Allergic asthma is caused by antigen reaction with IgE fixed to mast cells of the bronchi. The reaction causes release of slow-reacting substance of anaphylaxis and histamine, which cause bronchial constriction. Prophylactic treatment includes avoidance of antigens, desensitization and use of cromolyn sodium. The latter appears to impede release of chemical mediators by the mast cell. Although of no value during an acute attack, this agent reduces attacks and reduces the need for other drugs in patients with severe perennial asthma.

An outbreak of staphylococcal bacteremia in healthy, full-term neonates occurred in the newborn nursery at a large county hospital not employing prophylactic hexachlorophene bathing. Seven infants had staphylococcal bacteremia and one had omphalitis. Two of the three isolates obtained for phage typing were type 86, and the other was 3c/71. Cromolyn sodium is a recently introduced drug used in the prophylactic treatment of severe, perennial, bronchial asthma, particularly in the pediatric age group. In a multicenter trial, 276 chronic asthmatic patients of eight pediatric allergists entered a randomized, double-blind, placebo-controlled, crossover study lasting 12 weeks. As a supplement to earlier investigations, a case of a 46 year old female patient is reported who has suffered from necrobiosis lipoidica granulomatosa with ulceration for 17 years. A marked remission was obtained by treatment with anti-inflammatory and immunosuppressive therapy. It is pointed out that under the usual treatment when ulcers of the lower leg and other areas of the skin do not heal, after exclusion of malignancies, necrobiosis lipoidica diabeticorum or necrobiosis lipoidica granulomatosa must be considered, especially as prophylactic examinations may arise.

Viral interference is a phenomenon for which a cell infected by a virus becomes resistant toward a second outcoming infection by a superinfectant virus. The role of radiotherapy and adjuvant chemotherapy in the primary treatment of osteogenic sarcomas and of Ewing's sarcoma is reviewed. In osteosarcoma radiotherapy can take the form of prophylactic total irradiation of the lung, but preoperative irradiation of the primary tumor has not proved successful. On the other hand, in Ewing's sarcoma primary and local irradiation is the therapy of choice, and is followed by adjuvant polychemotherapy over a long period.

In a large-scale double-blind controlled trial of practolol (200 mg twice daily) in the long-term prophylactic treatment of 3038 patients recovering from acute myocardial infarction treatment was started one to four weeks after the acute attack. The trial was originally planned to include 4000 patients treated for at least a year but had to be terminated prematurely because of the serious oculocutaneous and peritoneal reactions reported elsewhere. Nevertheless, important findings, probably applicable to other beta-adrenoreceptor antagonists, have emerged in relation to mortality and morbidity.

A controlled trial has been carried out in 50 patients undergoing elective colonic surgery to assess the efficacy of prophylactic oral metronidazole specifically directed against anaerobic colonic microflora in combination with kanamycin to reduce the aerobic flora. The causal prophylactic activity of several reference and experimental antimalarial compounds was assessed in sporozoite-induced infections of NMRI mice with Plasmodium berghei yoelii (strain 17X). A new drug, AH 7725, with properties similar to those of disodium cromoglycate (Intal) has the additional feature of being effective when taken by mouth. Six patients with allergic asthma were studied by bronchial challenge tests with antigen.
