People without spleens have an increased risk of pneumococcal and other infections. Immunisation is advised for this group of patients, but the role of prophylactic antibiotics remains unresolved. Since 1992, general practitioners in South Buckinghamshire have been encouraged to immunise all asplenic patients against infections with Streptococcus pneumoniae, Neisseria meningitidis, and Haemophilus influenzae type b (Hib). In addition, an 'alert' card, similar in principle to a medical warning bracelet, has been produced for general practitioners to issue to asplenic patients. General practitio

RSV and PIV3 are responsible for about 30% of severe viral respiratory tract disease leading to hospitalization of infants and children. For this reason, there is a need to develop vaccines effective against these viruses. Since these viruses cause severe disease in early infancy, vaccines must be effective in the presence of maternal antibody. Currently, several strategies for immunization against these viruses are being explored including peptide vaccines, subunit vaccines, vectored vaccines (e.g., vaccinia-RSV or adenovirus-RSV recombinants), and live attenuated virus vaccines. The current

The transfer of host defence capacity to the human offspring provides a remarkable model of passive transfer of immunity. In fact it may also provide an example of active immunization. The transfer of mucosal protection via breast feeding offers many additional advantages for the mother and infant. Through its contraceptive effects it increases the spacing between births, thus diminshing the infant mortality and the burden on the mother. It also enhances bonding between mother and child, it seems to increase the IQ and school result of the infant and might decrease the risk of certain malignan

The acceptance of vaccinations depends on the quality of information on effects and side effects. This publication deals with the results of three questionnaire studies: Questionnaire 1 was directed to the general population and contained questions on vaccination history and acceptance of vaccinations, questionnaire two and three were addressed to practitioners (general medicine/paediatrics/internal medicine) with additional questions on the number of vaccinations performed per year. An average of 50% of the persons taking part in the questionnaire study had been vaccinated within the last 10

During a four-year period from November 1988 to October 1992, 41 cases of bacterial meningitis with a positive cerebrospinal fluid (CSF) culture and/or CSF antigen test were collected at the National Cheng Kung University Hospital. The ages of the subjects ranged from 32 days to 13 years, with a median of seven months. The male to female ratio was 2.4:1. The most common causative agent was Haemophilus influenzae type b (Hib, 29.3%), followed by group B beta-hemolytic streptococci (GBS, 24.4%), Streptococcus pneumoniae (22.0%), Escherichia coli (4.9%), Neisseria meningitidis (4.9%), Salmonella

Universal immunization of infants is essential to the control of hepatitis B in areas of high endemicity where infection commonly occurs in infants and children. It is also an attractive strategy for ultimately reducing hepatitis-B-associated acute and chronic liver disease in areas of lower endemicity where infections occur primarily in adolescents and adults. Integration of hepatitis B vaccine with other routine paediatric immunizations, using flexible scheduling, will enhance compliance while minimizing the need for additional resources. Clinical studies demonstrate that a very high proport

OBJECTIVES: To determine the rates and patterns of vaccination against influenza in the Victorian population. Further, to determine rates of vaccination against influenza in residents of Victorian nursing homes and hostels.
DESIGN: Between May and June of 1992 we conducted a telephone survey of 537 systematically selected Victorian residents. We obtained information on age, sex, vaccination status, and level of risk for the complications of influenza. We conducted a separate postal survey of 143 randomly selected Victorian nursing homes and hostels to establish rates of vaccination within the

About 50 percent of the patients who because of an underlying disease should be vaccinated annually against influenza, do not receive the vaccine. One of the major reasons is that they are not informed by their physicians about the need to be vaccinated. To understand the attitude of the physicians concerning influenza vaccination and the way the vaccination of these patients is organized, questionnaires were send to 250 general practitioners, 125 cardiologists and 125 pulmonologists in the Netherlands. Eighty-four percent of the questionnaires were returned. The results show that the physicia

A retrospective review of 71 paediatric patients admitted with bacterial meningitis to the King Fahad Hospital at Al-Baha, Saudi Arabia, during an 8-year period revealed a preponderance of males (67.6%) and young subjects with 88.7% being below 24 months of age. The commonest cerebrospinal fluid pathogens in the series were Haemophilus influenzae type B (HIB), Streptococcus pneumoniae and Group B-beta haemolytic streptococcus, which were responsible for 47.3, 34.5 and 9.1% of cases respectively. Neisseria meningitidis which is a major cause of meningitis in most other reports was uncommon in t

