Maternal Malignancies Identified Through Non-invasive Prenatal Testing: A Systematic Review and Meta-Analysis.

OBJECTIVE: Non-invasive prenatal testing (NIPT) may incidentally reveal chromosomal abnormalities of maternal origin. Although specific chromosomal aberrations have been associated with maternal malignancy, reported risks vary widely. This meta-analysis aimed to quantify the magnitude and consistency of these associations. METHODS: A systematic review and meta-analysis of observational studies reporting maternal malignancy among pregnant individuals with abnormal NIPT results was conducted. Odds ratios (ORs) with 95% confidence intervals (CIs) were pooled using a random-effects model. Risk of

Heat Stroke and Pregnancy.

A 33-year-old woman participating in a physically high-intensity event experienced a heat injury and was incidentally found to be pregnant. Other contributing factors to the heat injury were the event taking place during the hottest month of the year, consumption of creatine and preworkout supplements, and abstaining from breakfast on the day of the event. She suffered heat stroke with AST and ALTs reaching 8k and CK peak of 36k. The concern for acute liver failure prompted discussions for a possible liver transplant, ultimately not required. Viability of the pregnancy was indeterminable durin

Delayed Recognition of Maternal G6PD Heterozygous Status Across Prenatal and Newborn Care Interfaces.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is the most common red blood cell enzymatic disorder worldwide. Although many heterozygotes are asymptomatic, affected neonates have an increased risk for hyperbilirubinemia and related complications. In the United States, detection of maternal G6PD heterozygous status often occurs incidentally during expanded carrier screening. We sought to characterize the diagnostic timing, clinical management, and neonatal outcomes among pregnancies complicated by maternal G6PD heterozygous status. We conducted a retrospective cohort study of pregnant ind

Vertical transmission of Nipah virus in an experimentally infected nonhuman primate; a brief report and description of histopathological lesions.

Nipah virus (NiV) is a highly pathogenic henipavirus which causes severe, often fatal disease in humans and animals. African green monkeys (AGM) have been used extensively to investigate NiV disease and are the only nonhuman primate (NHP) model that accurately recapitulates human NiV pathogenesis. Here, we describe histopathological lesions in the reproductive tissues of an incidentally pregnant female AGM that succumbed to NiV disease. We employed multiplex immunohistochemistry techniques to profile specific cell populations which identified NiV antigen in both maternal and fetal cells of the

Advancing precision care in pregnancy through a treatable fetal findings list.

The use of genomic sequencing (GS) for prenatal diagnosis of fetuses with sonographic abnormalities has grown tremendously over the past decade. Fetal GS also offers an opportunity to identify incidental genomic variants that are unrelated to the fetal phenotype but may be relevant to fetal and newborn health. There are currently no guidelines for reporting incidental findings from fetal GS. In the United States, GS for adults and children is recommended to include a list of "secondary findings" genes (ACMG SF v.3.2) that are associated with disorders for which surveillance or treatment can re

Severe Hemodynamic Instability in a Young Pregnant Woman with Massive Pericardial Effusion and Pulmonary Embolism Secondary to Primary Mediastinal Non-Hodgkin's Lymphoma.

Background: Lymphomas account for approximately 10% of cancers diagnosed during pregnancy, with Hodgkin's lymphoma being the most common. However, non-Hodgkin lymphomas, including primary mediastinal large B-cell lymphoma (PMBCL), also represent a significant proportion. Both mediastinal lymphomas and pregnancy develop a hypercoagulable state, increasing the risk of venous thromboembolism and massive pulmonary embolism (PE), requiring extracorporeal membrane oxygenation (ECMO). Methods: Clinical data, blood test and imagings have been collected by the medical records of the patient. Results: W

Prenatal Cell-Free DNA Sequencing and Intrahepatic Cholangiocarcinoma.

