[The role of platelet activating factor in mesenterioangial microcirculatory disturbance complicated with acute pancreatitis in rats].

To study shock in early acute pancreatitis (AP), we observed the change of mesenterio-angial microcirculation in the rat with AP induced by sodium taurocholate infused into the pancreatic duct. Meanwhile, the effect of platelet activating factor (PAF) receptor antagonist BN52021 upon the animal with AP was also investigated. 18 Sprague-Dawley rats were divided into three groups: pancreatitis group pan + BN52021 group and control group. pan + BN52021 group was injected BN52021 (10mg/kg) intravenously 5 minutes and 180 minutes after AP induction. The other groups were injected normal saline. We

[CT findings of pancreatic carcinoma: evaluation with the combined method of early enhancement CT and high dose enhancement CT].

Computed tomographic (CT) findings of pancreatic ductal adenocarcinoma were studied with the combined method of early enhancement CT and high dose enhancement CT in 72 carcinomas. Common Findings were change in pancreatic contour, abnormal attenuation in a tumor and dilatation of the main pancreatic duct. The incidence of abnormal attenuation and dilatation of the main pancreatic duct and bile duct was constant regardless of tumor size. The finding of hypoattenuation at early enhancement CT was most useful for demonstrating a carcinoma. However, this finding was negative in ten cases, five of

Periampullary disorders: review of pathophysiology.

The ampulla of Vater is strategically located at the confluence of the terminal end of the bile duct and the pancreatic duct. It is entwined by smooth muscle fibers often referred to as the sphincter of Oddi. As a result, the ampulla demonstrates dynamic motor activity. A variety of structural and functional abnormalities can involve the ampulla and the periampullary region. Disorders involving the ampulla often produce remarkably similar clinical features, such as acute pancreatitis, biliary colic, or jaundice. Therefore, it is important that patients with periampullary disorders are systemat

[Pancreatic function and quality of life after resection of the head of the pancreas in chronic pancreatitis. A prospective, randomized comparative study after duodenum preserving resection of the head of the pancreas versus Whipple's operation].

Given an indication for surgery in patients with chronic pancreatitis, such as distal common bile duct obstruction, duodenal stenosis, or dilated pancreatic duct with stones and congestion, the surgeon must decide the type of operation to perform. A duodenopancreatectomy, the Whipple procedure, is widely considered to be the gold standard. It is highly effective in relieving pain and eliminating the structural abnormalities noted above. Duodenum-preserving resection of the head of the pancreas (DPRHP) seems to be an attractive alternative to pancreaticoduodenectomy (PD) in the treatment of chr

Gallstone-containing duodenal duplication communicating with the pancreatic duct.

A 10-year-old female patient with duodenal duplication containing gallstones and communicating with the pancreatic and bile ducts is described. Although endoscopic retrograde cholangiopancreatography and computed tomography following intravenous cholangiography showed a periampullary cystic lesion, the precise relationship between the lesion and the pancreaticobiliary ductal system was not clear. After removal of the gallstones by opening the cystic lesion, a pinhole opening noticed inside was cannulated and contrast injected, demonstrating a connection between the cystic lesion and the pancre

[Surgical treatment of symptomatic pancreas divisum].

Pancreas divisum is found in 5% of the population. It is linked to three clinical entities; recurrent epigastric pain, and acute and chronic pancreatitis. The relation between chronic pancreatitis and pancreas divisum is, however, uncertain. Pancreas divisum is controversial as a cause of acute pancreatitis and abdominal pain. However, this association probably exists as surgical sphincteroplasty of the minor papilla alleviate symptoms in a high proportion of patients. We have treated two patients surgically. Patient 1 was hospitalized 11 times because of recurrent acute pancreatitis. Two and

Is biliary lithiasis associated with pancreatographic changes?

The aetiological role of biliary lithiasis for chronic pancreatitis remains controversial. Previous studies based on pancreatographic studies reported changes in the pancreatic duct system caused by biliary lithiasis. This study analysed retrospectively the endoscopic retrograde cholangiopancreatography of 165 patients presenting with biliary lithiasis and of 53 controls. Among the 165 patients, 113 had choledochal stones (53 with gall bladder stones, 50 had had a cholecystectomy, 10 with a normal gall bladder), 35 had gall bladder stones without choledochal stones, 17 had cholecystectomy for

Pancreatic surgery in children.

Pancreatic surgery in children is a relatively uncommon occurrence, and this unfamiliarity can be associated with significant morbidity and potential mortality. Surgical therapy of pancreatic disorders has been significantly facilitated by endoscopic retrograde cholangiopancreatography, which can now be performed in children with success rates equaling those in adults. This technique delineates pancreatic duct anatomy, which is an essential part of the diagnosis and surgical treatment of important pancreatic disease processes. Pancreatic diseases requiring surgical treatment in children can be

Endoscopic approach to pancreas divisum.

