Relationship between aortic isthmus and ductus venosus velocity waveforms in severe growth restricted fetuses.

OBJECTIVE: To evaluate the incidence of abnormalities in velocity waveforms from ductus venosus (DV) and aortic isthmus (AoI) in fetuses with intrauterine growth restriction (IUGR) and their reciprocal temporal relationship. METHODS: DV and AoI velocity waveforms were recorded in 31 IUGR fetuses characterized either by absent end diastolic (20) or reverse flow (11) in umbilical artery. Abnormal velocity waveforms in DV and AoI were defined in presence of reverse diastolic flows. RESULTS: Abnormal DV velocity waveforms were present in 10 fetuses and were always associated with abnormalities.

Individualized growth charts for ultrasound measurements can significantly improve fetal monitoring.

OBJECTIVE: For monitoring pregnancies it is useful to reduce unnecessary examinations due to wrongfully assumed growth retardation in cases with a small fetal growth potential. It also makes sense to improve the detection of objectively retardated children in order to a disproportionately high growth potential. It was the aim of this study to modify the standard growth charts for ultrasound parameters according to parental influences to better display the individual growth potential. MATERIAL AND METHODS: In a prospective trial 1,390 ultrasound measurements of 174 completed pregnancies were o

[Foetal growth retardation].

BACKGROUND: Intrauterine growth : restriction (IUGR) occurs in 3-10% of all pregnancies : The condition has different adverse effects on the foetus, during childhood and even in adult life. MATERIAL AND METHODS: Literature was retrieved from the Pub Med and Cochrane databases. RESULTS AND INTERPRETATION: The most common limit for IUGR and severe growth restriction is a neonatal weight < 10th percentile. Placental failure, infections or foetal anomalies may cause IUGR before the 32 nd gestational week (early IUGR). Chromosome abnormalities may be the cause of up to 20% of all growth-restricte

[Screening and timing for the delivery of fetuses with intrauterine growth restriction of vascular origin].

The prenatal management of fetuses with intrauterine growth restriction (IUGR) has been a matter of perpetual evolving in obstetrics and has been the topic of numerous studies. The management of IUGR fetuses is currently available in tertiary centres using fetal heart monitoring, short term variability, or doppler investigation of the umbilical artery, ductus venosus and middle cerebral artery. The aim of the prenatal management is to detect anomalies that may predict severe fetal hypoxemia. On the other hand, any efforts should focus on the possibility to delay the time for delivery as gestat

Only two-phase models, computed independently for males and females, are appropriate to describe fetal head growth.

OBJECTIVE: To establish an accurate mathematical model describing fetal head growth, taking into account gender differences and changes in growth rate during gestation. METHODS: Ultrasound measurements of head circumference and biparietal diameter were made on 1,336 normal fetuses (684 males and 652 females) in the Maternité Régionale de Nancy (France). A new two-phase model, taking into account an alteration in growth kinetics at 30 gestational weeks, was computed independently for male and female data. The accuracy of this model was tested and compared with three current mathematical models

Characterization of growth-restricted fetuses with breakdown of the brain-sparing effect diagnosed by spectral Doppler.

OBJECTIVE: To characterize serial findings of the middle cerebral artery (MCA) and umbilical artery (UA) flow patterns, their relationship to each other, and neonatal outcomes in growth-restricted fetuses. METHODS: Serial pulsatility indices (PIs) from MCA and UA Doppler waveforms were measured in 41 growth-restricted fetuses until Cesarean delivery. We found three patterns, as follows: phase 1 (n = 27), UA PI < MCA PI (no brain-sparing effect); phase 2 (n = 11), UA PI > MCA PI (brain-sparing effect); phase 3 (n = 3), both PIs elevated with the absence of end-diastolic flow or presence of rev

Sonographic diagnosis of intrauterine growth restriction.

Estimation of fetal weight in utero using multiple ultrasonic parameters remains the mainstay in screening for IUGR. The use of various fetal morphometric ratios and/or measurements of other fetal parameters may provide additional useful information. Serial evaluation to assess interval growth may be necessary to clarify the diagnosis. The use of Doppler ultrasound, especially the evaluation of the umbilical artery and middle cerebral artery velocity flow, is an important adjunct for both the diagnosis of IUGR caused by uteroplacental insufficiency and its continued management.

Characterisation of the possible effect on birthweight following frequent prenatal ultrasound examinations.

