Developing and validating a machine learning approach for prediction of euploid pregnancy loss in the Copenhagen pregnancy loss study.

OBJECTIVE: To study whether a machine learning algorithm can effectively predict fetal genetic status, aneuploid or euploid, in cases of pregnancy loss, based solely on readily available clinical data. Accurate early prediction may enable improved clinical decision-making and personalized patient management. DESIGN: Prospective multicenter cohort study within the Copenhagen Pregnancy Loss study, with a development cohort (n = 788) from Copenhagen University Hospital Hvidovre and external validation cohorts from Copenhagen University Hospitals Herlev (n = 229) and North Zealand (n = 199). Construction and validation of a risk prediction model for recurrent pregnancy losses in patients with positive antiphospholipid antibodies.

To identify independent risk factors for recurrent pregnancy loss (RPL) among antiphospholipid antibody (aPL)-positive individuals who were either in early pregnancy (≤12 weeks' gestation) or not pregnant, and to develop and validate a risk prediction model to support early clinical risk stratification and intervention. We retrospectively analyzed 197 aPL-positive patients with a history of adverse pregnancy outcomes. Of the 197 aPL-positive patients, 31 met the diagnostic criteria for APS, and 166 had isolated aPL positivity. The patients were divided into an RPL group (n = 76) or a non-RPL g

Adverse events following self-administered abortion pills: A retrospective observational study.

Background and objectives In India, limited awareness regarding the potential adverse effects of medical termination of pregnancy (MTP) pills, coupled with their non-prescribed over-the-counter availability, has led to their widespread unsupervised use. Such practices may result in serious maternal complications. This study aimed to describe the clinical profile and outcomes among women presenting to a tertiary care hospital following unsupervised self-administration of MTP pills. Methods A record-based retrospective study was carried out at a tertiary academic medical centre. Case files of wo

The impact of anti-SSA/Ro antibodies on pregnancy outcomes in patients with systemic lupus erythematosus.

OBJECTIVE: We aimed to investigate the influence of anti-SSA/Ro antibodies and other factors on adverse pregnancy outcomes (APOs) in patients with SLE. METHODS: We retrospectively extracted data on first pregnancies after SLE onset from the Lupus Registry of Nationwide Institutions (LUNA, 2016-2022, n = 1775). We excluded patients with unclear anti-SSA/Ro status or pregnancy outcomes and cases of abortion. We used two-group comparison and logistic regression, adjusting for aPL, to analyse factors related to APOs and the influence of anti-SSA/Ro antibodies. RESULTS: The 247 pregnancies includ

Long-term safety of human papillomavirus vaccination in relation to early pregnancy outcomes: registry-based follow-up of a community-randomized trial.

Despite the demonstrated safety of human papillomavirus vaccines, concerns have arisen regarding potential associations of human papillomavirus vaccination on reproductive health. The objective of this registry-based study was to compare overtime association of vaccination and early pregnancy outcomes. Twenty thousand four hundred twenty-six women born in 1992-1995, who participated in a Finnish community-randomized human papillomavirus vaccination trial, and age- and community-aligned unvaccinated controls born in 1990-1991 (n = 19 473) were followed up for 11 years. Data was obtained from th

Structural fragility in digital public health communication.

Inherited genetic disorders are progressive in nature and lead to organ dysfunction or death in severe cases. At present, there are no permanent treatment options for >95% of inherited disorders. Different modes of inheritance, type of gene(s) involved, and population-based variations add further complexity to finding suitable cures for approximately 400 million patients worldwide. Gene therapy is a very promising molecular technique for the treatment of rare genetic disorders. Gene therapy functions on the basis of restoration, replacement, inhibition, and, most recently, editing of gene(s) t

Patients' experiences with pain and bleeding in first-trimester abortion care.

BACKGROUND: Abortion care is safe and commonly needed. While pain and bleeding are expected, patients' experience of pain and bleeding at the time of abortion can affect patient satisfaction and the abortion experience. There is limited research characterizing factors associated with patient-reported severe pain and heavy bleeding with contemporary abortion methods. OBJECTIVE: To assess clinical predictors of patient-reported heavy bleeding and pain among first-trimester abortion patients. STUDY DESIGN: We conducted a secondary analysis of a multicenter prospective cohort study of 644 partic

Pregnancy outcomes in a recurrent miscarriage population after common uterine surgeries: a multicenter cohort study.

OBJECTIVE: To establish the effect of uterine surgery on miscarriage rates. Some surgical procedures risk compromising endometrial function and lead to reproductive dysfunction. There is no long-term data to demonstrate the effect of uterine surgery on miscarriage rates. Surgical management of miscarriage and cesarean section are commonly chosen management options. The effect of these on future fertility prospects should be established. DESIGN: A prospective multicenter cohort study. SUBJECTS: A total of 1,473 women with recurrent miscarriage. EXPOSURE: We evaluated the association between

COVID-19 infection history as a risk factor for early pregnancy loss: results from the electronic health record-based Southeast Texas COVID and Pregnancy Cohort Study.

