Traditional second trimester prenatal diagnosis (PD) by amniocentesis was introduced about 20 years ago. It is still the most frequently used procedure for this purpose. About 85% of all requests for PD are based on maternal age alone. The commonest indication is a maternal age of 35 or higher at delivery. Attempts are being made to improve utilization of the limited laboratory capacity by evaluating the indication for PD by combining age and other indicators, ie. maternal-serum-alpha-fetoprotein and unconjugated estriol. Chorionic villus sampling so far has appealed to an additional group of

After undergoing amniocentesis and delivering their child 504 women were asked by questionnaire for their attitude towards and their experience of pregnancy and amniocentesis. The majority of the women reported that they experienced their decision to utilise amniocentesis as largely unproblematic. In most cases they had already come to a decision before genetic counselling resulting from talking things over with their partner and their gynaecologist. The decision was mostly based on risk assessment and less by considerations of potential consequences of the operation. On the other hand, nearly

Seventeen centres from Australia, Britain, France, and the United States collaborated in a study to compare amniotic fluid acetylcholinesterase (AChE) determination by gel electrophoresis and amniotic fluid alpha-fetoprotein (AFP) measurement as diagnostic tests for open neural tube defects. The study was based on 32,642 women with singleton pregnancies (including 428 with open spina bifida and 238 with anencephaly) who had an amniocentesis at 13-24 weeks' gestation. The AChE test yielded a detection rate for open spina bifida of 99 per cent (95 per cent confidence interval 98-100 per cent), 9

We have established reference ranges for three microvillar intestinal enzymes--alkaline phosphatase (EC 3.1.3.1), gamma-glutamyltransferase (EC 2.3.2.1), and leucine aminopeptidase (EC 3.4.1.1)--measured in amniotic fluid in a reference population of 1875 women presenting for routine amniocentesis. These data were derived for use in prenatal diagnostic studies in a population at risk (1:4) for cystic fibrosis. False-positive or indeterminate results were noted for fewer than 3.5% of all low-risk cases for each enzyme evaluated. Total alkaline phosphatase and its isoenzymes and leucine amino-pe

This article has detailed the common prenatal sonographic findings that have been detected in fetuses with Down syndrome, trisomy 13, trisomy 18, Turner syndrome, and triploidy. It should be noted that not all fetuses with these five chromosomal abnormalities will have sonographically detectable malformations and that some may not be easily recognizable even at birth. It is crucial that the approach to the patient with a sonographically detected fetal malformation be thorough, systematic, and expeditious. The first step is to attempt to accurately define the abnormality. Next, syndromes with w

The first born offspring of first-cousin parents was affected with a keratinization disorder thought to be nonbullous congenital ichthyosiform erythroderma (CIE). In each of three subsequent pregnancies, the parents elected to have prenatal diagnosis based on evaluation of fetal skin biopsies. The epidermis of fetus 1 was identical to normal 21-wk estimated gestation age (EGA) fetal epidermis, but because keratinization begins normally around 24 wk EGA, the procedure was repeated 4 wk later. A thin epidermis with a few layers of stratum corneum indicated a normal fetus and a healthy infant was

Distinguishing between balanced and unbalanced chromosome complements segregating from parental rearrangements may be difficult using only classical cytogenetic techniques if banding morphology is similar under both expectations. In these situations, supplementing cytogenetic analysis with molecular genetic techniques and flow cytometry may provide increased diagnostic accuracy. To illustrate this, we present a case in which similar band pattern morphology would be expected for both the balanced carrier (heterozygote) and the recombinant dup q chromosome complements segregating from a mother w

Twenty-two selective terminations in multiple gestations were performed by a number of different methods. In 17 dichorionic pregnancies there was a successful delivery in surviving singletons or twins. In five monochorionic pregnancies undergoing selective termination there was a successful delivery in only one and a pregnancy loss in the other four. Six of the 18 delivered pregnancies were complicated by premature labor and delivery. Among the several methods used for selective termination, intracardiac potassium chloride injection appears to be the procedure of choice in dichorionic pregnanc

Alarm over the prospect that prenatal diagnostic techniques, which permit identification of fetal sex and facilitate abortion of healthy but unwanted female fetuses has led some to urge their outright prohibition. This article argues against that response. Prenatal diagnosis permits timely action to preserve and enhance the life and health of fetuses otherwise endangered, and, by offering assurance of fetal normality, may often encourage continuation of pregnancies otherwise vulnerable to termination. Further, conditions in some societies may sometimes render excusable the inclination to abort

