Differentiating pancreatic acinar cell carcinoma from solid pseudopapillary neoplasm using contrast-enhanced computed tomography.

BACKGROUND: Solid pseudopapillary neoplasm (SPN) and pancreatic acinar cell carcinoma (PACC) represent two uncommon pancreatic tumors with distinct biological behaviors and management strategies. Both entities can present with overlapping appearances on contrast-enhanced computed tomography (CECT), posing a diagnostic challenge preoperatively. Therefore, reliable imaging differentiation between these tumors is clinically important. This study aimed to evaluate computed tomography (CT) imaging features for differentiating SPN from PACC and to develop a combined model to improve preoperative dia

Rapid Radiological Disappearance of Pancreatic Duct Stones Following Oral Trimethadione.

A 59-year-old man with chronic pancreatitis, surgically altered anatomy, and an implantable cardioverter-defibrillator presented with acute exacerbation caused by cast-like obstructive pancreatic duct stones. Endoscopic therapy was considered technically difficult, and extracorporeal shock wave lithotripsy was avoided. While delayed surgical drainage was being considered, off-label oral trimethadione was started after institutional ethics approval. Computed tomography 2 weeks later showed disappearance of the stones and improvement of ductal dilatation. No adverse events occurred, and surgery

Difficult biliary cannulation during ERCP: techniques and an evidence-informed practical algorithm.

Difficult biliary cannulation remains the pivotal technical barrier in therapeutic endoscopic retrograde cholangiopancreatography (ERCP) and is a major modifiable driver of post-ERCP pancreatitis (PEP). Contemporary practice has moved from prolonged standard attempts toward earlier recognition of difficult access, guidewire-based primary cannulation, targeted rescue techniques, and routine pancreatitis prophylaxis. This review summarizes current evidence and proposes a practical, evidence-informed algorithm for native papilla ERCP. Difficult cannulation is best recognized using objective proce

Distal cholangiocarcinoma in a patient with portal annular pancreas and aberrant common hepatic artery: a case report.

BACKGROUND: Portal annular pancreas (PAP) is a rare congenital anatomical variation characterized by pancreatic parenchyma encircling the portal vein (PV) or superior mesenteric vein (SMV). An aberrant common hepatic artery (CHA) originating from the superior mesenteric artery (SMA) represents another uncommon vascular anomaly. The coexistence of these two variations with a distal common bile duct (CBD) malignancy is extremely rare and poses significant technical challenges for pancreaticoduodenectomy (PD). CASE PRESENTATION: We report a case of a 55-year-old male with obstructive jaundice ca

Impact of postoperative hyperamylasemia and post-pancreatectomy acute pancreatitis following pancreaticoduodenectomy in a single-center cohort.

BACKGROUND: The clinical significance of postoperative hyperamylasemia (POH) and post-pancreatectomy acute pancreatitis (PPAP) following pancreaticoduodenectomy (PD) is understudied. Hence, we aimed to determine the incidence, risk factors, and short-term clinical impact of post-PD POH and PPAP. METHODS: A retrospective, single-center, observational study was conducted with consecutive adult patients who underwent surgery between January 2012 and September 2025. POH and PPAP were defined and graded using the 2022 International Study Group of Pancreatic Surgery (ISGPS) consensus criteria. RES

Transmural migration of an internal pancreaticojejunostomy stent causing delayed intra-abdominal sepsis: a case report.

BACKGROUND: Internal pancreatic duct stents are commonly used during pancreaticojejunostomy (PJ) reconstruction following pancreaticoduodenectomy to facilitate drainage and potentially reduce postoperative pancreatic fistula (POPF). These stents are typically expected to migrate distally and pass without consequence. However, rare complications related to stent migration may occur and can present with atypical clinical and biochemical findings, creating diagnostic challenges. CASE DESCRIPTION: A 63-year-old woman underwent pancreaticoduodenectomy for pancreatic adenocarcinoma with placement o

Right Coronary Artery Thrombosis Causing ST-Elevation Myocardial Infarction After a Coronary Artery Disease Reporting and Data System Score of 0, With Disseminated Intravascular Coagulation During Idiopathic Acute Pancreatitis: A Case Report.

Acute pancreatitis may cause electrocardiographic abnormalities and troponin elevation that mimic acute myocardial infarction. True coronary thrombosis is less common. The development of disseminated intravascular coagulation shortly after percutaneous coronary intervention creates a difficult balance between bleeding control and prevention of early stent thrombosis. A 68-year-old man with no previous pancreatitis presented with acute interstitial pancreatitis. Evaluation did not identify biliary disease, alcohol use, hypertriglyceridemia, hypercalcemia, medication exposure, autoimmune pancrea

Ultrathin endoscope-assisted pancreatobiliary interventions: from direct transpapillary access to created access routes.

