Written on the Skin: Participants' Perspectives on Scarring, Discolouration and Implant Site Reactions Experienced During a Phase I Clinical Trial for a Subdermal HIV Prevention Implant in KwaZulu-Natal, South Africa.

To address challenges associated with the uptake of daily oral pre-exposure prophylaxis (PrEP) by young women at high risk of acquiring HIV, a range of novel, long-acting, slow-release products are being developed and clinically evaluated. One such technology is an annual implant of tenofovir alafenamide (TAF). This study explores the perspectives of participants from a first-in-human trial of an annual TAF implant to inform considerations for future PrEP implant innovation. The study, conducted in Durban, South Africa between 2022 and 2023, included 29 in-depth interviews (IDIs) and three foc

Use of prescription medications potentially harmful during pregnancy among women of reproductive age by pregnancy status and contraceptive use in the United States, 2009-2020.

BACKGROUND: Despite recent changes in the labeling of safety risks for prescription medications during pregnancy, information on the use of prescription medications potentially harmful or teratogenic among women of reproductive age is not known. In this study, we examine trends in the use of prescription medications potentially harmful during pregnancy based on pregnancy status and the use of contraceptives in the U.S. METHODS: We conducted a repeated cross-sectional analysis using five cycles of the National Health and Nutrition Examination Survey (NHANES), 2009-2010 through 2017-2020. Pre-Weaning Administration of a PGF2α Analogue (Cloprostenol) and Its Influence on Early Reproductive Indicators in Sows Under Commercial Farm Conditions.

Efficient reproductive management in commercial swine production requires minimising non-productive days and ensuring rapid return to oestrus after weaning. The present study evaluated the effect of pre-weaning administration of cloprostenol, a PGF2α analogue, on early reproductive indicators in sows under commercial farm conditions. A total of 734 clinically healthy multiparous sows (parity 2-9) from a commercial farm were randomly allocated to two groups before weaning. Control sows (n = 367) received no treatment, whereas treated sows (n = 367) received an intramuscular injection of clopros

Lived experiences of self-injectable contraceptive use among women in rural Malawi.

Drawing on women's lived experiences using self-injectable (SI) contraception, this study explores the method's integration into rural Malawi and the impact on women's lives. We conducted in-depth interviews with 18 SI users from Ntchisi and Mulanje districts. A two-phase analysis-first thematic, then narrative-identified benefits, challenges, and motivations for use. Our findings illustrate that many women transitioned from provider-administered injectables to SI, seeing value in its convenience and cost savings when health facilities were distant. Having multiple units at home offered protec

Epidemiological and clinical data link depot medroxyprogesterone acetate to meningioma.

Meningiomas are among the few intracranial tumors that are more common in females than males. The majority of meningiomas express progesterone receptor (PR), and differences in exposure to progestogens are thought to contribute to sex-based disparity in meningioma. Previous epidemiological and clinical data have demonstrated that multiple progestins, such as cyproterone acetate (CPA), are linked to meningioma. More recently, international epidemiological studies have found a significantly increased risk of meningioma with depot medroxyprogesterone acetate (DMPA), a widely used injectable contr

Injectable contraceptives differentially affect the hypothalamic-pituitary-gonadal axis and amenorrhea incidence†.

Hormonal contraceptives modulate the hypothalamic-pituitary-ovarian (HPO) axis; however, underlying mechanisms and differences between contraceptives are underexplored. The Women's Health Injectable Contraception and HIV trial randomised 521 women to intramuscular depot medroxyprogesterone acetate (DMPA-IM) or norethisterone enanthate (NET-EN) and showed similar decreased estradiol levels, but more amenorrhea for DMPA-IM users. This secondary study excluded for misreporting contraceptive use for 128 participants (DMPA-IM n = 65; NET-EN n = 63). Peripheral blood serum collected at initiation an

Hormonal contraception and hemoglobin levels in urban Malawi: a longitudinal analysis.

BACKGROUND: Studies have highlighted potential non-contraceptive benefits from women’s use of hormonal, reversible contraceptive methods. Given the physiological pathways through which hormonal contraception may interact with menstruation, a growing body of work has documented the extent to which hormonal method use might be associated with increased hemoglobin levels and reductions in the risk of anemia in women. While these findings are promising, the scope for causal inference from these studies is restricted by their cross-sectional design, which limits the extent to which bias from indivi

Impact of the COVID-19 Pandemic on Contraceptive Utilization and Costs in the United States: An Interrupted Time Series Analysis Using ARIMA.

OBJECTIVES: We aimed to characterize the trends in contraceptive utilization rates and unit costs of contraceptive prescriptions, and to evaluate the impact of the COVID-19 pandemic on contraceptive utilization and costs. STUDY DESIGN: We conducted an interrupted time series analysis using the IQVIA PharMetrics® Plus for Academics database from 2018 to 2022. We identified female enrollees aged 15-44 years with pharmacy coverage to estimate their monthly contraceptive utilization rate (number of contraception pharmacy claims per 100,000 female enrollees) and unit cost of contraceptive prescrip

Bayesian probabilistic projections of proportions with limited data: An application to subnational contraceptive method supply shares.