BACKGROUND: The effects of SARS-CoV-2 infection before or during pregnancy on pregnancy outcomes are still largely unknown. We hypothesized that COVID-19 in early pregnancy is a risk factor for adverse pregnancy outcomes, particularly miscarriage. METHODS: We examined the relationship between COVID-19 and adverse pregnancy outcomes, including spontaneous abortion, ectopic pregnancy, and preterm delivery in a large, retrospective, electronic health record (EHR)-based cohort, from 2019 to 2023. Generalized estimating equation modeling was performed to identify risk factors for adverse pregnancy

The association of infertility, miscarriage, stillbirth, and parity with osteoporosis: a pooled analysis of five cohort studies.

OBJECTIVE: To test, among women with natural menopause, whether: infertility, miscarriage, stillbirth, and low parity are associated with a higher risk of osteoporosis; menopause age can partly explain their associations. DESIGN: Pooled analysis of five retrospective and prospective cohorts within the International Collaboration for a Life Course Approach to Reproductive Health and Chronic Disease Events. SUBJECTS: A total of 141,222 naturally postmenopausal women with data on fertility factors (i.e., infertility, miscarriage, stillbirth, or parity), osteoporosis, and covariates (i.e., race,

Maternal and Infant Outcomes in a Subset of Patients with Sickle Cell Disease in South Carolina.

OBJECTIVES: Sickle cell disease (SCD), which disproportionately affects minorities, increases complications during pregnancy. Severe maternal mortality is increased in women with SCD, including morbidity related to the disease and other nondisease-related complications. It also can have devastating complications for fetuses, with increases in premature birth and low birth weight. This study aimed to describe the characteristics of women with SCD in South Carolina, with a specific focus on fetal and maternal outcomes and complications. The secondary aim of this study was to identify the effect

Association between hyperemesis gravidarum and depression: a national register-based controlled study in Finland.

BACKGROUND: Hyperemesis gravidarum is a severe form of nausea and vomiting that occurs during pregnancy. The connection between psychiatric morbidity, especially depression, and hyperemesis gravidarum has been debated from contradictory findings. Therefore, we aimed to evaluate the associations between hyperemesis gravidarum and both pre-pregnancy and new-onset post-pregnancy depression. METHODS: We conducted a nationwide register-based controlled study in Finland between Jan 1, 2004, and Dec 31, 2017. Data were collected from 2004 to assess at least 1 year of pre-pregnancy depression as deli

Predictors of spontaneous pregnancy loss in single ventricle physiology.

BACKGROUND: Pregnant patients with single ventricle (SV) physiology carry a high risk of spontaneous pregnancy loss (SPL), yet the clinical factors contributing to this risk are not well defined. METHODS: Single-centre retrospective study of pregnant patients with SV physiology seen in cardio-obstetrics clinic over the past 20 years with chart review of their obstetric history. Patients without a known pregnancy outcome were excluded. Univariable Bayesian panel-data random effects logit was used to model the risk of SPL. RESULTS: The study included 20 patients with 44 pregnancies, 20 live bi

Genetic Factors Associated With Adverse Pregnancy Outcomes in Chronic Pancreatitis.

INTRODUCTION: The effects of genetic factors on pregnancy outcomes in chronic pancreatitis (CP) patients remain unclear. We evaluated the impacts of clinical features and mutations in main CP-susceptibility genes ( SPINK1 , PRSS1 , CTRC , and CFTR ) on pregnancy outcomes in Chinese CP patients. METHODS: This was a prospective cohort study with 14-year follow-up. The sample comprised female CP patients with documented pregnancy and known genetic backgrounds. Adverse pregnancy outcomes were compared between patients with and without gene mutations. Univariate and multivariate analyses were perf

Trophectoderm biopsy is associated with lower risks of moderate to extreme prematurity and low birthweights: a national registry cohort study of singleton livebirths from frozen-thawed blastocyst transfers.

BACKGROUND: Trophectoderm biopsy has become the mainstay assisted reproductive technique performed for preimplantation genetic testing, accounting for 43.8% of embryo transfer cycles in the United States in 2019 alone. Despite its prevalence, data on the obstetric and perinatal outcomes post-trophectoderm biopsy remains sparse and mixed. OBJECTIVE: This study aimed to examine the risks of adverse perinatal outcomes in birthweights and prematurity after transfers of the vitrified-thawed blastocyst with trophectoderm biopsy for preimplantation genetic testing. STUDY DESIGN: This was a retrospe

Hydroxychloroquine in recurrent pregnancy loss: data from a French prospective multicenter registry.

STUDY QUESTION: What are the outcomes of pregnancies exposed to hydroxychloroquine (HCQ) in women with a history of recurrent pregnancy loss (RPL), and what factors predict the course of these pregnancies beyond the first trimester? SUMMARY ANSWER: In our cohort of pregnancies in women with a history of RPL exposed to HCQ early in pregnancy, we found that the only factor determining the success of these pregnancies was the number of previous miscarriages. WHAT IS KNOWN ALREADY: Dysregulation of the maternal immune system plays a role in RPL. HCQ, with its dual immunomodulating and vascular p
