Inverted microscopy-based assessment reveals major maturation gap between denuded and non-denuded GV oocytes: A novel r-IVM approach.

OBJECTIVE: To compare the maturation rate and developmental potential of immature oocytes subjected to and spared from cumulus-oocyte complex (COC) denudation. MATERIALS AND METHODS: This single-center prospective observational study was conducted between 15 November-15-December 2025. Germinal vesicle (GV) oocytes were allocated to two groups: Group 1 included oocytesobtained from follicles >10 mm and identified as GV following denudation, whereas group 2 included immature oocytes retrieved from non-dominant follicles with diameter <10 mm, and assessed under an inverted microscope immediately

Using deep learning to predict the sex of human embryos.

The existence of sex differences in human pre-implantation development remains an open question, with previous attempts based on human observations yielding inconclusive results. In this study, we combined manual annotation and deep learning analysis of a dataset comprising 515 time-lapse embryo movies to investigate whether birth sex influences early developmental dynamics. While manual assessment did not identify any developmental timing parameters that reliably distinguish male from female embryos, a deep learning model trained and tested on these videos achieves a statistically significant

Compassionate transfer: patient requests for embryo transfer for nonreproductive purposes: an Ethics Committee opinion.

Compassionate transfer refers to patient requests to transfer embryos into their bodies in a location or at a time when pregnancy is not expected to occur and reflects their personal preferences and values. It is ethical for physicians to honor or decline such requests if they do so in a nondiscriminatory and unbiased manner. This document replaces the document of the same name, last published in 2020 (Fertil Steril 2020; 113:62-5).

Impact of rapidly cleaving embryos on blastocyst formation and single-blastocyst transfer outcomes.

BACKGROUND: Clarifying the impact of day-3 cell number on blastulation and subsequent pregnancy outcomes is essential for optimizing blastocyst selection criteria and refining embryo assessment protocols. While slow cleavage on day 3 is well-recognized as detrimental, the prognostic value of rapid cleavage (>8 cells) remains ambiguous. METHODS: This retrospective cohort study (January 2015-April 2024) included 64,853 embryos undergoing blastocyst culture (Cohort 1) and 2,669 single-blastocyst frozen embryo transfer (FET) cycles (Cohort 2) at a large tertiary assisted reproduction center. Treatment management algorithm for natural frozen embryo transfer cycles using a real-time ovulation prediction machine learning model.

Our aim was to develop an AI-based NC-FET Treatment Management Algorithm (NTMA), decision-support system that predicts ovulation in real time to manage and optimize natural frozen embryo transfer cycle (NC-FET) scheduling. The algorithm was developed using a “teacher-student” machine learning approach and was trained on a total of 3,975 labeled NC-FET, including 3,432 training cycles and 543 test cycles. A second test group included 166 documented ovulation cycles (documented follicular rupture and LH surge in two consecutive days of ultrasound scans). The algorithm showed high ovulation detec

Acrylamide-induced oxidative stress triggers organelle dysfunction, DNA damage, and apoptosis in porcine embryo development.

Acrylamide (ACR) forms in carbohydrate-rich foods during cooking at high temperatures and is a common contaminant in the diet. Although ACR has neurotoxic, genotoxic, and carcinogenic effects, limited information is available regarding its reproductive toxicity. This study was performed to investigate the effects of ACR exposure on porcine embryonic development and to elucidate the underlying mechanisms. Following parthenogenetic activation and in vitro fertilization, ACR exposure significantly reduced developmental parameters, including the cleavage rate, proportion of 4-cell stage embryos, a

Enhanced predictive performance of artificial intelligence in individualized ovarian stimulation of in vitro fertilization: a retrospective cohort study.

BACKGROUND: Over 2.5 million cycles in vitro fertilization (IVF) are conducted annually, and numbers are expected to rise with the aging population. The controlled ovarian stimulation (COS) process, key to IVF success, is inherently complex. Given advances in artificial intelligence (AI), this study investigated whether a series of AI models can outperform traditional clinical practices in predictive accuracy and COS optimization. METHODS: This retrospective cohort study analyzed first-cycle ovarian stimulation patients (Oct 2017-Dec 2020) and was validated using an independent cohort (Jan 20

Ejaculatory frequency and male fertility: a literature review of evidence-based recommendations.

BACKGROUND AND OBJECTIVE: Ejaculatory frequency is a modifiable behavioral factor known to influence semen quality and male fertility. While abstinence may increase semen volume and total sperm count, extended intervals can impair sperm motility and DNA integrity. The optimal abstinence duration for natural conception and assisted reproductive technologies (ARTs) remains under active investigation. This literature review evaluates the impact of ejaculatory frequency and abstinence length on key sperm parameters and fertility outcomes and aims to inform evidence-based guidance recommendations f

Algorithm for azoospermic men with normal follicle stimulating hormone (FSH) levels and an unclear clinical diagnosis of obstructive versus non-obstructive azoospermia.

