The Utilisation of Genetic Counselling Services Amongst Prenatal Healthcare Providers in Gauteng, South Africa.

INTRODUCTION: Congenital anomalies and genetic disorders contribute substantially to perinatal morbidity and mortality, particularly in low- and middle-income countries. Prenatal healthcare providers play a key role in identifying affected pregnancies and referring to patients for genetic counselling; however, referral practices remain suboptimal. OBJECTIVE: To assess the utilisation of genetic counselling services and perceptions of genetic counselling among prenatal healthcare providers in Gauteng Province, South Africa. METHOD: An electronic survey was distributed to prenatal healthcare p

KCNQ2 neonatal epilepsy: Impact of prompt diagnosis and treatment, and early predictors of outcome severity.

OBJECTIVE: To determine whether prompt genetic diagnosis in children with KCNQ2 neonatal epilepsy enabling targeted therapy is associated with improved outcomes, and identify early predictors of developmental outcomes. METHODS: Thirty-seven children with KCNQ2 neonatal epilepsy were recruited from five pediatric centers. We reviewed demographic, clinical, EEG, and genetic data. We determined differences in outcomes between individuals with prompt (greater than 30 days from seizure onset) and later genetic diagnosis, and we identified neonatal factors associated with developmental outcome. Harmonizing the genetic counselor profession in Europe.

Genetics in medicine is rapidly becoming integral to European healthcare, yet access to high-quality genetic counseling remains inconsistent. Genetic counseling empowers patients to make informed decisions about genetic testing, improves clinical management, and mitigates psychosocial harm. Despite growing demand, the genetic counselor profession lacks legal recognition, standardized education, and harmonized regulation across European Union (EU) Member States. Current fragmentation, which is evident in separate national laws and variable practices, poses systemic risks, including inequitable

[Hereditary colorectal cancer].

Colorectal cancer is one of the most common types of cancer with hereditary background. Due to the progress in DNA sequencing techniques, the number of identified cancers predisposing gene mutations is rising constantly. For some morphologically defined cancer predispositions like serrated polyposis, however, a genetic background is rarely identified. Even if this would exclude a hereditary predisposition, the elevated cancer risk remains, emphasizing the importance of histopathologic diagnoses.Hereditary colorectal cancer can be categorized into polypous and non-polypous predispositions. To refer or not to refer? Exploring the cognitive process of genetic counselors' decision to refer a patient to another professional.

Genetic Counselors (GCs) hold the dual responsibility to engage in patient-centered psychosocial counseling and to recognize when a patient's needs require referral to another healthcare professional (HCP). While previous work has shown that GCs use a variety of patient factors to determine when they could benefit from additional supportive services, we sought to better understand how GCs determine whether to refer a patient to another HCP or engage in more complex psychosocial counseling. Utilizing a multimethods approach, we used two sets of surveys to learn about our participants but ultima

Exploring experts' views on "Not Parent Expected" discoveries via direct-to-consumer DNA ancestry testing.

BACKGROUND: Millions of people have taken Direct-to-Consumer (DTC) DNA ancestry tests to learn more about their ethnic ancestry and family history. A small but growing body of research has noted that these DTC DNA ancestry tests raise challenging ethical, psychosocial, and resource-related issues that have not been adequately considered nor discussed. One key issue concerns a "Not Parent Expected" (NPE) discovery, in which a user suddenly and unexpectedly learns from these tests that an assumed parent (typically the father) is not a biological parent. Such news can negatively affect mental hea

Appearance-related stigma and its implications for genetic counselling practice.

In an appearance conscious society, individuals with genetic conditions that affect appearance often face unique psychosocial challenges that can shape reproductive decision-making. While research has highlighted the role of stigma in shaping decision-making, little is known about how genetic counsellors perceive and address appearance-related concerns in clinical practice. This study explored the experiences of UK-based genetic counsellors working with individuals affected by congenital conditions that alter appearance, with a focus on reproductive decision-making. Using a qualitative design,

Non-invasive prenatal screening in practice: an audit of clinical management in high-probability cases.

