This is a report of initial experience gained with the transgastric diagnostic examination of the pancreas in a total of 80 patients. The examinations were carried out with two different instruments. One of the two fiberscopes carries on its tip a mechanical sector scanner, the scan plane of which is angulated through 90 degrees vis-a-vis the longitudinal axis of the instrument. The other ultrasonic endoscope is provided with an ultrasonic linear array assembly arranged along the longitudinal axis of the instrument. This paper presents clinical observations on 130 patients with carcinoma of the pancreas, including 19 with periampullary cancer, the first large study of this disease in Iraq. All of the patients were admitted to the First (one of three) Surgical Unit of the University of Baghdad Teaching Hospital (the Medical City) during the period 1965-1977. A soluble somatostatin binding factor was detected in cell-free extracts from chicken pancreas. For binding measurements Tyr1-somatostatin was radio-labeled with 125I by the lactoperoxidase technique. Specific radioactivity of about 18.5 MBq/nmol was achieved. Pancreatic islet cell antibodies and other autoantibodies were measured in 68 Nigerian diabetics and 68 non-diabetic controls matched for age and sex. Islet cell antibodies measured at the time of diagnoses, were found in only two diabetics (a male aged 20 on insulin and a male aged 41 years on tablets) out of 30 newly diagnosed cases. Drum-dried calf stomach waste (DDSW) from rennet production, after washing with water for salt elimination, as well as drum-dried porcine pancreas waste (DDPW) from protease extraction were used. Eighteen 2-year-old hens were infected orally with wild avian encephalomyelitis virus and examined using the immunofluorescent-antibody technique for 30 days. Viral antigen was detected in 10 organ tissues (liver, spleen, kidney, pancreas, and ovary) in 7 birds during the first half of the observation period and sporadically in alimentary tracts of 12 birds throughout this period, especially the lower parts of the intestines, but it was not detected in other organ tissues. Thirteen test-positive hens showed more localization of the antigen than baby chicks of other studies did.

The study was performed in 50 human autoptic pancreata. After postmortem ductography and formalin fixation the organs were cut in slices of 8 mm thickness. After paraffinembedding histological sections were made from these slices. Carcinoma of pancreas is not uncommon in Africa where the prognosis seems to be worse than in cases seen in Europe and North America. The reason for this poor prognosis is because patients with this disease are seen in medical institutions at late stages.There seems to be a rise also in the incidence of this disease in our environment. This increased incidence may be real or apparent, and may be related to the rise in hospital consciousness in the population.As seen in Ibadan, the pattern of the disease is not much different from that reported from other parts of Africa.

Autologous and allogenic transplantations of the caudal part of the pancreas into the omentum and the spleen were performed in 18 dogs. The fate of the transplants was followed histologically. In the grafts the Langerhans-islands preserved their integrity longer than in the other parts of the organ, but in 4 to 6 weeks they, too, necrotised.