Prenatal cell-free DNA (cfDNA) sequencing is a screening method that utilizes circulating cfDNA in maternal blood to identify fetal chromosomal abnormalities. Beyond its primary application in prenatal care, cfDNA sequencing can detect occult maternal malignancies before they manifest clinically, presenting a critical window for early therapeutic intervention. Here we report a single case in detail from the Incidental DEtection of maternal Neoplasia Through non-Invasive cell-Free DNA analysis (IDENTIFY) study, in which an abnormal cfDNA sequencing result from an asymptomatic pregnant woman ide

Case study: autoimmune hepatitis with cirrhosis and pancytopenia since 7 weeks' gestation.

OBJECTIVES: Autoimmune hepatitis (AIH) is a chronic inflammatory disease of unknown etiology and AIH in pregnancy is associated with many adverse maternal and fetal outcomes. The purpose of this report is to share insight into management of AIH-induced pancytopenia unresponsive to steroids and transfusions. CASE PRESENTATION: A 29-year-old G4P0121 female with history of spontaneous bacterial peritonitis (SBP) and severe pancytopenia secondary to AIH was found to be incidentally pregnant at 7 weeks gestation. Despite multiple blood transfusions and steroids, her pancytopenia was unresponsive t

Prenatal cfDNA Sequencing and Incidental Detection of Maternal Cancer.

BACKGROUND: Cell-free DNA (cfDNA) sequence analysis to screen for fetal aneuploidy can incidentally detect maternal cancer. Additional data are needed to identify DNA-sequencing patterns and other biomarkers that can identify pregnant persons who are most likely to have cancer and to determine the best approach for follow-up. METHODS: In this ongoing study we performed cancer screening in pregnant or postpartum persons who did not perceive signs or symptoms of cancer but received unusual clinical cfDNA-sequencing results or results that were nonreportable (i.e., the fetal aneuploidy status co

Body-piercing associated appendicitis in the pregnant patient incidentally discovered during Caesarian section.

Appendicitis is the most common general surgery condition encountered in pregnant patients. Its presentation and diagnosis can be confounded by physiological changes of pregnancy causing atypical symptoms and overlap between pregnancy symptoms and appendicitis. Diagnosis of appendicitis in pregnancy is important, as pregnant persons have a much higher rate of complication from appendicitis. In this case report, we describe a patient with a history of right lower quadrant pain thought to be related to round ligament pain. Her previously undiagnosed appendicitis was discovered during routine ins

Patient attitudes and preferences about expanded noninvasive prenatal testing.

Introduction: Noninvasive prenatal testing (NIPT) using cell-free DNA (cfDNA) is typically carried out to screen for common fetal chromosomal anomalies, with the option to screen for a wider range of chromosomal changes (expanded NIPT) becoming increasingly available. However, little is known about pregnant patients' attitudes and preferences regarding expanded NIPT. Methods: To address this gap, we surveyed general-risk patients having first-tier cfDNA screening at a private prenatal clinic on their expectations for expanded NIPT. Patients were asked questions regarding their current pregnanc

Patients' perspectives on prenatal screening results that suggest maternal cancer: A qualitative analysis.

OBJECTIVE: To explore patient perspectives after receiving non-invasive prenatal testing (NIPT) results that suggest maternal cancer. METHODS: Individuals who received non-reportable or discordant NIPT results during pregnancy and enrolled in a study were interviewed prior to and after receiving the outcome of their clinical evaluation for cancer. Interviews were independently coded by two researchers and analyzed thematically. RESULTS: Forty-nine participants were included. Three themes were identified: 1) limited pre-test awareness of maternal incidental findings caused considerable confus

Unmasking the Hidden Fetus: Introducing the Circled Delta Sign and Reversed Circled Delta Sign for Accurate Identification on Abdominal Radiography.

