Levonorgestrel intrauterine system: do the contraceptive benefits outweigh the risks in light of new FSRH guidance?

BACKGROUND: The levonorgestrel-releasing intrauterine system (LNG-IUS) is a long-acting reversible contraceptive. In 2023, the Faculty of Sexual and Reproductive Healthcare (FSRH) extended its recommended duration of use from 5 to 8 years. This change underlines the need to re-examine the balance of efficacy and risks associated with LNG-IUS. AIM: To systematically review evidence on contraceptive effectiveness, safety, and acceptability of LNG-IUS, and evaluate its alignment with updated FSRH guidance. METHOD: A systematic search of MEDLINE, Embase, PsycINFO, CINAHL, and Scopus (April 2025)

Managing healthcare for female BRCA carriers in the population screening era: developing a harmonized national policy for surveillance and risk-reduction.

BACKGROUND: The implementation of Israeli National screening for BRCA1/BRCA2 pathogenic variants (PVs) in 2020 has led to a significant increase in the number of unaffected female carriers who are referred to high-risk surveillance clinics (HRSCs). Lack of standardization in protocols for risk reduction and surveillance between HRSCs results in confusion and gaps in care. We aimed to identify discrepancies in existing practices and lead to policy development of a national policy for surveillance, management and risk reducing strategies in BRCA-PV carriers. METHODS: A comparative analysis of ri

Preventive Health Services in Reproductive-Aged Women with Diabetes Mellitus: A Scoping Review.

BACKGROUND: Diabetes mellitus (DM), both type 1 (T1DM) and type 2 (T2DM), complicates about 1% of pregnancies in the US. Current research frequently focuses on the effects of DM on pregnancy outcomes, overlooking the opportunity to examine a woman's health prior to pregnancy despite the significance of pre-conception health on pregnancy outcomes. METHODS: Recognizing the importance of a woman's overall health, and not just the period in which a woman is pregnant, we set out to explore whether women with DM received recommended preventive health services at the same rates as women without DM.

Progestin therapy in premenopausal women with incidental meningioma-a narrative review and recommendations for women's health specialists.

OBJECTIVE: To review recent evidence on the association between progestin exposure and meningioma risk and to propose practical recommendations for hormonal management in women requiring progestin therapy. METHODS: A narrative review of studies published between 2015 and 2025 evaluating the relationship between exogenous progestins and meningioma development, growth, or progression was performed. Evidence regarding different progestin compounds, cumulative exposure, reversibility after discontinuation, and implications for gynecologic practice was analyzed. RESULTS: Meningiomas account for m

Informational and financial barriers at hospitals advertising abortion services.

OBJECTIVES: To assess informational and financial barriers to abortion access at hospital facilities that advertise services by examining required calls, hold times, transfers, appointment availability, and costs encountered by callers seeking information about care. STUDY DESIGN: We used a mystery-caller design, posing as patients seeking abortions at a gestational age of 6 weeks, to contact all U.S. facilities currently advertising abortion services (N = 772). The primary outcome was whether hospital facilities (N = 48) presented informational barriers to abortion access. Secondary outcomes

Characterizing endometriosis and adenomyosis symptom clusters and their impact on quality of life in the All of Us Research Program.

STUDY QUESTION: Are there distinct symptom clusters in women with endometriosis and adenomyosis, and how do these clusters predict quality of life? SUMMARY ANSWER: Latent class analyses of 22 438 women with endometriosis identified four symptom phenotypes, including high pain-gastrointestinal and psychological-neurological clusters; adenomyosis cases were concentrated in high‑pain classes without a minimal‑symptom group, and high‑burden phenotypes had poorer quality of life. WHAT IS KNOWN ALREADY: Endometriosis and adenomyosis are heterogeneous conditions with diagnostic delays of 4-11 years

Spatial distribution, integration and determinants of family planning service provision in Lubumbashi, DRC: a cross-sectional analysis from a health facility census.