BACKGROUND: In men with azoospermia, it is critical to differentiate between obstructive azoospermia (OA) and non-obstructive azoospermia (NOA). Clinical and laboratory cues such as testicular volumes, dilatation of the epididymides, semen volumes, historical or physical examination clues of etiologies of azoospermia, and the serum follicle stimulating hormone (FSH) level can help differentiate between OA and NOA. However, in some cases there are mixed findings making the differentiation more challenging. The American Urologic Association/American Society for Reproductive Medicine (AUA/ASRM) g

Comparison of community-level deprivation indices and their association with adverse pregnancy outcomes.

BACKGROUND: The Social Vulnerability Index, Maternal Vulnerability Index, and Area Deprivation Index measure community-level resilience to stressors, vulnerability to adverse maternal health outcomes, and socioeconomic disadvantages, respectively. These indices are commonly used to gauge community-level influence on health outcomes. Thus, for them to serve as effective clinical tools, their ability to predict adverse obstetrical outcomes must be evaluated. OBJECTIVE: This study aimed to compare the 3 indices and their associations with adverse pregnancy outcomes. STUDY DESIGN: This was a sec

Addition of Multi-Level Technologies to Evaluate Eggs and Embryos and Improve Endometrial Quality Applied for In Vitro Fertilization to Achieve Pregnancy: A Case Study.

Preimplantation genetic testing for aneuploidies (PGTA) diagnoses chromosomal abnormalities and can be complemented by cumulus cell transcriptional analysis to assess egg and ovarian quality and development during embryo culture. In addition to selecting the best available embryo, intrauterine administration of platelet-rich plasma (PRP) and correction of insulin resistance (IR) through nutritional intervention can improve endometrial quality. This case report examines the potential feasibility of combining in vitro fertilization (IVF) with four techniques to achieve a viable pregnancy. A coup

The association between triglyceride glucose-body mass index and live birth outcomes in women with clinical pregnancy after frozen-thawed embryo transfer: a cohort study.

BACKGROUND: Metabolic syndrome is common among infertile women and is closely related to insulin resistance (IR). Triglyceride glucose-body mass index (TyG BMI) is a newly explored indicator of IR. Currently, the association between TyG BMI and live birth outcomes has been studied in women with polycystic ovary syndrome (PCOS) undergoing in vitro fertilization (IVF), However, there is a lack of research on the general population of frozen-thawed embryo transfer (FET). METHODS: This is a cohort study based on data from a reproductive center in China from January 2016 to December 2023. The assoc

Spatial dynamics of mTOR pathway activity during bovine embryo development.

The mTOR pathway regulates cell proliferation, growth, survival, and metabolism by integrating nutritional and growth factor signaling. In embryos, its activity is influenced by the availability of nutrients in the culture medium, and it can affect the first cellular differentiation event, driving trophectoderm (TE) formation in mice. We hypothesized that mTOR activity is increased in cells poised to become TE and in differentiated TE cells of early bovine embryos. To test this, we assessed mTOR pathway activity through immunofluorescence detection of phospho-S6 (pS6) using confocal microscopy

Optimizing Rat In Vitro Fertilization for Rat Model Cryo-Resuscitation from Frozen-Thawed Sperm.

Optimizing cryo-resuscitation from frozen sperm would improve access to cryopreserved rat models. In this study, the possibility of replacing intracytoplasmic sperm injection (ICSI) with in vitro fertilization (IVF) for model cryo-resuscitation from frozen-thawed sperm was investigated. Rat IVF protocol was modified to allow the procedures to be performed during a 9 h workday. The possibility of genetic background-specific modification of the superovulation protocol for the improvement in IVF outcomes was explored. Wild-type and genetically modified Sprague Dawley (SD), Long Evans (LE) and Fis

Lipid remodeling and circulating semaphorin 3A in diminished ovarian reserve.

Diminished ovarian reserve (DOR) lacks strong plasma biomarkers, and recent studies have implicated lipid remodeling in its pathophysiology. In our previous study, we noted elevated circulating levels of semaphorin 3A (SEMA3A) in women with DOR, especially among those who achieved beta-human chorionic gonadotropin (β-hCG) positivity following in vitro fertilization. In this study, we explored whether there are correlations between increased SEMA3A levels and the lipid profile in DOR compared to healthy controls. We performed integrated untargeted metabolomics of plasma lipid and amide classes