BACKGROUND Ionizing radiation poses potential health risk to fetuses, including growth retardation, organ malformations, neurological effects, fetal death, and cancer. Therefore, pregnant women rarely undergo X-ray imaging, unless absolutely necessary. However, they can be inadvertently exposed to X-rays while undergoing an examination without being aware of their pregnancy. Given that the likelihood of a fetus appearing on an abdominal radiograph is low, physicians can be unfamiliar with the appearance of a fetus on such images. If a fetus incidentally appears on an abdominal radiograph, the

Obstetric violence as immigration injustice: A view from the United States and Colombia.

In September 2020, Project South, along with numerous other organizations, released a report detailing abuses in a Georgia Detention Center - including forced hysterectomies. Whatever other factors are at play, one of them is an intrinsic connection between obstetric violence against pregnant migrants and immigration injustice. It is not incidental that these acts - in US detention centers, along the US-Mexico border, in Colombian hospitals and clinics - are being perpetrated on immigrant bodies. And it is not accidental or random which immigrant bodies are vulnerable to these violations. Unde

Vertical space use and thermal range of the great hammerhead (Sphyrna mokarran), (Rüppell, 1837) in the western North Atlantic.

The great hammerhead (Sphyrna mokarran) is a highly mobile, large-bodied shark primarily found in coastal-pelagic and semi-oceanic waters across a circumtropical range. It is a target or by-catch species in multiple fisheries, and as a result, rapid population declines have occurred in many regions. These declines have contributed to the species being assessed as globally critically endangered on the IUCN Red List. Although conservation and management measures have yielded promising results in some regions, such as the United States, high levels of at-vessel and post-release mortality remain a

Pregnancy-associated thrombotic thrombocytopenic purpura complicated by Sjögren's syndrome and non-neutralising antibodies to ADAMTS13: a case report.

BACKGROUND: Thrombotic thrombocytopenic purpura (TTP) is a severe and life-threatening disease. Given its heterogeneous clinical presentation, the phenotype of TTP during pregnancy and its management have not been well documented. CASE PRESENTATION: We report here a 25-year-old woman, G1P0 at 36 weeks gestation, who developed severe thrombocytopenia and anemia. She was performed an emergent caesarean section 1 day after admission because of multiple organ failure. As ADAMTS 13 enzyme activity of the patient was 0% and antibodies were identified by enzyme-linked immunosorbent assay, she was di

Treatment and prevention of viral hepatitis in pregnancy.

Viral hepatitis in pregnancy may be caused by many types of viruses that cause systemic infection or target hepatocytes in their pathogenesis. Because viral hepatitis during pregnancy may represent acute or chronic infection or the reactivation of a prior infection, a high clinical suspicion, medical history review, and awareness of risk factors for the acquisition of infection are important management principles. The route of infection varies widely and ranges from fecal-oral transmission for the hepatitis A and E viruses to vertical transmission for hepatitis B, blood-borne transmission for

Redox regulation of regulatory T-cell differentiation and functions.

The choice between immunity or tolerance is a consequence of T-cell fate determined by T-cell receptor affinity to cognate MHC-peptide complex, costimulatory molecules and cytokines from antigen presenting cells. While activated, effector and memory T-cells provide immunity against antigens, regulatory T-cells play a pivotal non-redundant role in immune tolerance and tissue repair. T-cell differentiation and functions are also well known to be governed by the redox status. Physiological redox status is determined by oxygen concentration, reactive oxygen species levels and antioxidant concentra

Extreme elevations of alkaline phosphatase in pregnancy: A case report.

BACKGROUND: The normal serum concentration of alkaline phosphatase (ALP) in adults over the age of 18 ranges from 37 to 116 U/L, while in pregnant women levels of up to twice that upper limit can still be normal. There have been very few reports of extreme elevations in ALP, and here we present the case of a 29-year-old pregnant woman with an incidentally found 30-fold increase. CASE: The patient, G6P2-1-2-4, received routine prenatal care, though presented to obstetric triage at 36 weeks and 1 day of gestation for diagnosis and management of viral rhinosinusitis and was found to have an ALP
