Genetic insights: proband- only exome sequencing in pediatric neurological disorders.

Exome sequencing (ES) is emerging as a cost-effective and useful diagnostic tool for identifying pediatric neurological disorders with diverse clinical presentations. This retrospective study reviewed ES data from January 2021 to November 2023 for children with neurological symptoms that met the inclusion criteria at the genetic laboratory, Lok Nayak Hospital, New Delhi. Eighty-three children with the mentioned neurological features underwent proband-only ES. Of these, 43/83 (51.80%) were diagnosed with pathogenic (P) or likely pathogenic (LP) variants, 36/83 (43.37%) had variants of unknown s

Integrating genetic counseling in endocrine practice: Prevention, reproductive care, and prenatal diagnosis.

Genetic counseling has become a cornerstone of care in inherited endocrine and metabolic disorders, complementing clinical evaluation with molecular diagnosis and personalized risk assessment. In endocrinology, although many conditions are initially suspected based on clinical and biochemical findings, genetic testing refines diagnosis, clarifies prognosis, and guides long-term management including genetic counselling. Identification of pathogenic variants in genes such as MEN1, NR5A1, GNAS, PHEX, or CYP21A2 enables tailored surveillance, early detection of complications, and cascade screening

A dual-modal machine learning framework integrating red blood indices and smartphone-captured microscopic images for β-thalassemia screening.

BACKGROUND: Thalassemia is the most prevalent single-gene disorder in Sri Lanka, imposing a significant socioeconomic and healthcare burden. Early carrier detection is essential for genetic counselling and the prevention of thalassemia major births. Current screening programs rely on red cell indices, manual examination of blood smears by expert haematologists, and expensive confirmatory tests such as High-Performance Liquid Chromatography (HPLC) or genetic analysis, which limit large-scale applicability in low-resource settings. This study aims to develop a machine learning–based automated sc

Preimplantation genetic testing for meiotic aneuploidy in trophectoderm biopsies - is it time to change our approach?

Direct evidence from live cell imaging and single-cell sequencing of disaggregated cells that mitotic abnormalities continue to occur in the trophectoderm at the blastocyst stage, resulting in clones of aneuploid cells, has important implications for preimplantation genetic testing (PGT) for aneuploidies. Here we argue that, to improve accuracy and minimize the deselection of potentially viable embryos with only mitotic aneuploidies of unknown clinical significance, the use of methods that, up until now, have only been used for PGT for monogenic/single-gene defects could provide the answer. The myofibrillar myopathy-linked variant DES-p.T341P impairs desmin filament assembly.

BACKGROUND: Desminopathies are clinical heterogenous and range from isolated skeletal myopathies to different cardiomyopathies or combinations of both. At the molecular level, an aberrant cytoplasmic desmin aggregation is a typical hallmark of pathogenic DES variants. Currently, it is difficult to predict an aberrant desmin aggregation of novel DES variants without functional analysis. METHODS: Therefore, we investigate in this study the impact of an uncharacterized myofibrillar myopathy-associated desmin variant (p.T341P) on filament assembly in transfected cells and performed atomic force mi

Canadian certification exam pass rates are impacted by genetic counseling program location: A review of Canadian Board of Genetic Counselling data from 2007 to 2025.

Despite significant growth of the genetic counseling profession worldwide, there are differences between genetic counseling training programs (GCPs), professional standards, and professional regulation. Certification of genetic counselors (GCs) in Canada was previously managed by the Certification Board of the Canadian Association of Genetic Counsellors and is now the responsibility of the Canadian Board of Genetic Counselling - Conseil Canadien de Conseil Génétique (CBGC). Since 2007, individuals who attended a Master's level GCP, irrespective of location, have been eligible to sit for the Ca

The role of germline mutations in non-small cell lung cancer: A systematic review of emerging genetic drivers and clinical implications.

BACKGROUND: Lung cancer is the second most common malignancy and the leading cause of cancer-related mortality worldwide. While tobacco exposure remains the main risk factor, 15-20% of cases occur in never-smokers, suggesting a role for genetic predisposition. Although infrequent compared to somatic alterations, germline alterations may contribute to non-small cell lung cancer (NSCLC) susceptibility, with implications for risk assessment, targeted therapy, and family counselling. METHODS: A systematic review was conducted following PRISMA guidelines (PROSPERO ID:CRD420251081416). PubMed, SCOP

The novel (TCTG)n motif in CNBP expanded alleles: composition, dynamics and genotype-phenotype correlation in Myotonic dystrophy type 2 (DM2).

