[Assessment of venous thromboembolism risk in obstetrics].

OBJECTIVES: Women are at increased risk of venous thromboembolism (VTE) during pregnancy. Because of the many risk factors (RFs) involved and the need for frequent risk reassessment, VTE risk prediction scores have been developed to increase the adequacy of thromboprophylaxis. This article reviews the various existing VTE risk prediction scores. METHODS: A review on PUBMED using the following medical subject headings - ([pregnancy OR postpartum] AND [score OR risk OR model prediction] AND [venous thromboembolism OR pulmonary embolism OR deep vein thrombosis]) - identified the articles publish

Mortality-weighted severe maternal morbidity: a novel approach to assessing maternal health outcomes.

OBJECTIVE: The aim of this study is to develop a robust mortality-weighted severe maternal morbidity (SMM) composite measure that provides increased clinical granularity by measuring the individual and combined risk of inpatient mortality across multiple SMM indicators. The enhanced metric better reflects the complexity gradient of individual patients through a weighted SMM (w-SMM) composite score expressed on a continuous scale. METHODS: This retrospective cohort study analyzed 7,174,412 inpatient deliveries from 2016 to 2023 extracted from the Premier Healthcare Database, an all-payer, nati

Pre-injury statin therapy is associated with lower VTE after traumatic brain injury.

BACKGROUND: Hemorrhage progression is a potentially devastating complication after TBI, mandating delay to VTE prophylaxis in high-risk patients. Statins have endothelial stabilizing effects associated with decreased VTE in other populations. OBJECTIVE: To explore whether prehospital statin exposure is associated with decreased incidence of VTE in TBI. METHODS: This retrospective cohort study included adult trauma patients and excluded bleeding disorder, disseminated cancer, pregnancy, and deaths <24 h. TBI progression, VTE incidence, VTE prophylaxis type/timing were assessed, grouped by pr

Risk Factors and Predictors of Mechanical Ventilation in Neonates With Meconium Aspiration Syndrome: A Retrospective Study at King Salman Armed Forces Hospital, Tabuk, Saudi Arabia.

Background Meconium aspiration syndrome (MAS) is a critical neonatal condition predominantly associated with term and post-term pregnancies, characterized by the aspiration of meconium-stained amniotic fluid (MSAF) leading to severe respiratory distress. Despite advances in obstetric and neonatal care, MAS remains a significant cause of neonatal morbidity, particularly in settings with limited access to advanced neonatal care. Objectives This study aimed to evaluate the prevalence of mechanical ventilation and explore potential predictors for respiratory support in neonates diagnosed with MAS

How to Manage Venous Thromboembolism Risk during Pregnancy in Patients with Inherited Antithrombin Deficiency?

Inherited antithrombin deficiency (ATD) is associated with a high risk of venous thromboembolic complications. Association of ATD with other conditions such as pregnancy obviously increases thromboembolic risk and may require anticoagulant therapy for prevention. Although there are several/heterogenous international guidelines regarding thromboprophylaxis in pregnant patients with ATD, data on anticoagulant prophylaxis in this context are scarce in the literature. Thus, this situation remains a challenge both in the antepartum period and during delivery. Physicians from the French Society of T

Preconception metformin and hyperemesis gravidarum in Denmark: clinical implications of growth differentiation factor 15's etiological role.

BACKGROUND: Hyperemesis gravidarum, a condition characterized by severe nausea and vomiting in pregnancy, is now understood to be partly attributable to high placentally derived growth differentiation factor 15 concentrations, which increase rapidly from early pregnancy. Intriguingly, accumulating evidence has suggested that low preconception growth differentiation factor 15 concentrations predispose to hyperemesis gravidarum, suggesting that growth differentiation factor 15 preconditioning may offer a modifiable factor, which could be harnessed as the first preventive strategy for hyperemesis

Intraoperative Hypotension After Neuraxial Anesthesia: A Systematic Review and Meta-Analysis of Randomized Controlled Trials of Vasopressors for Cesarean Birth Stratified by Maternal Risk.

Postspinal hypotension is a frequent complication during cesarean delivery, particularly in high-risk pregnancies. Although vasopressors are routinely used for its management, their relative efficacy and safety after the onset of hypotension remain uncertain. Following PROSPERO registration (CRD420251050321) and in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines, this systematic review and meta-analysis evaluated the efficacy and safety of norepinephrine, phenylephrine, and ephedrine administered for treatment, rather than prophyl

Use of a Temporary Pacemaker During Cesarean Section in a Pregnant Patient With Neurally Mediated Syncope: A Case Report.