BACKGROUND: ERCP is the standard endoscopic approach for pancreatobiliary diseases, but it relies mainly on fluoroscopic assessment and cannot provide direct intraductal visualization. Ultrathin upper gastrointestinal endoscopes, originally developed for transnasal or pediatric endoscopy, have therefore been used as forward-viewing platforms for direct observation and therapy in the bile duct, pancreatic duct, gallbladder, and peripancreatic cavities. This review summarizes ultrathin endoscope-assisted interventions through both natural and created access routes, excluding dedicated mother-bab

Chyle Leak Following Pancreatic Resection: A Potential Contributor to Delayed Postoperative Recovery.

Chyle leaks following pancreatic resection are often considered less clinically significant than postoperative pancreatic fistulas (POPFs), despite their potential to delay recovery. We evaluated postoperative outcomes associated with chyle leak and explored differences in morbidity relative to POPF. A retrospective review of consecutive pancreatic resections performed at a single institution between 2017 and 2022 was conducted. Among 369 patients, 24 (6.5%) developed chyle leaks, including 16 (4.3%) isolated chyle leaks and 8 (2.2%) combined chyle and pancreatic leaks. Isolated chyle leaks we

Mimicking Polycystic Liver Disease: A Case Report of Innumerable Cystic Hepatic Metastases From a Pancreatic Neuroendocrine Tumour and Diagnostic Role of Endoscopic Ultrasound.

Pancreatic neuroendocrine tumours (PNETs) are malignant processes and are identified as being the rarest of all pancreatic cancers. Here we present the case of a 62-year-old male with a background of chronic lymphocytic leukaemia (CLL) who was investigated over 18 months for progressive hepatic cystic lesions with no known aetiology. Imaging revealed main pancreatic duct dilatation without an identifiable intrinsic or extrinsic cause, coupled with enlarged mesenteric lymph nodes. Endoscopic ultrasound identified multiple cystic liver lesions of different sizes, some reflecting the presence of

Recurrent rupture of a pancreatic mucinous cystic neoplasm with suspected main pancreatic duct communication, presenting with a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion.

Spontaneous rupture of pancreatic mucinous cystic neoplasm (MCN) is rare, particularly outside pregnancy, and mediastinal extension has not been reported. We report a 34-year-old woman with a pancreatic tail MCN demonstrating continuity with the main pancreatic duct on computed tomography (CT), suggesting ductal communication. The neoplasm ruptured dorsally, and cyst fluid extended through the aortic hiatus, resulting in a mediastinal pancreatic pseudocyst, pancreatic pleural effusion, and pericardial effusion. Pleural fluid amylase was 1541 IU/L. Endoscopic ultrasonography showed a cyst-in-cy

Repair of Main Pancreatic Duct Injury during Surgery for Congenital Biliary Dilatation in a Child: A Case Report.

INTRODUCTION: In surgery for congenital biliary dilatation, it is essential to minimize residual intrapancreatic bile duct tissue to prevent future complications, such as intrapancreatic stones and carcinogenesis. However, dissection of the intrapancreatic bile duct is technically demanding and may result in injury to the main pancreatic duct. As a result, very few cases have been reported regarding the management of this complication in pediatric patients. We present a pediatric case of intraoperative main pancreatic duct injury and describe its surgical management. CASE PRESENTATION: A 3-ye

Dorsal Pancreatic Agenesis Associated With New-Onset Diabetes Mellitus and Acute Pancreatitis in an 18-Year-Old Man: A Case Report.

Dorsal pancreatic agenesis (DPA) is a rare congenital disorder resulting from failure of development of the dorsal pancreatic bud. Complete DPA implies absence of the pancreatic neck, body, and tail and can be associated with both diabetes mellitus and pancreatitis. The patient was an 18-year-old man admitted with a history of epigastric pain, nausea, vomiting, polyuria, polydipsia, and unintentional weight loss of 4 kg over 8 weeks. The lab investigations included elevated serum glucose (412 mg/dL), urine ketones 3+, venous pH 7.31, bicarbonate 17.2 mEq/L, anion gap 14.8 mEq/L, lipase 486 U/L

Spontaneous Bacterial Peritonitis as a Rare Complication of a Pancreatic Duct Leak in a Patient Without Cirrhosis: A Case Report.

Spontaneous bacterial peritonitis (SBP) is classically associated with liver cirrhosis and portal hypertension. Its occurrence in non-cirrhotic patients is rare and poorly characterized. Pancreatic ascites, resulting from disruption of the pancreatic duct (PD) or leakage from a pseudocyst, is an unusual cause of ascites in chronic pancreatitis. The enzyme-rich, exudative nature of this fluid can itself cause chemical peritonitis with neutrophilia, mimicking infection. We report the case of a 50-year-old man with alcohol use disorder and chronic calcific pancreatitis who presented with recurren