Haemophilus influenzae, one of the bacteria comprising the commensal flora of the human upper respiratory tract, is also pathogenic and causes both localized and invasive (septicemic) infections. The major focus of attention and research has been on infections caused by serotype b organisms, which cause several life-threatening illnesses in children, including meningitis and acute respiratory infection (ARI; e.g., epiglottitis, pneumonia). Type b polysaccharide-protein conjugate vaccines are at an advanced stage of development and implementation; however, these vaccines will not protect agains

We describe the use of a DNA probe for genotyping clinical isolates of Haemophilus influenzae. The probe, containing capsulation genes, differentiates between the six Haemophilus serotypes in a Southern blotting procedure. It also hybridizes with a distinctive pattern to DNA from capsule-deficient mutants of serotype b strains, while failing to hybridize to DNA from typical clinical isolates of non-serotypable H. influenzae. The probe can thus resolve issues of serotyping uncertainty such as arise, for example, when capsulate strains are found to have lost reactivity with serotyping reagents a

Recent studies in the United States and Europe have shown that Haemophilus influenzae type b polysaccharide-protein conjugate vaccines can induce protective antibody levels in young infants, but it was not clear that this would be the case in African infants, to whom H influenzae vaccines must be given at a very early age to prevent disease caused by H influenzae. Therefore, antibody responses to an H influenzae type b polysaccharide-Neisseria meningitidis outer membrane protein conjugate vaccine were measured in very young Gambian infants. In the first group (n = 85), to whom the vaccine was

There are good data to recommend routine use of vaccines against measles, rubella, tetanus, influenza, and pneumococcal infections in adults. An adolescent or an adult born after 1956 is considered to be susceptible to measles unless he or she has received two doses of live measles vaccine or has suffered a physician-diagnosed case of measles. Tetanus is largely a disease of the elderly, and there is a universal need for immunizations with tetanus toxoid. Influenza continues to be a major public health problem, and influenza vaccine should be given annually to the elderly and to those at high

Since 1985 the number of cases of systemic haemophilus influenzae type b (Hib) disease in children in the Oxford region has been recorded. This study shows that a cumulative incidence of children suffering systematic Hib infections before their fifth birthday is 1:600, 71 per cent of sufferers having meningitis. Because the incidence of haemophilus infections may be increasing in the UK and there is increasing resistance of the organism to antimicrobial therapy, the need for primary prevention through immunisation is becoming increasingly attractive. We have therefore undertaken a safety and i

A general review of vaccines is presented. The available vaccines, their composition, route and dose recommendations and side effects are discussed. Underutilization of vaccines in the adult population is the rule. Vaccines of potential use in adults are presented. The susceptibility of adults to tetanus and the need for periodic booster immunization is stressed. A detailed discussion of the role of influenza, typhoid, pneumococcal and hepatitis B vaccination is included. Post exposure prophylaxis for vaccine preventable diseases and hepatitis A is reviewed. A different dose scheduled is recom

A survey was conducted in a general pediatric practice to determine parents' attitudes to and compliance with the recommended Haemophilus influenzae type b vaccine program. Of 133 families surveyed 127 (95%) responded to the questionnaire. About one third of the parents did not have their children vaccinated. The decision against vaccination was made despite parent education, follow-up telephone contact and the pediatrician's expressed support of the vaccine program. Most of the respondents (86%) had no previous knowledge of the vaccine. The factor of greatest concern was the possibility of an

The estimated average economic cost of influenza in the United States exceeds $1 billion each year, and the actual cost is more likely on the order of $3 to $5 billion per year. Benefit-cost models generally indicate that the benefits of influenza immunization for persons aged 65 and over outweigh the cost (when one excludes the cost of medical care in the additional years of healthy life gained by immunization). The low acceptance of influenza immunization suggests that individuals do not see the benefit-cost equation in the same way that policymakers do. This article presents a benefit-cost

The treatment of patients with community-acquired pneumonia can be expensive, particularly if care is hospital-based. Cost control begins with prevention. Current influenza vaccines are about 80 percent protective, but grossly underused. Amantadine and rimantadine are effective chemoprophylactic agents against influenza A, but also underused. Use of pneumococcal vaccine is controversial, but patients who are thought to be at increased risk should be immunized. Management decisions in patients with pneumonia that have major cost implications include the need for hospitalization and choice of di