Pancreas divisum has been claimed to be a harmless congenital variant or to occasionally cause acute relapsing pancreatitis (ARP), chronic pancreatitis (CP), or a chronic abdominal pain (CAP) syndrome. Both surgical and endoscopic approaches to accessory papilla decompression have been promulgated and widely disparate results reported in the literature. We retrospectively reviewed a five-year experience with dorsal pancreatic duct decompression at our institution utilizing a variety of endotherapeutic techniques. Data collected included procedural complications; patient interpretation of pre-

Aberrant biliopancreatic duct. A probable cause of acute pancreatitis?

Abnormal biliopancreatic ducts are very uncommon in adults with the exception of those associated with abnormalities of either the pancreatic or the bile ducts. The case presented herein is unique in that a biliopancreatic connection occurred proximal to the papilla of Vater and the patient was symptomatic because of the aberrant connection. The surgical therapy consisted of cholecystectomy and ligation of the aberrant duct, with complete relief of severe, debilitating symptoms. This interesting clinical observation is discussed in the light of Opie's theory of biliary reflux into the pancreat

[Chronic pancreatitis and pancreatic dysfunction associated with Sjögren's syndrome].

Some pancreatic involvement was observed in Sjögren's syndrome (SS). Episodes of abdominal pain occurred infrequently in the course of SS, but there were some abnormal findings of the pancreatic functions and/or morphologic examinations. The level of serum amylase was affected by the destruction of both the pancreas and the salivary gland. Hyposecretion of bicarbonate in secretin test seemed to be characteristic in SS. ERP showed mild or moderate changes of the pancreatic duct, but their severities did not correlate with those of the salivary gland. Imaging of the pancreas tended to reveal def

Findings of cholangiopancreatography in pseudotumorous pancreatitis.

The cholangiopancreatographic findings in 22 patients with pseudotumorous pancreatitis (PTP) were evaluated on the basis of endoscopic retrograde cholangiopancreatography (ERCP) and percutaneous transhepatic cholangiography (PTC). All patients were found with a focal mass in the pancreas based on the findings of ultrasound and/or CT. However, only one patient had been diagnosed correctly as suffering from chronic pancreatitis by ERCP. Normal pancreatic duct was found in 4 patients. There were five patients having abnormal pancreatic ducts, but findings of ERCP could not provide definite diagno

[Digestive hemorrhage undiagnosed by conventional endoscopy].

Digestive bleeding undiagnosed by conventional endoscopies occurs in about 5% of the patients with gastro-intestinal bleeding. Vascular ectasias are the most common cause. More than half of these tumors are benign. Meckel's diverticulum is the cause of bleeding in two thirds of men under the age of 30. Hemorrhage from the biliary tree or pancreatic duct occurs more often in patients with a suggestive history. According to the abundance and the cause of hemorrhage numerous investigations can be undertaken and the diagnostic value of new endoscopic methods (enteroscopy, endoscopic ultrasonograph

Pancreaticobiliary maljunction without choledochal cysts in infants and children: clinical features and surgical therapy.

Pancreaticobiliary maljunction (PBM) usually is associated with choledochal cyst. PBM without dilatation of the common bile duct is rare in infants and children. This rare type of the anomaly may lead to the development of malignancy of the bile duct in later life. The authors report the clinical presentation and surgical treatment of seven pediatric patients with PBM. Symptoms and signs included repeated episodes of abdominal pain (7 of 7), nausea and vomiting (6 of 7), intermittent jaundice (3 of 7), and acholic stools (2 of 7). Some patients presented with high levels of serum and urinary a

Left-sided gallbladder with anomalies of the intrahepatic portal vein and anomalous junction of the pancreaticobiliary ductal system: a case report.

A 66-year-old man was admitted for an early cancer in the gastric corpus. At laparotomy, the gallbladder was located on the left side of the round ligament and attached to the lateral segment of the liver. This finding was compatible with a left-sided gallbladder. Postoperative ultrasonograms and portograms demonstrated anomalies of the intrahepatic portal vein as follows; The left umbilical portion and the posterior point of the portal branch were lacking. The posterior point was seen as an umbilical portion. An endoscopic retrograde cholangiopancreaticogram showed the cystic duct entering in

Annular pancreas associated with carcinoma in the dorsal part of pancreas divisum.

A carcinoma in the dorsal part of the pancreas divisum with an annular pancreas in the anterior part is reported. A 79-yr-old female was admitted in our hospital complaining of epigastralgia. Computed tomography (CT) and ultrasound (US) showed an irregular mass in the pancreatic body. A pancreatogram obtained through the major duodenal papilla demonstrated only the ventral pancreatic duct that encircled the duodenum. Contrast medium injected from the minor duodenal papilla showed Santorini's duct obstruction at the neck portion of the pancreas without communication with the ventral pancreatic

Isolated tuberculosis of the pancreas masquerading as a pancreatic mass.

A 65-yr-old woman presented for evaluation of a pancreatic mass. She had been suffering from severe constitutional symptoms for 18 months; those symptoms included weight loss, increasing fatigue, night sweats, and recurrent fever attacks up to 40 degrees C. Later, bluish subcutaneous nodules developed on her lower limbs. Laboratory tests yielded signs of chronic inflammation and impaired glucose tolerance with elevated serum insulin and glucagon concentrations. Skin biopsy revealed lobular panniculitis.
