Perinatal morbidity and mortality are still high in cases of intra-uterine growth retardation. Present screening methods select large risk groups and have a low positive predictive value. Instruments which could be used as indicators for those cases within the risk group needing close fetal monitoring would be valuable. The aim of this prospective study was to evaluate fetal ponderal index with respect to signs of fetal distress and neonatal outcome. By means of a risk scoring system, 73 pregnancies with increased risks of intra-uterine growth retardation were compared with 61 controls. By mea

Many sonographic methods have been suggested for identification of the fetus with a growth abnormality. Clearly, optimal management depends on early diagnosis. In recent years, advances in sonography have improved the ability to identify abnormal growth patterns and evaluate fetal well-being. When abnormal fetal growth is suspected, a thorough sonographic evaluation should be performed. This evaluation includes measurements of the abdominal circumference, femur length,) BPD, HC, AC, FL, and amniotic fluid volume. If these measurements confirm the suspicion of abnormal fetal growth, careful sea

Intrauterine growth retardation is associated with an increased risk of fetal asphyxia as well as greater perinatal morbidity and mortality. Ultrasound fetometry enables detection of fetuses that are small for gestational age. Doppler velocimetry of the umbilical artery has good predictive ability for fetal distress, but it is not yet clear whether it could replace cardiotocography in antenatal surveillance of small-for-gestational-age fetuses. We have done a randomised comparison of the two methods. At four obstetric departments in Sweden, women with fetuses found to be small on ultrasound ex

To assess the usefulness of continuous wave Doppler ultrasonography as an antenatal screening tool for the detection of intrauterine growth retardation and fetal compromise 2097 singleton pregnancies were studied. Umbilical artery velocity waveforms were obtained at 28, 34, and 38 weeks of gestation, from which the pulsatility index, A/B ratio, and resistance parameter were calculated. No abnormal features or indices of neonatal outcome were adequately predicted. The most sensitive index for being delivered of a growth retarded infant (less than 5th centile birth weight for gestation) was an A

Doppler pulses or continuous flow in the umbilical artery is a technique recently used in practical obstetrics and of value particularly in screening at risk pregnancies. The authors wish to study its value as a diagnostic and prognostic tool in the special cases where ultrasound has given rise to a suggestion that there may be intra-uterine growth retardation (I.U.G.R.). In a preliminary publication they report their experience in 100 normal pregnancies so that they have obtained a control diagram. Then, working on a population of 117 pregnancies with ultrasonically diagnosed I.U.G.R. they st

Normal fetal growth is a logarithmic process, marked by rapid mitosis at its early stages and by cellular hypertrophy and the accumulation of fat, glycogen, and connective tissue later in gestation. Growth-retarding influences can alter cell number, with a symmetric pattern of IUGR resulting if they occur early. Later insults, the result of uteroplacental compromise, affect cell size and may cause an asymmetric growth retardation. Fetuses with asymmetric growth retardation are at particular risk for intrauterine fetal demise and fetal distress in labor. The assessment of fetal growth is compli

The diagnostic efficiency in the prediction of intrauterine growth retardation (IUGR) of repeated measurements of the symphysis-fundus (SF) distance and repeated ultrasonic measurements of the biparietal diameter (BPD) was investigated in 377 pregnancies, all at risk for IUGR. Measurements of the SF distance were found to be more effective than ultrasonic BPD measurements for antenatal diagnosis of IUGR. For every correct diagnosis there were three false positive when using SF measurements and ten when using ultrasonic BPD measurements. When the SF method is used, repeated ultrasonic BPD measu

This paper presented the results of ultrasonic measurements and non-stress tests (NST) in 60 light-for-dates (LFD) fetuses. BPD, FFL, mean abdominal diameter (MAD), HC/AC ratio and EFW were used as the parameters and the rates of LFD detection were evaluated. The effects of antenatal therapy for IUGR were assessed by the analyses of normal deviate values of EFW in treated and untreated groups. Qualitative and quantitative analyses of NST findings were presented. As to the detection of LFD, sensitivity values for MAD and EFW were 66.7% and 53.8% respectively; meanwhile both parameters disclosed

As methods for identifying the small-for-dates fetus in twin pregnancies, serial ultrasonic cephalometry and a new two-stage ultrasound examination schedule have been evaluated and compared. Serial cephalometry is associated with technical and interpretative problems, and only 24 (56 per cent) of 43 small-for-dates fetuses in 66 retrospectively studied twin pregnancies showed detectably abnormal biparietal diameter growth. The two-stage schedule comprises an accurate assessment of gestational age in early pregnancy, and determination of the product of the crown-rump length and trunk area at 34

The aim of the study was to evaluate the use of ultrasound biparietal diameter (BPD) in the timing of elective repeat cesarean section on a large indigent service. One hundred sixty-five patients were evaluated over 2 years. If the BPD was 9.3 cm or more at or later than 38 weeks' gestation, a repeat procedure was undertaken without further testing of fetal maturity. This occurred in 55% of the study group. None of these neonates developed hyaline membrane disease (HMD). The remainder of the study group either did not attain a BPD of 9.3 cm by term, were diabetic, or had other factors to deter
