Structured postmortem genetic counselling and testing in sudden unexplained death among young adults in the Netherlands: results and implications regarding hereditary risk.

INTRODUCTION: Sudden unexplained death (SUD) in young adults may result from hereditary cardiovascular disorders that remain undetected even after conventional autopsy. Routine postmortem genetic testing may identify these disorders, improving determination of the cause of death while providing diagnostic and preventive opportunities for surviving blood relatives. METHODS: Between September 2022 and October 2025, 100 cases of SUD in young adults in the Netherlands were investigated using the NODOV (Nader Onderzoek naar de DoodsOorzaak bij (jong)Volwassenen) procedure. Genetic counselling was

Child Neurology: Neonatal Management of Tyrosine Hydroxylase Deficiency: How Early Is Early Enough?

Tyrosine hydroxylase deficiency is a rare, treatable, autosomal recessive disorder of monoamine neurotransmitter biosynthesis that typically presents in infancy or early childhood. Despite potential benefits of presymptomatic treatment with levodopa/carbidopa, prenatal or neonatal diagnosis remains extremely rare. We report 2 preterm neonates from unrelated families, both with known family history of tyrosine hydroxylase deficiency. After early diagnosis, levodopa/carbidopa therapy was initiated during the neonatal period. This proactive approach proved feasible and well tolerated, leading to

Addressing stigma in differences in sex development: twenty years of community genetics practice by the Sudanese Intersex Working Group (SIWG).

BACKGROUND: Genetic diagnosis in differences in sex development (DSD) intersects profoundly with social, cultural, and institutional stigma, particularly in settings where consanguinity, traditional gender norms, and limited genetic literacy converge. OBJECTIVE: To examine how the Sudanese Intersex Working Group (SIWG) identifies and mitigates DSD-related stigma across the life course through two decades of community-embedded practice. METHODS: This Short Communication synthesizes longitudinal clinical, psychosocial, and advocacy experience from the SIWG, a multidisciplinary initiative that

Familial 46,XX Ovotesticular Differences in Sex Development Associated With an NR5A1 p.Arg92Trp Variant.

INTRODUCTION: NR5A1 encodes steroidogenic factor 1 (SF-1), a key regulator of adrenal and gonadal development, steroidogenesis, AMH transcription, and the balance between pro-testicular and pro-ovarian pathways. The recurrent NR5A1 c.274C>T (p.Arg92Trp) variant is associated with SRY-negative 46,XX testicular and ovotesticular differences in sex development (DSD), but intrafamilial expression remains difficult to predict. Case Presentation We report a rare multigenerational familial presentation of SRY-negative 46,XX ovotesticular DSD associated with the heterozygous NR5A1 p.Arg92Trp variant.

Thalassaemia prevention in Asia: progress, challenges, and future directions.

Thalassaemia remains a major public health burden in Asia, where high carrier frequencies, diverse mutation spectra, and variable healthcare capacity continue to influence disease prevention and control. This Series paper summarises the status of thalassaemia prevention programmes across 14 Asian countries and highlights the key challenges and opportunities affecting their implementation. Singapore, Malaysia, the Maldives, and Thailand have established the most structured prevention strategies, incorporating combinations of public awareness, carrier screening, genetic counselling, and prenatal

From Variant Detection to Clinical Actionability: Expanding the Clinical Perspective on Targeted NGS Profiling in Breast Cancer.

Plain Language SummaryBreast cancer is not a single disease. Each tumour carries its own set of genetic changes, and reading these changes can help doctors choose the most suitable treatment. A recent study from Ukraine used a modern genetic test to examine tumours from breast cancer patients - one of the first efforts of its kind in that country. It found that certain gene changes were common, especially one that tends to appear in slower-growing, more treatable tumours. In our letter responding to that study, we make one main point: finding a gene change is only the first step. What matters

Inherited epidermal differentiation disorders (EDD) in pediatric patients from a single Italian referral Center.