A 39-year-old pregnant woman with a history of neurally mediated syncope (NMS) and a documented eight-second sinus arrest on her Holter electrocardiography (ECG) was scheduled for cesarean delivery. A temporary pacemaker (PM) was inserted preoperatively to prevent severe bradycardia. Combined epidural and spinal anesthesia (CESA) was carried out. Intraoperatively, severe bradycardia triggered PM activation. Prompt administration of ephedrine and atropine sulfate stabilized her vital signs, and the PM was deactivated within two minutes. The cesarean section proceeded without further complicatio

Effective Sirolimus Use in Prenatal and Postnatal Management of Symptomatic Extensive Congenital Capillary-Lymphatic-Venous Malformations (CLVMs): A Report of Two Cases.

INTRODUCTION: Congenital capillary-lymphatic-venous malformations (CLVMs) often result in life-threatening complications, which may begin in utero. Sirolimus, a mammalian target of rapamycin (mTOR) inhibitor, has been successfully used in children and adults with CLVMs due to its antiproliferative and antiangiogenic properties. However, only two cases of prenatal sirolimus treatment of fetal CLVMs have been published to date, with very limited data on optimal therapeutic scheme and drug dosing. CASE REPORTS: Here we report two cases of effective prenatal and postnatal sirolimus treatment of e

Evaluation of a new prediction model for the estimation of risk of obstetrical anal sphincter injuries.

BACKGROUND: Obstetrical anal sphincter injuries are complications of vaginal birth that have the potential to cause substantial maternal morbidity. Predicting these injuries might help to improve maternal care as well as antenatal counseling and patient education. Previous attempts to create prediction models have in many cases involved variables only known postpartum, which limits their use in an antenatal setting. Other models include parameters that are not applicable to a Northern European population. OBJECTIVE: This study aimed to develop and validate a clinically useful model for the pr

Caesarean Scar Pregnancy Presenting With Septic Shock and Multiorgan Dysfunction in a High-Risk Cardiac Patient: A Case Report.

Caesarean scar pregnancy (CSP) is a rare form of ectopic pregnancy characterised by the implantation of the gestational sac at the site of a previous caesarean section scar. With the global increase in caesarean delivery rates, the incidence of CSP has been rising correspondingly. We present a case of a 28-year-old woman with an undiagnosed CSP and approximately eight weeks of amenorrhea, who presented in septic shock, further complicated by preexisting cardiac comorbidities. This case highlights the critical importance of prompt recognition, timely diagnosis, and coordinated multidisciplinary

Maternal and Infant Outcomes in a Subset of Patients with Sickle Cell Disease in South Carolina.

OBJECTIVES: Sickle cell disease (SCD), which disproportionately affects minorities, increases complications during pregnancy. Severe maternal mortality is increased in women with SCD, including morbidity related to the disease and other nondisease-related complications. It also can have devastating complications for fetuses, with increases in premature birth and low birth weight. This study aimed to describe the characteristics of women with SCD in South Carolina, with a specific focus on fetal and maternal outcomes and complications. The secondary aim of this study was to identify the effect

Risk prediction models for ovarian hyperstimulation syndrome: a systematic review and meta-analysis.

BACKGROUND: Ovarian hyperstimulation syndrome (OHSS) is a serious complication of controlled ovarian stimulation (COS). The main clinical manifestation of OHSS is increased ovarian volume. OHSS can cause local and systemic tissue oedema, electrolyte disturbances, cardiorespiratory dysfunction, coagulation dysfunction, and other symptoms. These symptoms greatly affect patients' quality of life. As infertility rates rise and assisted reproductive technology (ART) becomes more common, the risk of OHSS increases. Therefore, early identification of high-risk patients and timely intervention are cru

Personalised risk-prediction tools for cryptococcal meningitis mortality to guide treatment stratification in sub-Saharan Africa: a prognostic modelling study based on pooled analysis of two randomised controlled trials.

BACKGROUND: Cryptococcal meningitis is a major driver of global HIV-related mortality, and validated approaches to stratify mortality risk could help to target effective treatment strategies. We aimed to develop and validate models to predict risk of all-cause mortality in people with HIV-associated cryptococcal meningitis in sub-Saharan African countries. METHODS: For this prediction modelling study, we pooled individual-level data from the ACTA (ISRCTN45035509) and AMBITION-cm (ISRCTN72509687) randomised controlled trials. Data in ACTA were collected between Feb 12, 2013, and Jan 10, 2017,