BACKGROUND: Universal access to family planning (FP) services is essential for safeguarding sexual and reproductive health and rights. Integration of FP and routine maternal and child health (MCH) services has been associated with increased uptake of long-acting reversible contraceptives and contributes to reduced unintended pregnancies, unsafe abortions and complications associated with short interpregnancy intervals. In the DRC, FP use remains limited despite national efforts to expand services, and rapidly growing cities such as Lubumbashi face additional challenges due to health system fra

Multidisciplinary Management of Women Suffering from Migraine: Rationale, Design and Results of a National Delphi Consensus.

Background/Objectives: Migraine is a common, disabling neurological disorder disproportionately affecting women during hormonally sensitive phases like menarche, pregnancy, and menopause. Despite awareness of sex-specific risk factors, management remains fragmented and predominantly neurologist-led, with limited coordination. To address these gaps, this project aimed to develop a national expert consensus, endorsed by scientific societies, on multidisciplinary migraine management in women across the lifespan, integrating perspectives from neurology, gynecology, endocrinology, cardiology, and g

Strategies for the Care of Mothers With Substance Use Disorder.

Substance use disorder and opioid use disorder in pregnancy have serious maternal and neonatal sequelae, including neonatal abstinence syndrome, preterm labor, and intrauterine growth restriction. Despite this, there are limited resources and guidelines to help clinicians support these mothers in all practice settings. The primary aim of this review will be to provide evidence-based clinical, practitioner, and sociopolitical recommendations for obstetric teams to implement. A holistic approach that includes multidisciplinary team-based care, a stepwise approach to chronic pain management, trea

Hormonal contraceptives and the risk of meningioma: A Swedish register-based case-control study.

BACKGROUND: Studies of hormonal contraceptives and the risk of meningioma has often relied on self-reported information, primarily focusing on oral contraceptives. Recently, an association between progestins and meningioma was suggested. We aimed to analyze the association between hormonal contraceptives and meningioma using data from Swedish national registries. METHODS: We used the Swedish Cancer Register to identify women born 1955-1995, diagnosed with meningioma at age ≥20 years, between 2007 and 2015 (N = 1055). Twenty controls per case were randomly selected from the Swedish population

Nonoperative Management of Hepatic Adenomas: A Review.

Hepatic adenomas (HAs) are relatively common benign liver tumors that carry small risks of hemorrhage and malignant transformation, both of which increase with tumor size. While traditionally linked to estrogen-containing oral contraceptives (OCPs), other risk factors including obesity have been associated with HA formation and growth. Treatment strategies depend on the risk of malignant transformation or hemorrhage, tumor size, histological subtype, and patient-specific factors. While traditionally, surgical resection has been the standard recommendation for large, symptomatic, or high-risk t

Kallmann Syndrome in a 30-Year-Old Female With Primary Infertility: A Case Report.

Kallmann syndrome (KS) is a rare genetic disorder characterized by hypogonadotropic hypogonadism and anosmia. This case report highlights a 30-year-old woman with KS who presented with secondary amenorrhea and uterine hypoplasia after repeated use of combined oral contraceptives (COCs). She had a 5-year history of primary infertility, hyposmia, and primary amenorrhea. Initially, it was unclear whether her amenorrhea was primary or secondary. However, after interviewing her mother, it was determined that she had primary amenorrhea. Physical examination showed a normal female appearance, with Ta

Glucagon-Like Peptide-1 Receptor Agonists and Reproductive Health: A Narrative Review for Obstetrician-Gynecologists.

The expanding use of glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and dual glucose-dependent insulinotropic polypeptide (GIP)/GLP-1 RAs for obesity and type 2 diabetes has important implications for reproductive-aged patients. As these medications are increasingly prescribed before conception and after delivery, obstetrician-gynecologists need practical guidance on contraception, pregnancy planning, inadvertent pregnancy exposure, medication discontinuation, gestational weight trajectory, postpartum use, and lactation. This narrative review synthesizes current evidence on GLP-1 RA and