Introduction. Myotonic dystrophy type 2 (DM2) is an autosomal dominant disorder caused by (CCTG)n repeat expansions in intron 1 of the CNBP gene. Recent evidence from long-read sequencing suggests these expansions may be more complex than previously recognized. Aim. To comprehensively characterize the composition, intergenerational dynamics, and clinical impact of novel (TCTG)n motifs within the CNBP expanded alleles in a large DM2 cohort. Methods. We analyzed 100 genetically confirmed DM2 individuals (45 sporadic, 55 familial). The presence of (TCTG)n blocks was detected using an optimized qu

Implementation of an Inherited Diseases Gene Panel to Accelerate Precision Medicine in the South African Public Healthcare System.

BACKGROUND: While whole genome and exome sequencing is already being implemented in clinics across the globe, there is still very little uptake of these technologies in African healthcare facilities. Significant hurdles remain that impede the translation of these highly effective mutation screening strategies into appropriate and effective clinical services in low- and middle-income countries. METHODS: We developed, validated, and implemented a phenotype-driven genetic testing platform using a 500-gene Ion AmpliSeq Inherited Diseases Panel (IDP) within the South African State healthcare syste

Genetic Testing in Breast Cancer: Narrative Review and Clinical Insights.

BACKGROUND: Identifying hereditary breast cancer is increasingly important, as germline pathogenic variants not only influence lifetime cancer risk but also guide surveillance, preventive strategies and targeted treatments. While breast cancer genes (BRCA1 and BRCA2) remain central to hereditary breast and ovarian cancers, progress in cancer genetics has shifted clinical practice toward broader multigene testing approaches. METHODS: We reviewed the recent literature on the molecular basis of hereditary breast cancer, current genetic testing strategies, guideline-based indications, and the psy

Karyotype Analysis and Chromosomal Microarray Analysis in 568 Couples with Balanced Translocation for Prenatal Diagnosis.

BACKGROUND: Chromosomal balanced translocation is a prevalent structural chromosomal abnormality. Carriers typically exhibit no phenotypic differences, as the genetic material remains unchanged. However, during germ cell meiosis, unbalanced gametes may be produced, leading to genetic effects in offspring and potentially resulting in adverse outcomes. This study aims to explore the prenatal diagnostic value of combining chromosome karyotype analysis with chromosomal microarray analysis (CMA) for carriers of balanced chromosomal translocations, and to provide a reference for clinical genetic cou

Importance of genetic testing in childhood cancer survivors for hereditary cancer predisposition syndromes.

BACKGROUND: Around 10% of children with cancer carry a germline pathogenic or likely pathogenic variant (P/LPV) in one of the cancer predisposition genes. The objective of this study was to evaluate the frequency of genetic testing for hereditary cancer predisposition syndromes (HCPSs) among adults treated for childhood cancer in Slovenia and to characterize the P/LPVs identified in those who opted for genetic testing. We also aimed to analyse how a positive genetic test influences cascade testing in blood relatives. METHODS: We included patients born between 1970 and 2000 who were diagnosed a

Navigating uncertainty during non-invasive prenatal testing: Genetic counselors' experiences of supporting autonomous reproductive decision-making.

There is limited understanding of how healthcare professionals understand and apply the concept of reproductive autonomy in their clinical practice with non-invasive prenatal testing (NIPT). Existing research indicates potential misalignment between theoretical understandings of reproductive autonomy and its operationalization by healthcare professionals in clinical practice. Despite genetic counselors (GCs) having unique expertise and experience in prenatal genetic counseling, there is a dearth of research regarding their understanding and practices relating to reproductive autonomy. BRCA Testing in Asian Patients with Ovarian Cancer: Standard Clinical Practice or Mutation Prediction Model?

BACKGROUND: Germline BRCA1/2 testing is recommended for all patients with ovarian cancer, as identifying pathogenic variants (PV) informs treatment and enables family cascade testing. However, in resource-limited settings, high testing costs often limit feasibility. An alternative approach is to use predictive models to prioritize patients at risk, optimizing resource allocation. Existing models are largely Western-derived; in Asian populations, models exist for breast cancer-not ovarian cancer. METHODS: Using data from a multicenter study of 1,126 Asian patients with ovarian cancer (includin
