Respiratory tract infections. Goals for 1995.

Goals to be identified for 1995, a decade hence, in the prevention, diagnosis, and management of respiratory tract infections may conveniently be divided into diagnostic goals and goals in therapy and prophylaxis. Major diagnostic goals for bacterial, viral, and mycoplasmal infections of the respiratory tract focus on the development of systems to identify microbial components, such as specific antigens or segments of DNA, using monoclonal antibody techniques or DNA probes for hybridization. Sputum cultures, in the traditional sense, should ultimately become obsolete. Management goals include

The new alpha interferons.

Alpha interferons are biological response modifiers that regulate immune function, slow cell proliferation, and inhibit virus replication. Large supplies of purified preparations are now available for clinical trials. Common toxicity includes an influenza-like syndrome to which tolerance occurs after several doses, and chronic fatigue and anorexia that may be dose-limiting. Myelosuppression is mild. Alpha interferons have established clinical activity against several human cancers, including melanoma, Kaposi's sarcoma, multiple myeloma, non-Hodgkin's lymphoma, hairy cell leukemia, and renal ce

Rational design of vaccines against enveloped RNA viruses.

The enveloped RNA viruses are responsible for many important infectious diseases both in the UK and worldwide. The most familiar of these would probably be influenza, measles, mumps, rubella, rabies, dengue and yellow fever. Conventional vaccines against all of the most widespread diseases have been available for several years, although with widely varying degrees of safety and efficacy. Although vaccines against diseases such as measles, rubella, and yellow fever have been fairly successful, all vaccines against diseases caused by this group of viruses still have several drawbacks and are in

[Vaccination of animals and human health].

Prophylactic immunization of animals against obligat and nonobligat pathogenic zoonoses benefit human health in many ways both directly and indirectly. Typical examples of a direct protective effect are the vaccinations of dogs, cats and foxes against rabies as well as the vaccinations against respiratory diseases in cows, horses, dogs and cats to which the most varied species of pathogens of noncompulsory zoonoses contribute. A considerable contribution to the protection of human health is made by the vaccination against salmonellosis and leptospirosis, against vesicular stomatitis, American

Oral route as method for immunizing against mucosal pathogens.

In the past three decades significant strides have been made in attempts at nonparenteral immunization. Appreciation of the importance of secretory immunity led to attempts to stimulate antibody production locally. The vaccines developed against respiratory pathogens as a result of this new knowledge have many practical limitations, such as the need for highly trained personnel, expensive equipment, very cooperative recipients for intranasal or aerosol administration, and a vaccine that is both adequately attenuated, immunogenic, and stable during storage. With recognition of the presence of a

Induction of cytolytic T- and B-cell responses against influenza virus infections.

Inoculation of mice with live influenza virus results in the induction of cytotoxic thymus-derived (T) lymphocytes and of bone marrow-derived (B) cells producing antiviral antibodies. An assay system was developed to evaluate both types of immune responses on a cellular basis within the same lymphocyte pool with no need to separate out the different effector cell classes. The test system represented a modification of the 51Cr-release assay. T-cell activity was measured exclusively in the absence of active complement using targets that were compatible for determinants encoded by the mouse major

Prospects for the prevention of bacterial meningitis with polysaccharide vaccines.

MOST SUPPURATIVE INFECTIONS OF THE MENINGES ARE CAUSED BY FIVE BACTERIAL SPECIES: Escherichia coli, Haemophilus influenzae type b, Streptococcus pneumoniae, Neisseria meningitidis, and group B streptococcus. The immune response of adults to pneumococcal capsular polysaccharides has been studied in great detail and their responses to meningococcal and H. influenzae type b capsular polysaccharides are quite similar. Immune responses of adults to E. coli and group B streptococcal antigens are disappointing. The responses of children below the age of 7 years differ both quantitatively and in durat

Use of influenza vaccine in non-high risk populations.

The aim of most strategies for vaccination against influenza is the prevention of mortality. Since individuals in the high risk group are mainly elderly, and the elderly have a low frequency of influenza infection, this strategy can have no significant controlling effect on morbidity. It has been shown in the longitudinal community study in Tecumseh, Michigan that highest frequency of infection with influenza is seen in the school-age population; this pattern is quite marked for type B influenza and less so for type A. In addition, it was possible in 1968-69 to show that vaccination of schoolc

Factors involved in immunization program for swine influenza.

The decision to undertake a nationwide program of vaccination against swine influenza requires assessment of the status of immunity of those in various age groups in our population against this agent. Pools of serum were collected from persons born in the years from 1889 to 1943; they were tested for hemaggultinin inhibiting (HI) antibody against the HSW 1N1 influenza virus strains isolated in 1931 and 1976. The titers secured serve as an indication of the average level of immunity of those of different ages. Persons less than 43 years of age are found to be without antibody protection. The ne

Influenza.

The most universally employed measurement of the impact of epidemics and pandemics is the excess of mortality due to influenza and pneumonia. Other criteria are absenteeism from school and work, and all three will show positive indications when epidemics are of substantial size. During the 1974-1975 influenza season in Houston, school and industrial absenteeism and the increase in influenza and pneumonia deaths, despite a newly devised statistical procedure, did not signal an epidemic. However, a system of community surveillance of febrile respiratory illness with cultures for influenza virus

Immunization for the internist.

The emphasis of immunization programs and schedules has traditionally been directed to infants and children, since most of the vaccine-preventable diseases are seen predominantly in these age groups. Immunization procedures in adults are less well defined but still of importance. Diseases for which immunizations are given before disease exposure include tetanus, diphtheria, influenza, rubella, and mumps; travelers to foreign countries may need immunizations against typhoid, cholera, yellow fever, typhus, poliomyelitis, plague, and viral hepatitis; other vaccines are available before disease ex

A TRIAL OF OIL-ADJUVANT INFLUENZA VACCINE IN A NON-EPIDEMIC SEASON.

In the autumn of 1963, 1,679 employees at two U.K.A.E.A. establishments were inoculated with a single dose (0·25 ml.) of polyvalent oil-adjuvant influenza vaccine. In a non-epidemic winter, no significant improvement in certified sickness absence could be demonstrated in the vaccinated group when compared with a randomly selected uninoculated group. In particular, no real protection was shown, in those vaccinated, against influenza and acute upper respiratory tract infections. Once respiratory sickness had occurred the vaccine showed no significant effect on the length of sickness absence. In

RESULTS OF PROPHYLACTIC VACCINATION AGAINST PNEUMONIA AT CAMP WHEELER.

1. 13,460 men, or about 80 per cent of the entire camp strength, were vaccinated against pneumonia with pneumococcus lipovaccine. 2. The dosage employed in all cases was 1 cc. of the lipovaccine containing approximately 10 billion each of Pneumococcus Types I, II, and III. 3. Both the local and general reactions produced by the vaccine were usually mild. Only 0.7 per cent of those who received the vaccine were sufficiently affected to need hospital care. None of these was seriously ill, and a majority of them returned to duty on the 2nd or 3rd day after admission. 4. Most of the troops inocula