UNLABELLED: Inherited epidermal differentiation disorders (EDD) are a heterogeneous group of genetic skin diseases characterized by impaired epidermal differentiation and defective skin-barrier function. These conditions include congenital ichthyoses and palmoplantar keratoderma (PPK), which are particularly rare in the pediatric population and can present with evolving and, in mild forms, nonspecific or subtle clinical features, making early diagnosis challenging in children. A retrospective review was conducted on pediatric patients with a clinical and molecular diagnosis of EDD, who were ev

Awareness of hereditary breast cancer and readiness for genetic testing among newly diagnosed patients with breast cancer in Nigeria: a multicentre study.

BACKGROUND: Germline BRCA1/2 pathogenic variants account for up to 11% of breast cancer (BC) cases in Nigeria, approximately twice the prevalence reported in high-income populations. However, access to hereditary cancer services remains limited and patient awareness is largely uncharacterized. Prior studies have assessed knowledge of genetic testing among healthy populations or following structured genetic counselling, but none have evaluated baseline awareness at the point of clinical diagnosis. This multicentre study measured baseline awareness of hereditary BC and willingness to undergo BRC

Fraser Syndrome From Prenatal Diagnosis to Multidisciplinary Management and Long-Term Care: A Narrative Review.

Fraser syndrome (FS) is a rare autosomal recessive congenital malformation disorder characterised by cryptophthalmos, syndactyly, and variable respiratory, renal, genitourinary, craniofacial, and other systemic abnormalities. Considerable phenotypic heterogeneity creates challenges in prenatal recognition, prognostic counselling, neonatal management, and long-term care. Prenatal diagnosis may be particularly difficult because characteristic external abnormalities are not always detectable on ultrasonography, while oligohydramnios secondary to renal abnormalities may further limit fetal visuali

Clinical and Genetic Interplay Between Chronic Pancreatitis, Cystic Fibrosis and CFTR-Related Disorders.

Chronic pancreatitis (CP) is a clinically heterogeneous condition that may occur with a broad clinical spectrum, including cases of cystic fibrosis (CF), cystic fibrosis transmembrane regulator (CFTR)-related disorder (CFTR-RD), acute recurrent pancreatitis (ARP), and CP, including cases of idiopathic CP (iCP) and hereditary pancreatitis (HP). The specific contribution of CFTR variants and pancreatitis-predisposing genes (PPG), i.e., genes responsible for intrapancreatic activation of trypsinogen or pancreatic secretion pathways to CP susceptibility in these different contexts remains incomple

Genetic Spectrum and Family-Based Cascade Screening in Vietnamese Families With Autosomal Dominant Polycystic Kidney Disease.

AIM: Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common inherited causes of kidney failure. We performed a whole-exome sequencing study that aimed to characterize the spectrum of PKD1 and PKD2 variants in a Vietnamese ADPKD cohort and to evaluate the clinical utility of family-based cascade genetic testing. METHODS: This cross-sectional descriptive study included 40 unrelated patients with clinically diagnosed polycystic kidney disease. Variants were identified in probands using whole-exome sequencing. Family-based genetic testing was subsequently performed using S

Partial Trisomy 20qter on a Derivative Chromosome 22 in a Northern Italian Genetic Isolate with Oculo-Auriculo-Vertebral Spectrum-Related Features.

Duplications of distal 20q are uncommon and the connection to oculo-auriculo-vertebral spectrum (OAVS) is still not clear. In this paper we describe a family-based cytogenetic case of a geographically isolated community in Northern Italy. Clinical data, pedigree information, and cytogenetic report from the proband were collected. Relatives were examined for OAVS-related features. Subtelomeric FISH analysis was performed with probes for 20qter and D22S1726 using peripheral blood lymphocytes to assess the rearrangement in detail. Descriptive statistics with 95% confidence intervals were computed

Rare Double Reciprocal Translocations in a Man with Reproductive Success and Recurrent Pregnancy Loss.