Engaging the private sector in contraceptive method supply is critical for creating equitable, sustainable, and accessible healthcare systems. To achieve this, it is essential to understand where women obtain their modern contraceptives. While national-level estimates provide valuable insights into overall trends in contraceptive supply, they often obscure variation within and across subnational regions. Addressing localised needs has become increasingly important as countries adopt decentralised models for family planning services. Decentralization has also underscored the need for reliable s

Poor mood after oral contraceptive use is associated with increased vulnerability to peripartum depression, premenstrual dysphoric disorder, and higher genetic risk for depression.

PURPOSE: This study tested whether adverse mood effect of the oral contraceptive pill (OCP) is associated with reproductive depressive episodes, including peripartum depression (PPD), premenstrual dysphoric disorder (PMDD), and perimenopausal depression. METHOD: In a sample of 3,547 OCP users from the Australian Genetics of Depression Study, who reported a lifetime depression diagnosis, logistic regression was used to test the association of PPD, PMDD, and perimenopausal depression with OCP adverse mood effect. Polygenic scores (PGS) for major depression (MD) were also tested for association

Influence of oral contraceptive/low-dose estrogen progestin discontinuation on the incidence of alveolar osteitis after third molar extraction: a retrospective study at a single center.

Oral contraceptive (OC)/low-dose estrogen progestin (LEP) affects both the coagulation and fibrinolytic systems and may cause alveolar osteitis (AO). However, whether OC/LEP discontinuation affects the incidence of AO has not been evaluated. The aim of this study was to investigate whether preoperative OC/LEP discontinuation influences the incidence of AO after wisdom tooth extraction in outpatients. A total of 130 women aged 18-47 years (median, 27 years) participated, and the total numbers of procedure days and teeth were 205 and 274, respectively. We compared the incidence of AOs between pa

Universal Free Contraception Coverage Policy, Out-of-Pocket Payments, and Costs.

IMPORTANCE: While most individuals in Canada have some form of drug coverage, many public and private plans leave a substantial role for direct payment by individuals. For contraceptives, cost-sharing (deductibles and copayments) and the exclusion of some contraceptives from formularies often leads to access gaps and inequity. How universal, first-dollar contraceptive coverage policies change patient and public costs compared with a mixed public-private system is unclear. OBJECTIVE: To evaluate whether patient out-of-pocket (OOP) costs and payer type changed after the implementation of a univ

The association between depot medroxyprogesterone acetate and meningiomas: Emerging data and their relevance to the South African context.

In the past 2 years, evidence suggesting an association between depot medroxyprogesterone acetate (dMPA) use and meningioma has emerged. As dMPA remains one of the most widely used contraceptive methods worldwide, this finding has led to increasing media attention, and regulatory and legal proceedings. The South African Health Products Regulatory Authority (SAHPRA) has issued a statement acknowledging the association between dMPA exposure and the development for meningiomas. However, the paucity of local data on dMPA use and the incidence of and risk factors for meningioma in both the public a

Comparison of sperm concentration in fresh and postal post-vasectomy semen samples: a prospective agreement study.

BACKGROUND: Most guidelines recommend post-vasectomy semen analysis (PVSA) on fresh samples to confirm success. Postal submission increases compliance although reliability remains controversial. We assessed agreement between first PVSA sperm concentrations on the same sample, freshly and following postage, and determined if any sperm concentration levels other than 'no sperm seen' on a postal sample could be used to advise cessation of other contraceptive methods. METHODS: 12 weeks after vasectomy, men submitted fresh semen samples to laboratory A. Samples were analysed within 2 hours. The re

Leukemia in users of contemporary hormonal contraception: A nationwide registry-based cohort study among premenopausal women in Denmark.

BACKGROUND: Sex hormones have been implicated in leukemogenesis, but evidence regarding hormonal contraceptive use and leukemia risk remains limited and primarily based on older formulations. Given the widespread use of contemporary hormonal contraceptives, clarification of this potential association is needed. This study examines the association between contemporary hormonal contraceptives and leukemia risk. METHODS AND FINDINGS: In a nationwide cohort design, we assessed associations between the use of contemporary hormonal contraceptives and the risk of leukemia based on a cohort of all wo

Association of Levonorgestrel-Containing Emergency Contraceptive Pills with Breast Cancer in Premenopausal Latin American Women: The PRECAMA Study.

BACKGROUND: The emergency contraceptive pill (ECP) contains progestin, which can have proliferative effects when used intermittently and thus increase the risk of breast cancer. METHODS: We investigated the association between the use of ECPs and breast cancer risk in premenopausal Latin American women from 2012 to 2023. We hypothesized that ECP use increases breast cancer risk. We used data from a case-control study (the PRECAMA study) conducted among premenopausal Latin American women. We evaluated variables such as having ever used an ECP, sociodemographic factors, anthropometric and lifes

Contraceptive and condom use among women from key populations in Myanmar.

BACKGROUND: Contraceptive and condom use remain low among women in marginalized communities, including those who sell sex, use or inject drugs, or are sexual partners of people who use or inject drugs (key populations). This study examines how selling sex and drug use relate to contraceptive and condom use with different partner types in Myanmar, alongside structural risk contexts. METHODS: We analyzed a cross-sectional survey among women from key populations (women who sell sex, use drugs, or are sexual partners of men who use drugs) recruited through community-based organizations across fou
