Eustachian tube scoring criteria for balloon Eustachian tuboplasty.

INTRODUCTION: Balloon Eustachian Tuboplasty (BET) is a novel technique useful for the treatment of Obstructive Eustachian Tube Dysfunction. This article introduces new clinical scoring criteria for diagnosis of Obstructive Eustachian Tube Dysfunction (OETD) and as a selection criterion for BET. METHODOLOGY: A prospective observational study was conducted in which 14 patients (23 Eustachian tubes), with grade 2 and 3 pars tensa retractions or unresolved otitis media with effusion with OETD underwent BET. MAMC Clinical Eustachian tube score was used for preoperative assessment, diagnosis and se

Office-Based LA-BET Without Sedation or Nerve Block: Prospective Evaluation of a Simplified Local Anesthesia Protocol.

Background/Objectives: Balloon eustachian tuboplasty (BET) is an effective surgical option for obstructive eustachian tube dysfunction (OETD). However, the feasibility of performing BET under local anesthesia (LA) using simplified analgesic protocols remains underexplored. We examined the feasibility of a streamlined LA-BET protocol. Methods: Fifty patients (sixty-four ears) diagnosed with primary OETD between March 2024 and December 2025 were enrolled. All patients underwent BET under LA using intramuscular ketorolac and topical lidocaine gel without sedation or nerve blocks. Pain scores, blo

Eustachian Tube Balloon Dilation in Children: Prospective Clinical Outcomes of a Low-Cost Endovascular Balloon.

OBJECTIVE: Balloon dilation of the Eustachian tube (BDET) is increasingly used to treat pediatric obstructive Eustachian tube dysfunction (OETD); however, its broader adoption is limited by the cost of commercially available devices. Standard endovascular balloons may offer a cost-effective alternative, but pediatric outcome data are limited. This study evaluated the feasibility, safety, and early clinical outcomes of pediatric BDET performed using standard endovascular balloon catheters. METHODS: This prospective clinical study included 14 pediatric patients (≥5.5 y) with bilateral OETD (28

Histopathological changes of the Eustachian tube mucosa following balloon Eustachian tuboplasty in children: insights from standard endovascular balloon application.

Balloon Eustachian tuboplasty (BET) has emerged as an established intervention for obstructive Eustachian tube dysfunction; however, the high cost of proprietary balloon systems limits widespread accessibility. Standard endovascular balloons represent a low-cost alternative, yet their tissue-level effects remain insufficiently characterized, particularly in pediatric populations. This study aimed to evaluate histopathological changes of the Eustachian tube (ET) mucosa before and after BET using a standard endovascular balloon. Twenty-two Eustachian tubes (ETs) from eleven children with bilater

Evaluation of Safety and Effectiveness for Hysteroscopically Assisted Falloposcopic Tuboplasty in Women with Proximal Tubal Infertility.

OBJECTIVES: Tubal infertility accounts for approximately 30% of infertility cases in women. Falloposcopic tuboplasty (FT) is used in women with proximal tubal obstruction. However, it is technically challenging because of difficulties in locating the tubal ostium and confirming success. To address these challenges, we developed a hysteroscopically assisted FT (HA-FT). This study aimed to evaluate the efficacy of HA-FT in women with tubal infertility. MATERIALS AND METHODS: This prospective unblinded study included infertile women with proximal tubal obstruction who desired a natural conceptio

In-office balloon dilation for baro-challenge-induced Eustachian tube dysfunction: study protocol for a randomized sham-controlled trial.

BACKGROUND: Balloon dilation of the Eustachian tube (BDET) is a potential treatment for Eustachian tube dysfunction (ETD). However, high-quality evidence for its efficacy in baro-challenge-induced ETD (BCETD) is lacking. Blinded, sham-controlled trials are critical in this context, particularly given the largely subjective nature of this condition. The objective of this study protocol is to test the hypothesis that BDET is superior to a sham procedure in reducing symptoms of BCETD. METHODS: This randomized, sham-controlled, multicenter trial will be conducted at multiple tertiary care centers

Eustachian Tuboplasty as an Adjunct Treatment to Concurrent Tympanostomy Tube Insertion for Otitis Media With Effusion: A Systematic Review.

OBJECTIVES: Chronic otitis media with effusion (COME) is defined as an accumulation of nonpurulent fluid within the middle ear space without evidence of an acute infection. A primary factor in the development of COME is eustachian tube dysfunction. Tympanostomy tube insertion (TTI) has long been an established and effective means of treating OME; eustachian tuboplasty (ET) has emerged as another treatment measure. There are limited studies that combine TTI with ET and compare the effectiveness of combined treatments to TTI alone. The focus of this systematic review is to assess if the adjuncti

Sinonasal Inflammatory Burden on Computed Tomography in Adults With Obstructive Eustachian Tube Dysfunction: A Case-Control Study.

INTRODUCTION: Chronic rhinosinusitis (CRS) and Eustachian tube dysfunction (ETD) frequently coexist, yet objective data quantifying their relationship remain scarce. Because both conditions can reflect shared mucosal inflammation, we investigated whether adults with objectively verified ETD harbor a larger paranasal sinus inflammatory burden on computed tomography (CT) than controls, and whether this burden varies by drainage pathway or ear laterality. METHODS: This single-center, retrospective CT case-control study compared 140 adults who underwent balloon tuboplasty for obstructive ETD (Jan

Balloon Dilation of the Eustachian Tube: Past, Present, and Future.

Balloon dilation of the Eustachian tube has moved from an experimental concept to a mainstream option for selected adults with chronic obstructive Eustachian tube dysfunction that persists despite optimized medical management. Contemporary practice emphasizes careful phenotyping (obstructive vs patulous vs baro-challenge induced disease), correlation of symptoms with objective middle ear findings, and endoscopic evaluation of the nasopharyngeal orifice before intervention. Randomized trials and subsequent long-term follow-up demonstrate meaningful improvements in patient-reported symptoms and

[Otitis media with effusion in children: research advances from microecological mechanisms to precision diagnosis and treatment].

Otitis media with effusion(OME) is a prevalent otologic condition in children that may lead to persistent hearing impairment and speech-language developmental delays. Recent studies have demonstrated that nasopharyngeal-middle ear microecological dysbiosis plays a central role in OME pathogenesis and chronicity. Advances in precision diagnostics-including AI-assisted otoscopy, wideband acoustic immittance, and electronic nasopharyngoscopy-have improved early detection, while individualized interventions such as microbiota-targeted probiotics and balloon Eustachian tuboplasty offer promising st

Balloon dilatation of the Eustachian tube for obstructive Eustachian tube dysfunction in adults.

BACKGROUND: Eustachian tube dysfunction (ETD) causes symptoms and signs of pressure dysregulation in the middle ear, and is associated with tympanic membrane retraction, otitis media with effusion, and chronic otitis media. Interventions aiming to improve symptoms can be non-surgical or surgical, including balloon dilatation of the Eustachian tube, also known as balloon eustachian tuboplasty (BET) for obstructive ETD. However, existing published evidence for the effectiveness and safety of BET remains unclear. OBJECTIVES: To evaluate the effects of balloon dilatation of the Eustachian tube in