Double reciprocal translocations represent an extremely rare cytogenetic entity. Carriers have a significantly increased risk of producing unbalanced gametes during meiosis, which can lead to adverse reproductive outcomes such as miscarriages or offspring with congenital anomalies. Despite the high reproductive risk, they can give birth either to a balanced translocation carrier with a normal phenotype (similar to the parents) or to a chromosomally normal completely healthy infant, resulting only from alternate 2:2 segregation. Here, we report an exceptional case of double-balanced reciprocal

Position Statement on the Regulation of Genetic Counselors in Canada on Behalf of National and Provincial Genetic Counselor Organizations.

Genetic Counselors (GCs) are an integral part of the Canadian health system across clinics, laboratories, and industry. Although most GCs meet educational and certification standards required by professional organizations and workplaces, the profession lacks consistent legal recognition in Canada. As genetic testing options expand, GC practice is extending into novel settings such as primary care and becoming more autonomous to meet growing demand and improve access to service. While GCs are regulated in countries with similar public healthcare systems such as Australia, New Zealand, and the U

DICER1 genetics and their impact on the diagnosis and management of lung lesions: A scoping review.

Congenital lung malformations (CLMs) are rare benign lung lesions for which no international management guidelines exist. Distinguishing CLM, especially congenital pulmonary airway malformation (CPAM), from pleuropulmonary blastoma(PPB) is clinically important for the development of future evidence-based guidelines on follow-up and treatment. PPB is strongly associated with pathogenic DICER1 variants. This suggests that genetic testing could aid in distinguishing benign from potentially malignant lung lesions. This scoping review summarises current evidence regarding DICER1 status in radiologi

Intrafamilial phenotypic variation in four Palestinian siblings with CFAP418-related ciliopathy: a familial case series.

BACKGROUND: Biallelic CFAP418 variants cause retinal degeneration with variable extraocular findings. The principal contribution of this familial case series is a comparison of four affected siblings carrying the same previously reported splice-donor variant, with explicit separation of original clinical records from recalled history and historical summaries. CASE PRESENTATION: Four Palestinian siblings from a consanguineous family had homozygosity for CFAP418 NM_177965.3:c.155 + 1G > A reported in a 2018 family research letter; a 2019 clinical laboratory report confirmed the variant in the o

Implementing genetic counselling supervision in southwestern Europe: the MAG-SUDOE programme.

BACKGROUND: Advances in medical genetics and the expansion of personalized medicine have made genetic counselling essential. The increasing workload of genetics professionals highlights the need for strategies that strengthen the services resilience. Genetic counselling supervision (GCS) is a structured routine of reflective practice that promotes safe practice, prevents burnout, and prepares professionals to adapt to new challenges, while supporting continuous improvement. However, GCS practices vary widely between countries: in many, these are not even known, while in others they are mandato

Is neonatal autopsy still relevant? Insights from very preterm infants.

BACKGROUND: Neonatal autopsy is the gold standard for establishing the definitive cause of death, particularly in preterm infants whose clinical complexity often limits antemortem diagnostic accuracy. Despite its value, autopsy rates have declined, raising questions about its current role. We aimed to assess autopsy rates in preterm neonates under 32 weeks of gestation and evaluate the concordance between clinical and pathological diagnoses. METHODS: We conducted a retrospective study of clinical and pathological records of deceased newborns who underwent autopsy between 2008 and 2022 in a Po

How I Identify and Diagnose Carriers of Hemophilia and Women and Girls with Hemophilia.

Carriers of hemophilia and women and girls with hemophilia remain underdiagnosed despite increasing recognition that they may experience clinically significant bleeding, reduced clotting factor levels, and important reproductive and psychosocial challenges. Advances in coagulation testing, molecular genetics and international recommendations have improved their recognition and classification, yet substantial variability in clinical presentation and limited awareness continue to delay diagnosis and access to appropriate care. Furthermore, important disparities persist across healthcare systems,
