepatitis B markers was approximately 1% per year. Serial positive tests for antibody to hepatitis B core antigen or antibody to hepatitis B surface antigen (or both) were present in 16.2% of dentists and 13.0% of dental auxiliary personnel. Oral and maxillofacial surgeons composed the highest preval

The use of blood spotted on filter-paper is a cheap and convenient method for collecting, storing and transporting samples for analysis of markers of hepatitis virus B. Vaccine against viral hepatitis B is now available but is expensive, and, in order to make the best use of it, large-scale screening programmes need to be carried out in endemic areas prior to immunization campaigns. The sensitivity of the filter-paper method was compared with that of the analysis of whole serum, and the epidemiological data produced by the two methods were analysed. It was found that analysis of eluates of blo

To evaluate the immunogenicity of hepatitis B vaccine in renal transplant recipients, we administered three 40-microgram doses of vaccine to 17 patients who had previously undergone transplantation and were on immunosuppressive therapy. Life-table analysis revealed a cumulative antibody response rate of only 17.6% at 12 months, and the three responders had low titers of antibody to hepatitis B surface antigen. There were no serious adverse effects and no episodes of graft rejection in responders or nonresponders. In addition, the ratio of helper/inducer (T4) to suppressor/cytotoxic (T8) T cell

The worldwide importance of human hepatitis B virus infection and the toll it takes in chronic liver disease, cirrhosis and hepatocarcinoma, make it imperative that a vaccine be developed for worldwide application. Human hepatitis B vaccines are presently prepared using hepatitis B surface antigen (HBsAg) that is purified from the plasma of human carriers of hepatitis B virus infection. The preparation of hepatitis B vaccine from a human source is restricted by the available supply of infected human plasma and by the need to apply stringent processes that purify the antigen and render it free

Advances in immunology, microbial genetics, molecular biology and biochemistry are opening prospects for new purified and synthetic antigens against infectious diseases, cancers of viral origin and some immunodeficiencies. Immunization programmes against major childhood diseases with commonly used vaccines gave excellent results and are leading to their virtual eradication in developed countries. However some vaccines like the one against pertussis proved to be of considerably lower effectiveness. Results achieved in developing countries are in general lagging behind due to lower immunization

An increase in the average human lifespan from ancient times to the present is due mainly to control of infectious diseases, and vaccines are a keystone to their prevention. Several important new vaccines should emerge during the present decade, the most important being those against human hepatitis A and B. Disease eradication and elimination by vaccines are worthwhile targets for at least some of the more important diseases and will probably be accomplished during this century. Further increase in lifespan should be achieved by vaccination in this century but major future advances will need

Cytomegalovirus infections are common throughout the world. Certain populations, including pregnant women and their fetuses, immunosuppressed patients, and recipients of large amounts of transfused blood, are at increased risk. Although the majority of infections in all groups of patients are clinically inapparent, variable symptoms, including fever, rash, pneumonitis, and hepatitis, can occur. The infected host develops antibodies against CMF, but frequently, despite this appropriate immune response, infection becomes chronic with prolonged excretion of virus. In some instances, a latent infe

The emphasis of immunization programs and schedules has traditionally been directed to infants and children, since most of the vaccine-preventable diseases are seen predominantly in these age groups. Immunization procedures in adults are less well defined but still of importance. Diseases for which immunizations are given before disease exposure include tetanus, diphtheria, influenza, rubella, and mumps; travelers to foreign countries may need immunizations against typhoid, cholera, yellow fever, typhus, poliomyelitis, plague, and viral hepatitis; other vaccines are available before disease ex