Non-invasive prenatal screening (NIPS) is widely recommended for fetal chromosomal aneuploidies. This study describes patterns of NIPS requests, including follow-up and post-test genetic counselling, and discusses how these compare to clinical guidelines in Australia and New Zealand. We performed a retrospective analysis of increased-probability NIPS cases reported by a private pathology provider from April 2022 to May 2024. Descriptive statistics were used to summarise referral patterns, the type of anomaly detected, uptake of complimentary genetic counselling, and diagnostic follow-up. Of th

Personal values, professional responsibilities and non-invasive prenatal testing: reflections of genetic counsellors in Australia.

The reproductive autonomy rationale endorses the provision of non-invasive prenatal testing (NIPT) to enable individuals to make decisions about their pregnancy in accordance with their personal values. While the personal values of healthcare professionals may promote autonomous reproductive decision-making in light of increasingly complex genetic information, they may also impinge upon reproductive autonomy through limiting patient access to care. This paper provides timely insight into genetic counsellors' perceptions and implementation of professional responsibilities as well as their views

Development of a predictive matrix for pre-test genetic counselling in inherited retinal diseases based on data from 1001 patients enrolled in the IRD-PT registry.

Genetic testing plays a pivotal role in the management of Inherited Retinal Diseases (IRDs), yet a significant proportion of patients remain without a molecular diagnosis. This study aimed to develop a predictive matrix to estimate the likelihood of diagnostic success in order to manage patient expectations prior to genetic testing. Conducted as a retrospective, single-center cohort study, it analyzed data from 1001 IRD patients across 804 unrelated families enrolled in the IRD-PT registry. Key clinical variables-including age at symptom onset, family history, consanguinity, and referral timin

Uniparental Disomy: A Scoping Review of Diagnostic Challenges, Clinical Spectrum, and Counselling Implications.

INTRODUCTION: Uniparental disomy (UPD) is a rare chromosomal anomaly causing significant clinical implications in many affected individuals. In the case of UPD, a pair of homologous chromosomes or segments of it is inherited from only one parent. This can disrupt genomic imprinting, create long stretches of homozygosity, leading to diverse genetic and epigenetic syndromes. This scoping review has aimed to summarize postnatally reported UPD cases, tabulate chromosomespecific clinical characteristics, molecular diagnostic methodologies being utilized, and to formulate a standardised diagnostic w

Inherited retinal disease genes with dual inheritance patterns: insights from the IRD-PT registry.

BACKGROUND: Inherited retinal diseases (IRDs) typically follow a single inheritance pattern, but some genes cause disease through both autosomal recessive (AR) and autosomal dominant (AD) patterns, challenging genetic counselling. This study aims to identify dual inheritance genes in a Portuguese cohort and characterise the prevalence of each inheritance mode and associated phenotypes. METHODS: Cross-sectional study at Portugal's largest IRD referral centre. Genes reported with dual inheritance were identified through literature search and screened in IRD-PT registry. For each gene, AR versus

Monoallelic germline RAD51C, RAD51D, and BRIP1 variants in hereditary cancer testing: Variant spectrum and clinical counselling implications.

Defects in homologous recombination repair genes contribute to hereditary cancer susceptibility beyond BRCA1 and BRCA2, yet clinical interpretation of intermediate-penetrance genes remains challenging. We retrospectively evaluated unrelated carriers of pathogenic or likely pathogenic germline variants in RAD51C, RAD51D, and BRIP1 identified through hereditary cancer multigene panel testing in a real-world cohort of approximately 8000 individuals. Fifty-two carriers were included (RAD51C n = 14, RAD51D n = 15, BRIP1 n = 23). Ovarian cancer represented the predominant malignancy, followed by bre

How parents decide whether to have genomic newborn screening: experiences from BabyScreen.

Education and consent processes for genomic newborn screening (gNBS) must be informed by evidence about parental decision-making. BabyScreen+ piloted gNBS for 1000 newborns in Victoria, Australia. Prospective parents consented online with genetic counselling available on request. We aimed to explore how BabyScreen+ participants (n = 1139) made decisions about gNBS using a convergent parallel mixed methods design. Demographic (n = 1080), survey (n = 1010), and interview (n = 24) data were analysed using descriptive statistics, logistic regression, and deductive content analysis mapped to the He
