The outcome of pregnancy following amniocentesis was studied prospectively in 517 consecutive patients undergoing amniocentesis in a single centre. The outcome in 289 of these pregnancies was compared with that in 289 control patients strictly matched for social class, age and parity. There were no significant differences in fetal loss, perinatal mortality or vaginal bleeding between the amniocentesis and control groups. Maternal serum alpha fetoprotein (MSAFP) measurement between 16 and 21 weeks gestation is used to define a group of women with an increased risk of fetal abnormality, particularly open neural tube defect. A bladder neck obstruction was suspected after ultrasound investigation at 16 weeks' gestation. Evaluation of protein content in the amniotic fluid, fetal ascites, and fluid from the overdistended bladder supported the diagnosis. Bladder outflow obstruction in the second trimester of pregnancy was not associated with raised alphafetoprotein levels in the amniotic fluid and maternal serum.

In the Kuopio and North-Karelia districts of Finland 10724 pregnancies were screened for congenital nephrosis by maternal serum alpha-fetoprotein (AFP) measurement. Outcome was known for 10504 (98%) pregnancies, of which 509 (4 X 8%) had a serum AFP level greater than or equal to 2 X 5 multiples of the normal median (MoM) at 15-18 weeks gestation. D2 is a glycoprotein existing in both membrane-bound and soluble forms. Employing a specific rabbit antibody against purified human brain D2, we developed an enzyme-linked immunosorbent assay (ELISA) for the quantification of D2 and applied it to amniotic fluids from 87 normal and 36 pathological pregnancies. Maternal serum alpha-fetoprotein (AFP) was measured during the second trimester in 219 twin pregnancies uncomplicated by neural-tube defects (NTD) and in 11 twin pregnancies discordant for NTD. An analysis has been made of indications for amniocentesis in the Edinburgh area from 1979 to 1981. About 5% of all mothers underwent the procedure. Among 2137 amniocenteses, 37% were performed on mothers 35 years old or more, and 30% on patients with raised serum alpha-fetoprotein. Five cases of abnormality in twin pregnancies, detected by ultrasound examination during the second trimester, are reported. Cytospin preparations of amniotic fluid samples from 200 pregnancies, taken between 16 and 20 weeks' gestation, were examined without knowledge of the fluid alpha-fetoprotein (AFP) level. In a retrospective study of low birth weight babies (less than 2.5 kg) born in one year it was found that statistically more maternal serum alpha-fetoprotein levels were above twice the median level than a control group of mothers producing larger babies (more than 3 kg). However, from the results the test cannot be accepted as being a prospective indicator or as a screening test of low birth weight in pregnant women.