The objective of this study was to evaluate and characterise by study of newborn biometry a possible effect on birthweight which we observed previously in a randomised controlled trial of multiple prenatal ultrasound examinations. A total of 2743 women with single pregnancies had been allocated at random to either a protocol of ultrasound imaging and continuous wave Doppler studies at 18, 24, 28, 34 and 38 weeks gestation (intensive group), or to a protocol of a single imaging examination at 18 weeks and further imaging scans only as clinically indicated (regular group). When compared with tho

[Blood circulation in retarded and normal offspring of hypertonic pregnant women].

Doppler velocimetry was carried out on 62 women with hypertensive disorders in pregnancy. From this group, 19 infants with intrauterine growth retardation (IUGR, defined as gestational age related birth weight of < 10.percentile) were born. The rest 43 were appropriate for gestational age (AGA, > 10. percentiled). Blood flow velocity studies were performed in maternal uterine vessels, and fetal umbilical, cerebral and renal arteries. Patients with IUGR exhibited a not always significant increase of indices in uterine, umbilical and renal arteries, and a non significant decrease in middle cereb

Can placental surface area and neonatal weight be predicted from placental surface measurements? Preliminary observations on normal-term pregnancies.

OBJECTIVE: To evaluate the relations between chorionic plate measurements and neonatal weight as prerequisites for possible prediction of fetal weight using antenatal placental measurements. METHODS: We examined freshly delivered placentas. The surface area (chorionic plate) was measured using millimeter paper, and the two longest diameters (at right angles to each other) were determined (L1 and L2). The mean longest diameter [Lm = (L1 + L2)/2] was calculated. The relation of these diameters to neonatal and placental weight was studied. RESULTS: We examined 57 randomly selected placentas aft

Evaluation of twin growth status at birth using individualized growth assessment: comparison with conventional methods.

OBJECTIVE: We sought to evaluate the growth status of twins at birth using individualized growth assessment methods and to compare this assessment with that obtained with conventional methods. STUDY DESIGN: Twenty twin pregnancies were studied longitudinally with ultrasound. Measurements of the head and abdominal cubes (A,B), head circumference (HC), abdominal circumference (AC), thigh circumference (ThC), and femur diaphysis length (FDL) made in the 2nd trimester were used to specify Rossavik growth models for each parameter in each fetus. These models were used to predict weight (WT), HC, A

Low dose aspirin for the treatment of fetal growth restriction: a randomized controlled trial.

The purpose of this study was to investigate the hypothesis that maternal administration of 100mg aspirin each day will improve birth-weight and other measures of neonatal size when given as a treatment to pregnancies complicated by fetal growth restriction and umbilical-placental insufficiency. A randomized, double-blind, placebo controlled study design was employed; 51 pregnant women were enrolled. The entry criteria were a fetal abdominal circumference < 10th per centile together with an umbilical artery Doppler systolic/diastolic ratio > 95th per centile between 28 and 36 weeks' gestation.

[Maternal-fetal circulation in premature labor].

Color and pulsed wave Doppler ultrasonographic studies were performed of the maternal uterine, umbilical, and fetal middle cerebral and renal arteries in 70 singleton preterm pregnancies, within 24 hours of delivery. There were 29 fetuses that had a small weight for gestational age (SGA) at the time of delivery. From these 25 fetuses were small as a result of placental insufficiency. Results of these 25 were compared with a group of 41 fetuses that had appropriate weight for gestational age (AGA). The mean index values of blood flow velocity in the uterine, umbilical, and renal arteries in the

An analysis of birth weight by gestational age using a computerized perinatal data base, 1975-1992.

OBJECTIVE: To develop birth weight-for-gestational age nomograms based on a computerized perinatal data base collected prospectively from 1975-1992. METHODS: Using information from over 60,000 singleton deliveries (January 1975 through October 1992) at the MetroHealth Medical Center in Cleveland, Ohio, standard curves for normal birth weights were computed. Nomograms were developed for the overall population and for subgroups determined by factors known to affect fetal growth, including sex, race, smoking status, and gestational diabetes. The nomograms included the tenth, 50th, and 90th perce

[Systematic antepartum fetal evaluation ("Safe"). A concept for diagnosis of fetal function in threatened hypoxia].

TARGET: The significant aim of the study was to prove whether it makes sense to apply the classical approach of diagnostic procedures to the fetus. PATIENTS AND METHODS: In a group of a total of 534 singleton pregnancies we examined the value of various biochemical and biophysical tests for detecting the risk of perinatal hypoxia. The sequence and combination of the most decisive tests was considered as well as the different approach within the observation period. RESULTS: Maternal laboratory findings like HPL and E3 were significantly less predictive of growth disturbance and compromise tha