Between 1978 and 1984 we performed 3 122 amniocentesis for prenatal diagnosis in second trimester. Based on a four week follow-up in more than 96% of all amniocentesis the percentage of fetal loss was 0.74% (n = 23). In 13 cases abortion was accompanied by chorioamnionitis. These cases were examined for various technical details and it could be concluded, that the incidence of febrile abortion is related to the early gestational age for amniocentesis. On the other hand, multiple insertions or the localisation of placenta both show no relation to the short-time febrile complications. To investi

Medical advances in obstetrics and gynecology, combined with the depersonalization of health care and changes in sexual and reproductive behaviors in the general population, lead to challenging new problems for the psychiatric consultant who deals with reproductive issues. The author describes contemporary issues encountered in consultation-liaison work with the obstetrics and gynecology service in a general hospital. They include the reproductive implications of medical illnesses, the stress of deciding whether to continue a pregnancy known to carry genetic risk, the intrusiveness of certain

Amniocentesis was performed under ultrasound control on 501 third-trimester gravidae out of the 509 referred. Eight patients were excluded based on the ultrasound examination, since practically no amniotic fluid could be detected. Ultrasound was used to find a pocket of amniotic fluid in which the puncture would carry no risk to the foetus. The puncture was made immediately after the examination, on the same examination table. In 17% of cases the puncture was made in the lower uterine segment behind the foetal neck. A diagnostic sample of the amniotic fluid was obtained in 87.4% of cases. In t

The aim of the study was to investigate the rationale of the current indications for fetal chromosome analysis. 5372 women had 5423 amniocentesis performed, this group constituting a consecutive sample at the chromosome laboratory, Rigshospitalet, Copenhagen from March 1973 to September 1980 (Group A + B). Pregnant women 35 years of age, women who previously had a chromosomally abnormal child, families with translocation carriers or other heritable chromosomal disease, families where the father was 50 years or more and women in families with a history of Down's syndrome (group A), were compare

Twenty-two twin pregnancies were subjected to second-trimester genetic amniocentesis. Arguments for and against tapping both sacs were discussed. Nineteen of the 22 patients studied underwent amniocentesis because of advanced maternal age (35 years or greater). The remaining indications included previous omphalocele, previous Down's syndrome, and previous neural tube defect. Except for one instance, amniotic fluid was obtained from both sacs. There were no other adverse sequelae other than transient leakage of fluid in one case. Two sets of twins were stillborn; in neither case was there a tem

Early second trimester amniotic fluid is classified by stepwise discriminant analysis involving the computer program BMDP77, based upon biochemical analyses for alpha-fetoprotein and eight other easily assayed variables. The outcome of about 800 pregnancies has been predicted on this basis, over 80 of which were associated with fetal neural tube defects. Classification is shown to be more reliable than using alpha-fetoprotein and gestational age alone. Although in practice this approach has now been superceded by the introduction of the analysis of acetylcholine-esterase isoenzyme activity in

A case of prenatally diagnosed thanatophoric dysplasia is presented. Other syndromes accompanied by chondrodysplastic tetramicromelia were excluded and diagnosis was based on the narrow thorax, secondary pulmonary hypoplasia and macrocephaly detected by ultrasound and on radiological findings of disturbances of bone formation. At the mother's request labour was induced and radiological, anatomic and histological examination of the newborn confirmed the prenatal diagnosis. By electron microscopy, large conspicuous, homogeneous intracytoplasmic, usually single vacuoles were found in virtually ev

There was a decrease in the recorded number of cases and in the incidence rate of Down's syndrome in Hawaii between 1963-1969 and 1971-1977. Independent of all other factors, induced abortion accounted for 43 percent of the decline in the number of cases, based on the assumption that a substantial number of clandestine abortions were being performed in Hawaii before the 1970 legalization of abortion. However, if we assume that very few illegal abortions were performed prior to 1970, there would have been an actual 3.5 percent increase in the number of cases of Down's syndrome in the absence of

In a study of Georgia women aged 40 years and older, 15% made use of prenatal chromosomal diagnosis. There was, however, substantial racial and geographic variation, ranging from a use ratio of 60% among whites in two large urban counties to 0.5% among blacks outside Augusta and Atlanta health districts. This simple population-based epidemiologic analysis suggests that future program planning for genetic services in Georgia should address ways to increase access by rural women, especially blacks. Similar analyses in other states could be used for planning genetic services.

Prenatal examination is indicated when there is a risk of hereditary disease causing alterations in the amnion cells, the amnion fluid, in the blood or the morphology of the fetus. Routine screening is performed in four groups: suspected chromosomalaberration, x-chromosomal heredopathy, metabolic defect or neural tube defect. Dermatologists have special interest in the second and third groups (Fabry disease, Xeroderma pigmentosum and recently Ichthyosis vulgaris). An interruption was proposed in 2,9% of all examinations performed up to now. On the other hand, most of pregnant women whose child
